Book excerpt: The Department of Veterans Affairs (VA) provides health care to about 2.6 million veterans each year, but veterans in different parts of the country traditionally have not had equal access to these services. A shift of the veteran population from the northeast and the midwest to the south and the west without appropriate reallocation of resources has created inequities in access to services. In April 1997, VA launched the veterans equitable resources allocation system as part of a strategy to improve the equity of veterans' access to health care. The system is designed to allocate resources to 22 regional VA health care networks, which are responsible for distributing resources to hospitals and clinics. This report assesses VA's (1) implementation of the veterans equitable resources allocation system, (2) monitoring of changes in health care delivery resulting from the system, and (3) oversight of the network allocation process used to give veterans equitable access to service.

Book excerpt: Pursuant to a congressional request, GAO reviewed the Department of Veterans Affairs' (VA) resource allocation system, focusing on the: (1) extent to which VA resources are distributed equally among VA facilities; and (2) causes of unequal resource allocations among VA health care facilities. GAO found that: (1) the VA resource allocation system enables VA to identify potential inequities in resource allocations and forecast facility workload changes, but VA has made only minimal changes in facilities' funding levels; (2) there is a significant difference between comparable health care facilities' operating costs and patient workloads; (3) VA has not used its resource planning and management system (RPM) to ensure that resources are allocated to facilities within the same priority category; (4) VA excluded over $4 billion of its medical care appropriation from the RPM process during the first 2 years of RPM because it wanted to give VA facilities more time to adjust to the reallocation process and large budget changes; (5) the RPM system does not address veterans' unequal access to outpatient care; and (6) VA plans to reallocate a larger portion of its fiscal year 1996 facility budgets based on the RPM process and implement a decision support system to better compute the costs of specific services provided to each patient.

Book excerpt: The Veterans Equitable Resource Allocation (VERA) system was instituted by the Veterans Health Administration (VHA)-the organization in the Department of Veterans Affairs (DVA) that is responsible for providing health care to veterans-in 1997. The system was designed to improve the allocation of the congressionally appropriated medical care budget to the 22 regional service networks that comprise the VA health system. In recent legislation (H.R. 4635), the U.S. Congress asked the DVA to conduct a study on whether VERA adequately meets the special needs of some veterans. In response to this legislation, the VHA asked RAND's National Defense Research Institute to undertake this study. Specifically, this study examines the degree to which VERA accounts for differences in the age and geographic location of facilities, their patient case mixes, and other factors. The study also examines cost issues associated with affiliations between VA facilities and academic medical centers. The findings and recommendations from the study are documented in this report. Study findings should be of interest to VA personnel, Congress, and other policymakers-particularly those interested in health care for veterans. Health economists and policy planners may also have an interest in the findings. This research was sponsored by the DVA and was carried out jointly by RAND Health's Center for Military Health Policy Research and the Forces and Resources Policy Center of the National Defense Research Institute. The latter is a federally funded research and development center sponsored by the Office of the Secretary of Defense, the Joint Staff, the unified commands, and the defense agencies.

Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

Book excerpt: We are pleased to be here today to discuss the Department of Veterans Affairs (VA) health care resource allocation system and how it could be improved. In fiscal year 2001, VA used the Veterans Equitable Resource Allocation (VERA) system to allocate $17.8 billion of its $20.3 billion health care budget to 22 regional health care networks. Networks allocate the resources they receive from VERA to their respective facilities. VERA was intended to equitably allocate resources by providing comparable resources to networks with comparable workloads. Before VERA was implemented, resources were allocated to facilities primarily on the basis of their historical expenditures. By aligning resources with workloads, VERA shifted approximately $921 million among VA's networks in fiscal year 2001 compared to what the allocations would have been under the previous allocation system.
