Thailand is a country in which family members have a fundamental role in assisting other family members in sickness and in health. Family-oriented interventions, therefore, have the potential to enhance health outcomes for individuals with T2DM. Randomised controlled trials conducted on family-carers of individuals with diabetes in Thailand are limited and none has investigated the potential benefit of a family-oriented DSME program, which includes the family-carer in the intervention. Aims: The primary aim of this study was to test the effectiveness of a family-oriented, theoretically derived (based on self-efficacy) DSME for Thai individuals living with T2DM. The specific objectives of this research are to develop and deliver a family-oriented DSME for Thai individuals with T2DM and carers; to evaluate the effectiveness of a family-oriented DSME in improving diabetes knowledge, glycaemic control, self-efficacy, self-management, and quality of life among Thai individuals with T2DM; to develop and test the validity and reliability of the family-carer diabetes management self-efficacy scale (F-DMSES) that measures diabetes management self-efficacy among family-carers of Thai individuals with T2DM; and, finally, to measure and compare diabetes management self-efficacy between individuals with T2DM and their carers.

Ventura County is consumed with many diabetics. There are not any comprehensive diabetes education programs in the county. Purpose/Scope: The purpose of this evidence-based practice intervention was to evaluate the effectiveness of a Diabetes self-management education (DSME) program on patient's health care utilization, medication use, self-efficaacy for diabetes and general disease management and ability to exercise, and glucose testing. A pres-test/post-test design was utilized to survey subjects participating in the program. The participants received education in four sequential sessions delivered at consistent time intervals over an eight week period. Goal: The goal of developing this program was to empower patients to develop the knowledge and skills necessary to improve health outcomes and to provide access to a diabetes self-management education program open to the general public residing in Ventura County in hopes to improve patient's lives by reducing potential long-term diabetes complications following gaining knowledge of preventative care, improving early diabetes diagnosis, and reducing the workload of primary care practices. Objectives: the objectives of this project included: following the completion of the DSME program, the subject will gain improved scores on the Chronic Disease Self-Efficacy Scales: Exercise Regularly, Manage Disease in General, Health Care Utilizations, Glucose Testing, and self-efficacy to perform self-management behaviors comparing pre-course versus post-course scores. Plan: Phase I and II involved developing the proposal draft and submitting the document to the GPD. The proposal was submitted to the IRB and approved on 3/20/2007. The class content slides were finalized. Phase III involved recruiting subjects and advertising. The classes began April 9, 2007 and continued for four separate sessions through 5/14. Pre-test and post-test questionnaires were handed out to subjects on the first day and last day of classes. Following completion, the data was analyzed. Outcomes and Results: The pre-test and post-test mean scores were compared using paired t-test and the differences were statistically significant.

The program resulted in an increase in both self-efficacy and self-management but did not meet the program benchmark of a 20% increase. This program supports the use of DSME guided by SCT but additional work must be done to tailor the program to the target population and to reach the identified benchmark. Book excerpt: Type 2 diabetes mellitus (T2DM) is a chronic and progressive medical condition that has reached pandemic levels, currently affecting 368 million people worldwide. With previous research demonstrating that T2DM is largely irreversible, the focus is now on developing and disseminating structured diabetes self-management education and support (DSME) programs to help patients live as healthfully as possible without exacerbating their illness. In recent years, several DSME interventions have demonstrated clinically meaningful changes in T2DM patients; however, there appears to be little translation of these programs from research into real world settings because these settings often lack clear blueprints for how to create or implement DSME programs that best fit their organizational needs and structure. Thus, the current study aimed to address this research-practice gap through the development of a brief, theoretically-driven, and behaviorally-focused group-based DSME curriculum for use in real world settings. Phase 1 of the study entailed concentrated efforts to join behavioral health researchers, community health leaders, and T2DM experts in combining the Common Sense Model of Self-Regulation (a prominent health behavior theory), cognitive behavioral principles, and current guidelines from the American Diabetes Association into a novel DSME curriculum. Phase 2 of the study then focused on implementing the resulting four-session curriculum, An Active Approach to Diabetes Self-Management, in local community centers and examining its early acceptability, feasibility, and effectiveness in producing clinical and psychosocial outcomes of interest. A total of 22 adults with T2DM, ranging in age from 35 to 87 years, were recruited from four community organizations and entered into four separate groups. Results indicate statistically significant improvements in participants' glycemic control, diabetes knowledge, and diabetes self-efficacy over the course of the study. In addition, results indicate that both participants and community liaisons were highly satisfied with the intervention. These findings contribute to the field's growing knowledge of how best to capitalize on clinical, research, and real world expertise to design thoughtful and practical DSME programs.

