Book excerpt: The revised management structure introduced as a result of the nhs management inquiry places responsibility for the systematic search for vfm squarely on general managers at all levels within the nhs. However, with this devolving of responsibility through the general management function down to unit level comes the need to establish and maintain effective lines of accountability. Health authorities and dhss need to ensure that the framework of accountability and re- view procedures now introduced continues to develop in such a way as to focus sharp attention on the achievement of vfm at local level and to prevent cuts in services being presented as real savings. The first round of cost improvement programmes has produced a significant step forward in the search for efficiency in the hospital service.

Available in PDF, EPUB and Kindle. Book excerpt: Policy-makers are increasingly trying to assign economic values to areas such as ecologies, the atmosphere, even human lives. These new values, assigned to areas previously considered outside of economic systems, often act to qualify, alter or replace former non-pecuniary values. Valuing Development, Environment and Conservation looks to explore the complex interdependencies, contradictions and trade-offs that can take place between economic values and the social, environmental, political and ethical systems that inform non-monetary valuation processes. Using rich empirical material, the book explores the processes of valuation, their components, calculative technologies, and outcomes in different social, ecological and conservation domains. The book gives reasons for why economic calculation tends to dominate in practice, but also presents new insights on how the disobedient materiality of things and the ingenuity of human and non-human agencies can combine and frustrate the dominant economic models within calculative processes. This book highlights the tension between, on the one hand, a dominant model that emphasises technical and ‘universalising’ criteria, and on the other hand, valuation practice in specific local contexts which is more likely to negotiate criteria that are plural, incommensurable and political. This book is perfect for researchers and students within development studies, environment, geography, politics, sociology and anthropology who are looking for new insights into how processes of valuation take place in the 21st century, and with what consequential outcomes.

Available in PDF, EPUB and Kindle. Book excerpt: While care coordination and quality of care remain paramount policy topics everywhere, countries again turn to payment and efficiency challenges. Issue 12 of Health Policy Developments examines how health systems are trying to maximize value for money - exploring new avenues and mixing incentives. Preceding the value question there is a much simpler question: Where does the money come from? Clearly, someone needs to shoulder the burden of higher healthcare costs, but who should pay for what? Should it be private health insurers or the pharmaceutical industry as is happening in France and Australia, the whole population as it is in Finland or Canada, or should people above a certain body mass index be taxed, as in Alabama? Further topics in this issue are governance in Bismarckian systems, responsiveness of health systems to vulnerable groups, access and equity, and patient safety and quality. The International Network Health Policy and Reform aims to narrow the gap between health services research and health policy. Network partners are research institutions and health policy experts from 20 industrialized countries.

Book excerpt: This review summarizes the new arrangements for the NHS proposed in the Health White Paper (Cm. 7881, ISBN 9780101788120). In that paper the Government proposed a number of reforms to the structure and operation of the NHS, which have since been refined and developed in 'Liberating the NHS: legislative framework and next steps' (Cm. 7993 ISBN 9780101799324). The Health and Social Care Bill is designed to create the necessary legislative change. The review's purpose is to inform the Public Accounts Committee so that it can take stock of the proposals as they currently stand and discuss their implementation with the Department of Health and NHS.

Book excerpt: Health Policy and the NHS provides a thorough and up-to-date review of the changes in the structure and organisation of the health service. It focuses on how sucessive governments have approached problems of health care, their policy assumptions and the economic and political context of their decision making. Divided into four parts the text considers in turn: the foundations and framework of the NHS, policy issues within the NHS that dominated the government's policy agenda until the late 1980s, health and society and the critiques of health policy which developed in the late 1970s and 1980s, and new directions for health policy in the future.

