Available in PDF, EPUB and Kindle. Book excerpt: Sudden cardiac death and ventricular arrhythmia play a prominent role in mortality in our era. One of the biggest milestones in the therapy of ventricular arrhythmias was the invention of cardiac defibrillation. There were several important developments in the last decades, making nowadays automated external and internal defibrillators widely available. However, the rapid evolution and high differentiation of available options presents a challenge to be kept "up-to-date". With this book, we would like to review the actual guidelines and give practical advices concerning of indications in cardiomyopathy patients, possible contraindications and complications, the perioperative management including anticoagulation and antibiotics, and the programming and follow-up of defibrillator devices.

Book excerpt: This compendium gives a comprehensive overview of the advances in fibrillation-defibrillation knowledge ? recognition of fibrillation as a unique life threatening cardiac arrhythmia; discovery of the electric discharge in its double role of culprit and savior; and technological improved contributions. The book stands on the well-known philosophy of Education-Based on Problems (or EBP), that is, take fibrillation as a medical daily problem and search for that knowledge, technique or principle trying to solve it.The book is interdisciplinary, multidisciplinary and transdisciplinary. It addresses undergraduate and graduate biomedical engineering students, physicians going into cardiology, clinical engineers and clinical engineering technicians, nurses, paramedics and emergency medical personnel.

Book excerpt: Implantable defibrillators as originally conceived by Michel Mirowski were limited to the detection and automatic termination of ventricular fibrillation. In the original "AID" device, the detection algoritlun sought to distinguish sinus rhytlun from ventricular fibrillation by identifying the "more sinusoidal waveform of ventricular fibrillation. " The therapeutic intervention was elicited only once deadly polymorphic rhythms had developed. It was rapidly learned, however, that ventricular fibrillation is usually preceded by ventricular tachycardia. Mirowski recognized the pivotal importance of developing algoritllms based on heart rate. Ventricular tachycardia detection allowed the successful development of interventions for the termination of ventricular tachyarrhythmias before they degenerated into ventricular fibrillation. Current device therapy no longer confines itself to tlle termination of chaotic rhythms but seeks to prevent them. Diagnostic algorithms moved upward along the chain of events leading to catastrophic rhytlulls. Rate smoothing algorithms were developed to prevent postextrasystolic pauses from triggering ventricular and atrial tachyarrhytlmlias. Beyond the renaissance of ectopy-centered strategies, long-term prevention received increasing attention. Multisite pacing therapies provided by "Arrhythmia Management Devices" were designed to reduce the "arrhytlunia burden" and optimize the synergy of cardiac contraction and relaxation. Clinical evidence now suggests that atrial fibrillation prevention by pacing is feasible and tllat biventricular pacing may be of benefit in selected patients with heart failure. However, these applications of device therapy that generally require ventricular defibrillation backup remain investigational and were not considered in this book.

Book excerpt: One of the most time-consuming tasks in clinical medicine is seeking the opinions of specialist colleagues. There is a pressure not only to make referrals appropriate but also to summarize the case in the language of the specialist. This book explains basic physiologic and pathophysiologic mechanisms of cardiovascular disease in a straightforward manner, gives guidelines as to when referral is appropriate, and, uniquely, explains what the specialist is likely to do. It is ideal for any hospital doctor, generalist, or even senior medical student who may need a cardiology opinion, or for that ma.

Book excerpt: The publication embodied here represents the life work of a premier Russian scientist studying Sudden Cardiac Death. As one can gather from more than 35 first authored publications cited in the References, Dr. Rajskina has been involved with the investigation of mechanisms responsible for Sudden Cardiac Death for over 30 years. She has brought a classical approach to the sub ject, considering the effects of blood supply disturbances, electrophysiological changes that occur after regional ischemia, metabolic alterations, and the role of the autonomic nervous system in modulating these changes. These studies naturally lead to a consideration of interventions, based on her research, to prevent ventricular fibrillation after coronary artery occlusion. This is a wide ranging treatise indicative of a lifetime of study of the problem and filled with the richness of scientific experiments generated in its pursuit. There is so much in here that will be of interest to the arrhythmologist interested in Sudden Cardiac Death, whether this is on a single channel level, in vitro study of hearts, in vivo investigation of intact animals, or at the bedside. And throughout it all, statements are copiously documented with more than 850 references. That alone is worth hours of computer searching. I am very proud to have been asked by this outstanding scientist to write a brief Preface to her monumental contribution. All of us involved in the study of arrhythmic mechanisms responsible for Sudden Cardiac Death can hold Dr.

Book excerpt: Since the first use of basic life support (BLS) and defibrillation in the pre-hospital setting in Belfast in 1966, few would argue that there have been major improvements in the rate of survival for out-of-hospital cardiac arrests. Indeed, until the widespread introduction of BLS and defibrillators to emergency medical service (EMS) vehicles, an out-of-hospital cardiac arrest would mean certain death. The initial rhythm of a patient in cardiac arrest is predictive of their chances of survival. In this regard, the rhythms with the highest rate of survival to hospital discharge are ventricular fibrillation (VF) and ventricular tachycardia (VT). As VF is a more common presenting rhythm in out-of-hospital cardiac arrest and much of our knowledge of VF is relevant to VT, this book focuses primarily on VF cardiac arrest.

Book excerpt: Updates on Cardiac Defibrillation, Cardioversion and AED Development delves deeply into the multifaceted world of arrhythmia treatment. Through various approaches, the chapters show a wide array of electrical and pharmacological methods to restore normal sinus rhythm in tachyarrhythmic cases. From the historical perspective of the first use of direct current electrical cardioversion to the effective deployment of Automated External Defibrillators (AEDs) in public-access defibrillation programs, this book underscores the crucial role of timely intervention. Focusing on atrial arrhythmias, the impact of tachyarrhythmia on heart function is highlighted and potential breakthroughs of cardioversion drugs are introduced. In the context of hypertrophic obstructive cardiomyopathy, the book uncovers treatment strategies, emphasizing the innovative cardiac myosin modulation. The chapter on the effectors of transthoracic impedance and their significant influence on electrical intervention success rate urges personalized optimization methods for increased efficiency. Collectively, these chapters provide a comprehensive overview of various aspects of arrhythmia treatment, presenting advancements, challenges, and strategies to enhance patient outcomes in the dynamic landscape of acute cardiology care.