The purpose of the study is to determine if there is a difference in diabetes knowledge, self-efficacy behaviors and HgbA1c for patients newly diagnosed with Type 2 diabetes who complete a certified DSME program compared to a non-certified DSME program in an outpatient setting. The proposed study will be a quasi-experimental, pre-test and post-test design. A total of 108, with 54 participants for each group, will be recruited in San Diego County. The comparison group will complete a non-certified DSME program while the experimental group will complete a certified DSME program. Three data collection instruments will be used: (1) HgbA1c lab test, (2) demographic survey, and (3) the Personal Diabetes Questionnaire. Scoring will be comparing the means of HgbA1c for both the comparison and experimental group and examining if there is a difference in the answers to the questionnaires before and after the study. The proposed study is significant to nursing because advanced practice nurses (APNs) are at a pivotal position for providing diabetes education and referring patients to the right DSME programs. APNs can provide patients the opportunity to increase diabetes knowledge and adapt attitudes and behavior to improve self-management and prevent complications. Latinos are at disproportionately greater risk for developing T2DM and often exhibit poorer adherence to diabetes self-management recommendations, and in turn, worse glycemic control and more diabetes-related complications, compared to non-Latino Whites. Project Dulce was a randomized controlled trial to investigate the effects of a peer-educator led, culturally-appropriate intervention designed to improve diabetes self-management and glycemic control among Latinos with poorly controlled diabetes. The current project investigated the effects of the Project Dulce intervention on diabetes self-management behavior, support resources, self-efficacy, depressive symptomatology, and culture-based beliefs about diabetes, using the Resources and Supports for Self-Management (RSSM) as a conceptual framework. Study participants (N = 207) completed clinical and self-report assessments at baseline, post-intervention (4 months), and follow-up (10 months). Study hypotheses were evaluated via multi-level models using Hierarchical Linear Modeling (HLM) 6.08. The study aimed to assess whether the Project Dulce peer-education group exhibited greater improvements over time in diabetes self-management behaviors, depressive symptoms, support resources for disease management and self-efficacy, as well as culture-based beliefs about diabetes, compared to the control ("care as usual") group. An exploratory aim of the study investigated whether "dosage" (i.e., classes attended) was associated with greater improvements in the Project Dulce intervention group. Analyses revealed improvements in dietary self-management behaviors, depressive symptomatology, self-efficacy, culture-based beliefs about diabetes, support resources for disease management (i.e., friends and family, healthcare providers, neighborhood, and personal support), and self-efficacy, over time, across groups. Analyses also revealed that Project Dulce group participants showed greater improvements in personal support resources for disease management and endorsed fewer culture-based beliefs about diabetes over time, compared to the control group. Furthermore, "dosage" (attendance) analyses, including only Project Dulce group participants, revealed that greater dosage (i.e., attending more sessions) led to greater improvements over time in support resources for disease management (self and neighborhood support), as well as exercise and blood-glucose checking self-management behaviors, compared to those who attended fewer classes. Although analyses revealed that Project Dulce group participants showed improvements over time with increased attendance, this was not significantly greater than improvements experienced by the control group. Possible explanations for improvements in the control group may relate to participants' motivation for change, or contact with the peer-educator during assessments which may have conveyed support or may have created expectancy effects (i.e., "social desirability effects") leading to biased self-report responding. Findings from "Dosage" analyses (including Project Dulce group participants only) indicate that individuals with type 2 diabetes who enroll in the Project Dulce intervention and attend more classes may experience greater support resources from their neighborhood and self-support, which may lead to improved self-management behavior, compared to individuals who attend fewer classes. Distinction of assessor and interventionist roles, and use of objective behavioral data collection methods, may shed light on the degree to which interventions such as Project Dulce can modify health behavior among this high-risk population.

