Significant challenges remain, however, in terms of improving equity with continuing low rates of enrollment. Ensuring financial protection also remains an elusive goal. The Master Plan for Universal Coverage approved in 2012 by the Prime Minister directly addresses both these deficiencies in coverage. The objective of this report is to assess the implementation of Vietnam SHI and provide options for moving towards UC. This is a joint assessment with development partners, World Health Organization, United Nations Children's Fund (Unicef) and Rockefeller Foundation. Expanding breadth of coverage, particularly for those hard to reach groups such as the near-poor and informal sector would require substantially increasing general revenue subsidies and fully subsidizing the premiums for the near-poor. High enrollment rates would, however, have little impact on financial protection and equity if OOP costs remain high. Achieving UC will require sustained efforts to improve efficiency in the system, and gain better value for money from available budgetary resources; without these efforts, any further progress towards UC would be financially unsustainable. There is considerable scope for improving efficiency in Vietnam. Fragmentation in the pooling of funds gives rise to unnecessary costs. Inefficiencies in resource allocation and purchasing arrangements include: (i) an overly generous benefits package; (ii) provider payment mechanisms and the mix of incentives facing providers which result in an oversupply of services; (iii) high prices, overconsumption and inappropriate use of pharmaceuticals; and (iv) the structure and incentives embedded within the delivery system. The organization, management and governance of SHI are fragmented and often dysfunctional. The present institutional setting for SHI needs to be assessed and changed.

Increasing temperatures, sea-level rises, changes in precipitation patterns and extreme events are expected to increase a range of health risks, from the direct effects of heat-waves, floods and storms, to more suitable conditions for the transmission of important infectious diseases, to impacts on the natural systems and socioeconomic sectors that ultimately underpin human health. Much of the potential health impact of climate change can, however, be avoided through a combination of strengthening key health system functions and improved management of the risks presented by a changing climate. The critical first step in this process is to carry out a vulnerability and adaptation assessment. This allows countries to assess which populations are most vulnerable to different kinds of health effects, to identify weaknesses in the systems that should protect them, and to specify interventions to respond. Assessments can also improve evidence and understanding of the linkages between climate and health within the assessment area, serve as a baseline analysis against which changes in disease risk and protective measures can be monitored, provide the opportunity for building capacity, and strengthen the case for investment in health protection. WHO has responded to this global demand by building on past guidance and technical tools to outline a flexible process for vulnerability and adaptation assessment. In 2009, the WHO Regional Office for the Americas and WHO prepared draft guidance for this process, which was pilot-tested in studies across all WHO Regions. In July 2010, representatives of ministries of health from 15 countries came together in Costa Rica with WHO and subject area experts to share their experiences and provide feedback on how to improve the guidance for the conduct of vulnerability assessments. This document is the result of this process. It is intended not as a final, definitive guide but as an important part of an evolving set of resources that will support effective and evidence-based action to protect health from climate change.

User fees charges, however, may not be the best approach or the most equitable approach to finance and/or reform health services in developing nations. This study investigates the status of Vietnam's current health system as a result of implementing user fees policies. A recent mandate by the government to increase the universal cover to 100% attempts to tackle inadequate insurance cover, one of the four major factors contributing to the high and increasing probability of destitution for Vietnamese patients (the other three being: non-residency, long stay in a hospital, and a high cost of treatment). Empirical results, however, suggest that this may be catastrophic for low-income earners: if insurance cover reimbursement decreases below 50% of actual health expenditures, the probability of Vietnamese falling into destitution will rise further. Our findings provide policy implications and directions to improve Vietnam's health care system, in particular by ensuring the utilization of health services and financial protection for the people.

Book excerpt: Vietnam's Health Care Fund for the Poor (HCFP) uses government revenues to finance health care for the poor, ethnic minorities living in selected mountainous provinces designated as difficult, and all households living in communes officially designated as highly disadvantaged. The program, which started in 2003, did not as of 2004 include all these groups, but those who were included (about 15 percent of the population) were disproportionately poor. Estimates of the program's impact-obtained using single differences and propensity score matching on a trimmed sample-suggest that HCFP has substantially increased service utilization, especially in-patient care, and has reduced the risk of catastrophic spending. It has not, however, reduced average out-of-pocket spending, and appears to have had negligible impacts on utilization among the poorest decile.