Book excerpt: This report is a follow-up to the Committee's report on Accountability for Public Money (HC 740, session 2010-11 (ISBN 9780215559029)) an issue at the core of the relationship between Parliament and government. Accounting Officers remain accountable to Parliament for funds voted to their departments but the policy intention is that local bodies will have significant discretion over the services they deliver. In the Government's response, 'Accountability: Adapting to Decentralisation', Sir Bob Kerslake drew a distinction between those services that government delivers directly and those that it may fund but are delivered in more decentralised arrangements. He proposed that Accounting Officers set out, in Accountability System Statements, the arrangements they have in place to provide assurance about the probity and value for money of funds spent through devolved systems. All departments are expected to produce Statements by summer 2012. Departments have made a genuine effort to develop arrangements which reconcile accountability and localism but the Statements so far are unwieldy and considerably more needs to be done to improve their clarity, consistency and completeness. There is concern that accountability frameworks must drive value for money and, critically, are sufficiently robust to address the operational or financial failure of service providers. Departments are placing increasing reliance on market mechanisms such as user choice to drive up performance and value for money, but there are limits to what these mechanisms can achieve. The Treasury needs to take ownership of the system and ensure that the Comptroller and Auditor General has the necessary powers and rights of access to examine the value for money of funds spent through devolved systems

Book excerpt: This joint report was prepared by the National Audit Office and the Audit Commission, and contains the findings from the NAO's audit of the NHS summarized accounts and the Audit Commission's appointed auditors' work on the 2003-04 accounts of individual NHS organizations. The report outlines the financial issues facing individual NHS organizations, with an overview of the effects of these issues at national level and how this will affect the national health economy. In 2003-04 the NHS spent a total of £63 billion, with expenditure costs in the NHS rising by 7.3 per cent each year. This will increase the costs to £76 billion for the 2005-06, and £93 billion for 2007-08 periods. Alongside this increasing expenditure, the Government has set out various reform plans, including the establishment of the NHS Foundation Trusts, new staffing contracts, the development of the information technology infrastructure, and the way hospitals are funded. In the Summary of the financial performance for 2003-04 period, the number of bodies failing to achieve a financial balance had increased, along with an increase in the number of bodies with significant financial deficits. In all, 106 NHS bodies failed to achieve an in-year financial balance, and 14% of the Primary Care Trusts failed to keep expenditure within their resource limit, also a small number of NHS bodies are struggling to manage large deficits. The report advocates four key themes for the improvement of financial management: the role of the Board - who should display better oversight and improve their financial acumen; forecasting - NHS bodies should continually test whether cost savings programmes are realistic, and take account more effectively for risk factors in their financial planning, as well as set realistic budgets at the beginning of the year; earlier preparation of accounts - improvements in financial reporting, and the provision of financial information throughout the year should closely reflect the standard and range of information required in the annual accounts; transparency - that boards, managers, stakeholders would benefit from clarity in the way the accounts are organized, and that the amount of financial support received by the trusts should be clearly stated. With the introduction of Payment by Results and the use of independent healthcare providers the income received by NHS Trusts is no longer certain. So overall improvements in their financial forecasting and modeling, with NHS Trusts in particular developing their commercial financial skills, would be beneficial especially if they intend to become foundation trusts.

Available in PDF, EPUB and Kindle. Book excerpt: The NHS came into existence in an atmosphere of conflict centred on the strong ideological commitment of the Post-war Labour Government and the opposition of the Conservative Party of that time to the idea of a universally available and centrally planned medical care service. There was also opposition from some sections of the medical establishment who feared the loss of professional autonomy. Setting health policy in both an historical and modern context (post 1997) Carrier and Kendall weigh up the successes and failures of the National Health Service and examine the conflicts which have continued for over sixty years, in spite of efforts to solve financial problems in the NHS through increases in funding as well as structural and organisational change. After looking at recent responses to supposed failures of the NHS, they conclude that the NHS has successfully faced the challenges before it and is likely to continue to meet the changing health needs of the population. Financial stresses, concerns about the quality of care and demographic change, with consequent issues for the elderly and the chronically ill, continue to be urgent and politically contentious issues. This book is appropriate for a wide range of undergraduate and postgraduate students studying health policy and the NHS.

Book excerpt: In this book the authors explore the state of the art on efficiency measurement in health systems and international experts offer insights into the pitfalls and potential associated with various measurement techniques. The authors show that: - The core idea of efficiency is easy to understand in principle - maximizing valued outputs relative to inputs, but is often difficult to make operational in real-life situations - There have been numerous advances in data collection and availability, as well as innovative methodological approaches that give valuable insights into how efficiently health care is delivered - Our simple analytical framework can facilitate the development and interpretation of efficiency indicators.