As a result, countless research hours and funds are devoted to developing and testing programs to improve the self-management skills of patients with diabetes. Some interventions produce clinically meaningful changes, but few programs are based on theoretically sound protocols, which makes it difficult to assess theory-based group or individual level variables that might be responsible for changes in self-management. The current study piloted "An Active Approach to Diabetes Self-Management", a novel diabetes self-management intervention based on the integration of two theories (the Common Sense Model of Self-Regulation and Social Cognitive Theory) with techniques from cognitive behavior therapy. The intervention was developed within a Community Based Partnership Research framework. The 4-week intervention consisted of weekly, two-hour group sessions that presented information on and experiences with diabetes self-management, including the self-monitoring of blood glucose, physical activity and nutrition. Sixteen participants participated in three groups held in the fall of 2011 and February of 2012. The primary outcome was hemoglobin A1C (A1C), which was measured before and three months after the start of the intervention. Data were also collected on diabetes self-efficacy, self-management behaviors, diabetes knowledge, general mental and physical functioning and feasibility. iii Results indicated that participants with baseline A1C levels above 6.5% had a significant reduction in A1C over the course of the study. Self-efficacy and understanding of diabetes increased over the course of the study. Diabetes-related negative affect decreased over the course of the study and there was a trend towards a decrease in BMI between baseline and the end of the study. Results also indicate that the workshop was feasible with regards to participant and community staff member satisfaction, study curriculum and the group process. As described within, future iterations must amend inclusion criteria and the curriculum, improve the usability of questionnaires and increase sample size in order to further test feasibility and to determine effect and sample sizes for a larger trial.

Access to education, transportation to appointments, and other social determinants of health can reflect poor self-management, adverse outcomes, and increased morbidity and mortality. The purpose of this quality improvement (QI) project was to evaluate the effectiveness of implementing a series of expert-led, educational classes with a toolkit for patients diagnosed with prediabetes or T2DM. The Health Belief Model was the framework used to develop and implement educational intervention through a 4-month period. This project was conducted at two sites in Northeast Kansas: one which provides primary care services to an underserved population, while the other is a satellite clinic located within a housing authority. The PDSA cycle was used to develop and evaluate program implementation throughout the project. A toolkit was developed and provided to individuals who attended the sessions. Educational sessions focused on T2DM overview, risk factors, comorbidities, medication, diet, exercise, stress, and self-management. Expert specialists were recruited for two of the educational sessions. A diabetes self-management questionnaire (DSMQ) score and glycated hemoglobin (A1c) value were obtained prior to the first educational session. A final DSMQ score and A1c value were obtained at the conclusion of the last educational session. Qualitative measurement revealed improved perceived self-management of T2DM as well as an overall improvement of A1c control after a 4-month period.