Book excerpt: Over the course of several weeks in June and July of 2014, Rubicon Strategy Group conducted close to forty interviews with country managers of pharmaceutical and device companies, director level administrators in public and private hospitals, successful pharmacy and medical device shop owners, and entrepreneurs at the forefront of the cutting edge in the Vietnam healthcare market. In addition the team went out and conducted a validated survey questionnaire of consumers to judge their preference for particular products, and their preference for healthcare access options in the public and the private sector. The goal of this paper revealed itself in the course of compiling the data: to bring out insights from the front lines of the sales channels and the business models that make up the pharmaceutical and medical device markets in Vietnam. It is the hope of the authors that the information presented in this way can help inform sales strategies and the development of value add services for companies involved in the marketing and/or distribution of drug and medical device products in Vietnam. On the eve of a pair of large negotiations, - a free trade agreement negotiation between the EU and Vietnam, set to be finalized in October 2014, and the negotiation of the Trans Pacific Asean partnership – it was found that much of the conversation with healthcare system company representatives and sales channel participants turned to the issue of the public tender system as well as some of the hardships brought on by Vietnam’s ongoing healthcare market reform. Consequently, one section of this report is geared towards exploring how the policy and regulatory level challenges of the current tender process. However, in constructing this section it became clear that the value of the research conducted during this study is not simply in explaining the tender process as it is supposed to function at the policy level and the attendant issues that stem from that design, but also highlighting how the tender process impacts the decision making of pharmaceutical and medical device executives in-country, in real time, as well as how it impacts the various operators across sales channels. In other words, a core value of the study is necessarily attendant to its exploration of the strategies currently being employed by executives active in Vietnam’s healthcare market. In trying to present a picture of the ground-level impacts of policies and regulatory structures impacting Vietnam’s healthcare space, it is of course important to present a clear outline of the issues that the research revealed. At the same time, it became apparent to fully communicate the ground-level happenings as they related to the tender process, it was also helpful to present a series of case studies that would help add color and nuance to the issues clearly presented.

Book excerpt: The health sector reform programme which began in Vietnam in 1989 in order to improve the efficiency of the health system has altered the way in which Vietnamese hospitals operate. The programme put the spotlight on input savings. This study aims to examine the relative efficiency of hospitals during the health reform process and assess - by looking at the relative efficiency of hospitals - the effects of the regulatory changes. The study employs the DEA two-stage approach referring to data from 101 general public hospitals over the period 1998-2006. The study revealed that there was evidence of improvement in the productivity of Vietnamese hospitals over the period 1998-2006, with a progress in total factor productivity of 1.4% per year. Furthermore, the differences in hospital efficiency can be attributed to both the regulatory changes and hospital-specific characteristics. The user fees and autonomy measures were found to increase technical efficiency. Provincial hospitals were revealed to be more technically efficient than their central counterparts and hospitals located in the North East, South East and Mekong River Delta regions performed better that hospitals from other regions.

Book excerpt: “My husband doesn’t have a head for business,” complained Ngoc, the owner of a children’s clothing stall in Ben Thanh market. “Naturally, it’s because he’s a man.” When the women who sell in Ho Chi Minh City’s iconic marketplace speak, their language suggests that activity in the market is shaped by timeless, essential truths: Vietnamese women are naturally adept at buying and selling, while men are not; Vietnamese prefer to do business with family members or through social contacts; stallholders are by nature superstitious; marketplace trading is by definition a small-scale enterprise. Essential Trade looks through the façade of these “timeless truths” and finds active participants in a political economy of appearances: traders’ words and actions conform to stereotypes of themselves as poor, weak women in order to clinch sales, manage creditors, and protect themselves from accusations of being greedy, corrupt, or “bourgeois” – even as they quietly slip into southern Vietnam’s growing middle class. But Leshkowich argues that we should not dismiss the traders’ self-disparaging words simply because of their essentialist logic. In Ben Thanh market, performing certain styles of femininity, kinship relations, social networks, spirituality, and class allowed traders to portray themselves as particular kinds of people who had the capacity to act in volatile political and economic circumstances. When so much seems to be changing, a claim that certain things or people are inherently or naturally a particular way can be both personally meaningful and strategically advantageous. Based on ethnographic fieldwork and life history interviewing conducted over nearly two decades, Essential Trade explores how women cloth and clothing traders like Ngoc have plied their wares through four decades of political and economic transformation: civil war, postwar economic restructuring, socialist cooperativization, and the frenetic competition of market socialism. With close attention to daily activities and life narratives, this groundbreaking work of critical feminist economic anthropology combines theoretical insight, vivid ethnography, and moving personal stories to illuminate how the interaction between gender and class has shaped people’s lives and created market socialist political economy. Book excerpt: Abstract: By international standards, and given its relatively low per capita income, Vietnam has achieved substantial reductions in, and low levels of, infant and under-five mortality. Wagstaff and Nguyen review existing evidence and provide new evidence on whether, under the economic liberalization program known as Doi Moi, this reduction in child mortality has been sustained. They conclude that it has, but that the gains have been concentrated among the better-off. As a result, socioeconomic inequalities in child survival are evident in Vietnam"a change from the early 1990s when none were apparent. The authors develop survival models to find the causes of this differential decline in child mortality, and conclude that a number of factors have been at work, including reductions among the poor (but not among the better-off) in coverage of health services and in women's educational attainment. They argue that if the experience of the late 1990s is a guide to the future, the lack of progress among the poor will jeopardize Vietnam's chances of achieving the international development goals for child mortality. The authors examine various policy scenarios, including expanding coverage of health services, water and sanitation, and find that such measures, while useful, will have only a limited effect on the mortality of poor children. They find that programs aimed at narrowing the gap between the poor and better-off may have large beneficial effects on the various determinants of child survival.