Approximately 1 in 10 adults in the United States have diabetes and a significant majority have inadequately controlled blood glucose, blood pressure, and cholesterol. Previous research shows that many adults with diabetes lack the knowledge, skills, behaviors, and support to properly manage their condition. Diabetes self-management education has become an important part of diabetes care guidelines as a way to improve the self-management of adults with diabetes in order to lead to improve patient health and reduce inappropriate health care use and medical costs. Despite this, it is unclear who is actually receiving diabetes self-management education in the US, overall and by mode of delivery and diabetes educator type, and whether education is associated with improved population health. Therefore, the objective of this dissertation research is to investigate the distribution of diabetes self-management education, patient sociodemographic factors that influence the receipt of that education, and its contribution toward diabetes outcomes, medical expenditures, and healthcare use in US adults. We achieved this research objective by completing three specific research aims. We used nationally representative data from the Medical Expenditure Panel Survey and the National Health and Nutrition Examination Survey. We found that although a substantial majority of individuals received some form of diabetes self-management education, receiving diabetes education was associated with race/ethnicity and income (Aim 1), and varied by mode of education delivery. Individuals who received diabetes self-management education were less likely to have glycemic control but were more likely to receive antiglycemic treatment (Aim 2). Receiving diabetes self-management education was associated with greater diabetes-related ambulatory and prescription use, depending on the mode of education delivery (Aim 3). Our findings highlight some strengths of the current state of diabetes management occurring in usual care, including the widespread delivery of some form of diabetes education and its association with greater ambulatory and prescription use. However, we also found evidence of the need for more effective diabetes education delivery systems in usual care in order to improve patient self-care behaviors and reduce inpatient and emergency room use.

Abstract: Objective: To systematically review the effect of self-management education on diabetic control in type 2 diabetes Research design and methods: PubMed was searched for English-language articles published between 2010 and 2013. All the studies were original articles selected manually and used randomized control trials generating results of self-management education in people with type 2 diabetes referring to diabetic control. Relevant data were divided and tabulated into factors of population characteristics, interventions and outcomes. Interventions were classified into three sections as collaborative information intervention, lifestyle intervention, and skills teaching intervention based on the patterns of education. Outcomes were categorized into glycemic control, cardiovascular disease (CVD) risk factors and nephropathy risk factors. Results: A total of 24 studies were identified of initial 41 articles for this review. Effects of self-management education on glycemic control were demonstrated to be positive both in short-term (10 months) follow-up, but more positive effects in short-term follow-up. The same effectiveness happens to CVD risk factors, including lipids, weight and blood pressure. On the other hand, with short-term follow-up, teaching skills intervention of self-management education is more effective than collaborative information intervention and lifestyle intervention on reducing glycemic control and CVD risk factors. Also with long-term follow-up, teaching skills intervention of self-management education had more effectiveness than collaborative information intervention and lifestyle intervention on reducing glycemic control. However, few studies including CVD risk factors in the long-term follow-up, so it is difficult to evaluate the effectiveness of on CVD risk factors with long-term follow-up. Conclusions: Evidences supports the positive effectiveness of self-management education with collaborative information intervention, lifestyle intervention and skills teaching intervention among type 2 diabetes patients on diabetic control, in both short-term follow-up and long-term follow-up, but short-term follow-up is more effective than long-term follow-up. Further research is needed to develop self-management interventions to maintain long-term follow-up effects on glycemic control, CVD risk factors and other diabetes complications. Book excerpt: Over 30 million Americans have been diagnosed with diabetes with 90 to 95% of these patients having Type II (Centers for Disease Control and Prevention [CDC], 2021). Diabetes self-management education (DSME) is a cost-effective but underutilized intervention for patients (CDC, 2021). There have been few studies conducted to examine the effect of simulation as a patient education method. Purpose: This study aimed to examine the effect of a simulation-based educational intervention on diabetes self-care management. Methods: Subjects were adults diagnosed with diabetes with a glycated hemoglobin test (HbA1c) greater than or equal to eight, obtained within the past six months. The participants were randomized into the simulation or educational handout group. The instrument used was the Diabetes Self-Management Questionnaire (DSMQ), which assessed self-care interventions required for glycemic control (Schmitt, 2013). Both groups completed the DSMQ pretest and demographic survey, and then completed the DSMQ again at six weeks post intervention. Results: No formal comparisons were made due to low sample. The mean total DSMQ score was in 7.6 the simulation group and 5.9 in the handout group at pretest. The mean total DSMQ score was 9.1 in the simulation group and 7.5 in the handout group at posttest. Conclusions: The study suggested that immersive learning experiences may be effective in adults with diabetes, regardless of time since diagnosis. More formal education is needed for patients with type II diabetes, especially regarding nutrition. Further studies are needed to further explore the gaps on the use of simulation in patient education.

Book excerpt: Diabetes self-management education: a powerful tool for diabetes control in IndiaDiabetes self-management education (DSME) is a critical element of care for diabetes management and is a powerful tool for reducing glycosylated haemoglobin (HbA1c), risk for complications, hospital admissions and cost of care.1 The purpose of this study was to determine the impact of DSME in improving the outcomes of diabetes care as measured by glycaemic (FPG, PPG & HbA1C), lipid (total cholesterol[TC], triglycerides[TG], low density lipoproteins[LDL] and high density lipoproteins [HDL]) and anthropometric parameters (weight, BMI & waist circumference) in individuals with type 2 diabetes mellitus (T2DM).A retrospective analysis was performed for 160 adult T2DM patients who received DSME training in 2013u20132014 from our center. DSME provided to the study participants was consistent with the requirements from the National Standards for Diabetes Self-Management Education.1 Participants received one time 6 hours intensive group training. All education was provided by a team of diabetologists, certified diabetes educators who in addition are dietician and exercise physiologist. Patients received instruction in: diet/exercise education; self-monitoring of glucose levels; medication management specifically for insulin users; diabetes related problem solving, and lifestyle changes to help reduce the risks and complications of diabetes. Subjects were judged for their acquired knowledge about the disease by using a set of questionnaire and were awarded a score based on their performance. Their socioeconomic status was also ascertained by using Kuppuswammy scoring (KS score)2 which looked at their education, monthly income and occupational status. The subjectsu2019 glycaemic parameters, lipid parameters and anthropometric measurements were checked before the DSME programme and at the end of 6 months and 12 months respectively. The DSME scores were divided into tertiles and lower tertile patients were compared to intermediate and higher tertile patients, to assess, whether differences in acquired knowledge about diabetes had impact on glycaemic, lipid and anthropometric parameters. It was revealed that waist circumference increased in all the three tertiles over the entire year following DSME without any statistically significant change in the weight and BMI over the year. Subjects with DSME score in the lowest tertile had an initial improvement in the FPG values at 6 month but a rebound increase at the end of the year made it lose statistical significance. There was however no statistically significant change in the HbA1C and PPG over the year. The lipid parameters (TC and LDL) showed statistically significant progressive deterioration which perhaps emphasises the need for an intermittent reinforcement of education. Subjects with DSME score in the intermediate tertile had an improvement in the glycaemic parameters (FPG and HbA1C) throughout the year but there was no improvement in the lipid parameters . Subjects with DSME score in the upper most tertile were already having better controls of their glycaemic parameters at the baseline and did not show significant improvements over the year but their lipid parameters (TC and LDL) did show a significant improvements over the year, perhaps secondary to their better understanding of the contribution of lipid control towards prevention of macro vascular complications. Further analysis of the data also revealed that DSME scores were independent of socioeconomic status even when analysed separately for income levels and educational qualification. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic. In conclusion, comprehensive diabetes outcomes were correlated with adequacy of baseline diabetes education. Subjects with even minimal diabetes education (lowest tertile) had transient improvement in outcomes whereas those with optimal education had sustained multifactorial benefits. Moreover DSME benefits were independent of socio-economic and educational status indicating its widespread applicability even in resource limited settings. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic.References :1. Book excerpt: The prevalence of type 2 diabetes mellitus (T2DM) has increased substantially in the United States in recent years. Teaching self-management to diabetic patients is essential to help them control their chronic disease. Albert Bandura’s theory of self-efficacy is commonly used in chronic disease self-management programs and is the theoretical framework upon which this Doctor of Nursing Practice (DNP) pilot project was built. This evidence-based change in practice project took place in a rural primary care clinic in the central valley of California, and involved the development of a diabetes logbook, which was a tool for patients to use to learn self-management of their disease. The logbook was created in response to an identified gap in knowledge among patients at the clinic. The book was composed based on current evidence in diabetes management and treatment. It was introduced to the patients and a validated tool (Diabetes Self-Efficacy Scale) was used before and after the project to determine the patients’ self-efficacy scores. The project implementation took place over a three-month period of time. Though the number of project participants was small, and difficulties were encountered with follow-up with some patients, overall the pilot project was successful at increasing self-efficacy scores, with a mean pre-project score of 7.57, and a mean post-project score of 8.08, which is an increase of 0.51. The mean Hemoglobin A1c (HbA1c) pre-project was 8.75, and the mean HbA1c post-project was 8.19, indicating a decrease of 0.56. Any decrease in A1c can be seen as clinically significant, as even small reductions can decrease short and long term complications of diabetes. This evidence-based change in practice project met its objective of increasing patients’ perception of diabetic self-management. This project was designed to be translatable to other primary practice settings. Sharing tools that are developed based on the current evidence will help to improve all patient healthcare outcomes.

The U.S. spends an estimated $245 billion in direct and indirect medical costs. The indirect costs include disability and lost productivity (CDC, 2014). Previous studies focusing on diabetes education acknowledge the significance of self-management activities in managing T2DM. Extant studies examining self-management and treatment outcomes for persons with T2DM have paid limited attention to the relationship between patients' disease perceptions, diabetes knowledge, self-efficacy, self-management practices, and glycemic control. The purpose of this investigation was to identify factors and outcomes associated with healthy diabetes self-management practices among a cohort of individuals with T2DM residing in southern California. Nola Pender's Health Promotion Model (Pender et al., 2015) guided the study. Specific aims include: 1) Provide conceptual clarity for the phenomenon of perception through a concept analysis using Walker and Avant (2011) methodology, 2) Identify factors and outcomes associated with diabetes self-management through a systematic review of the literature, and 3) Examine relationships among perceived self-efficacy, specific patient characteristics, diabetic knowledge, self-management practices, and HbA1C values among a sample of individuals with T2DM living in southern California. The research aims provided the structure to generate new scientific data and evidence towards the improvement of self-management practices and those characteristics leading to improved HbA1C outcomes.

Book excerpt: Diabetes mellitus (DM) has been a long standing healthcare problem and is the leading cause of morbidity by disease because of associated cardiovascular problems. It is a chronic disorder that requires lifetime control and management. Adherence to diabetes management which can contribute to glycemic control has been proven to prevent complications brought about by this endocrine problem. In spite of its importance, problems of non-adherence still exist because treatment regimens prescribed for patients are complex. Hence this study was conducted to develop and test the effectiveness of the researcher-devised ‘MEDS’ planner 9M-medication, E-exercise, D-diet, S-self-monitoring of blood glucose or SMBG self-efficacy adherence of patients to care plan. It was theoretically anchored on Azjen’s Theory of Planned Behavior and Bandura’s Social Cognitive Theory. This study employed two groups, random assignment, pretest-posttest quasi-experimental design and was conducted in Bustos, Bulacan, Philippines. A total of 48 DM 2 diagnosed patients currently following prescribed management were recruited through purposive sampling and were assigned randomly to experimental and comparison groups. The experimental intervention is ‘MEDS’ Planner, a researcher-devised self-management strategy which includes specific plan of care on the four aspects of DM 2 management 9medication, exercise, diet, and SMBG.) Outcomes were measured by the Summary of Diabetes Self-care Activities (SDSCA) and Self-Efficacy for Diabetes (SED). To analyze the pretest and posttest scores of both groups, Wilcoxon signed-rank test was used for SDCA and paired t-test for SEDC. To compare the improvements of both groups, Mann Whitney U test was applied for SDSCA and multiple linear regression analysis (ANCOVA) was used for SED. DM 2 patients who utilized ‘MEDS’ Planner with Motivational Interviewing in their care plan showed significant improvement on 2 components of SDSCA: exercise (p=0.0038) and self-monitoring of blood glucose (p=0.0001). Medication (p=0.6644) and diet (p=0.0705) components did not improve significantly. Significant improvement of self-efficacy (p=0.0001) of experimental group participants was observed.
