Book excerpt: The setting for this Clinical Nurse Leader (CNL) project was the Post Anesthesia Care Unit (PACU) at a level I trauma center in the Bay Area. The goal was to improve the discharge education performed by the PACU nurses to improve patient safety and decrease the chance of complications or readmissions to this hospital. With no clear instructions for how discharge teaching should be done, the nurses have many differing styles which leaves room for gaps in discharge planning. This influenced the implementation of the teach-back method during discharge planning in order to ensure proper education and increased patient understanding. A literature review revealed that the teach-back method has helped to decrease hospital readmissions and prevents complications patients may experience during home self-care. The Joint Commission, Agency for Healthcare Research and Quality, and the Institute for Healthcare Improvement all state that the teach-back method for discharge planning is best practice. Project data was collected through observation, microsystem assessments and through nurse and patient surveys. Interventions included nurse education at staff meetings, one on one in-service educations and teach-back handouts. The pre-intervention nurse survey (n=26) showed that 80% of the nurses were familiar with the teach-back method while only 36% of nurses used teach-back during discharge planning regularly. The patient survey (n=25) revealed that patients were able to retain 68% of the information presented during discharge education. One on one in-service education on the teach-back method is still being conducted at this time, with high nurse satisfaction. Follow-up surveys will commence when 100% of the staff have completed the in-service. The expected final outcome of this project is the implementation of the teach-back method during discharge planning by all nurses and increased patient understanding of discharge education.

Book excerpt: The purpose of this Direct Practice Improvement (DPI) project is to examine to what degree a relationship exists between one group of patients that used the teach-back method with return demonstrations at a local community hospital and another group that did not use these teaching strategies at the same hospital. Low health literacy could cause patients to have increased readmission rates to the hospitals, infections, and complications. The sample consisted of the postpartum population using Albert Bandura's social learning theory. Literature suggests that teaching strategies such as teach-back method and return demonstrations are proven to be useful tools in assisting patients in comprehending their patient teachings and decreasing health literacy in patients. The quantitative quasi-experimental design was used to measure the effect of retention teaching strategies. One group of patients received the retention teaching strategies of the teach-back method and return demonstrations, and the second group was given traditional instruction using narrative only. After the interventions for the respective groups, they both completed the Test of Functional Health Literacy in Adults (TOFHLA), to assess if the patients retained their instructions. The findings were as follows: 11 out of 11 patients in group one demonstrated good retention strategies and adequate functional health literacy upon receiving their patient teaching and completion of their TOFHLA. In group two, eight out of 11 patients demonstrated good retention strategies and adequate functional health literacy upon taking the TOFHLA. There were two patients who scored marginal functional health literacy, and one patient scored inadequate functional health literacy

While many elements may contribute to patient satisfaction with discharge education, use of the teach-back method has been found to improve retention of discharge instruction, as well as increase patient confidence in chronic disease management and self-care at the time of discharge. Patient satisfaction with discharge education is a notable component of hospital reimbursement by national standards, as measured by the Hospital Consumer Assessment of Healthcare Providers and Systems survey (HCAHPS) results. Following education of nursing staff participating in use of discharge teaching, patient satisfaction with discharge instructions taught using the teach-back method was measured via HCAHPS for a period of two months. Results suggested potential correlation to improved HCAHPS scores for the question “When I left the hospital, I had a good understanding of the things I was responsible for in managing my health,” associated with use of teach-back at time of discharge.

Book excerpt: The Centers for Medicare & Medicaid Services (CMS) have been moving from volume-based, fee-for-service payment to value-based payment (VBP), which aims to improve health care quality, health outcomes, and patient care experiences, while also controlling costs. Since the passage of the Patient Protection and Affordable Care Act of 2010, CMS has implemented a variety of VBP strategies, including incentive programs and risk-based alternative payment models. Early evidence from these programs raised concerns about potential unintended consequences for health equity. Specifically, emerging evidence suggests that providers disproportionately serving patients with social risk factors for poor health outcomes (e.g., individuals with low socioeconomic position, racial and ethnic minorities, gender and sexual minorities, socially isolated persons, and individuals residing in disadvantaged neighborhoods) may be more likely to fare poorly on quality rankings and to receive financial penalties, and less likely to receive financial rewards. The drivers of these disparities are poorly understood, and differences in interpretation have led to divergent concerns about the potential effect of VBP on health equity. Some suggest that underlying differences in patient characteristics that are out of the control of providers lead to differences in health outcomes. At the same time, others are concerned that differences in outcomes between providers serving socially at-risk populations and providers serving the general population reflect disparities in the provision of health care. Systems Practices for the Care of Socially At-Risk Populations seeks to better distinguish the drivers of variations in performance among providers disproportionately serving socially at-risk populations and identifies methods to account for social risk factors in Medicare payment programs. This report identifies best practices of high-performing hospitals, health plans, and other providers that serve disproportionately higher shares of socioeconomically disadvantaged populations and compares those best practices of low-performing providers serving similar patient populations. It is the second in a series of five brief reports that aim to inform the Office of the Assistant Secretary of Planning and Evaluation (ASPE) analyses that account for social risk factors in Medicare payment programs mandated through the Improving Medicare Post-Acute Care Transformation (IMPACT) Act.

Book excerpt: This practice improvement project determines the impact of patient teaching with the use of the teach-back technique on patient satisfaction. The purpose of this project was to see to what degree a relationship exists between the use of the teach-back technique and patient satisfaction for patients who were discharged from the study unit at the participating hospital, a small 280-bed acute care hospital in the southeastern section of the United States and completed a Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey. This project was based on Orem's self-care theory and the theory of symptom management. The clinical question for this project was: Does patient teaching with the use of the teach-back technique have an impact on patient satisfaction as evidenced by an increase in HCAHPS scores related to discharge teaching items? HCAHPS data was reviewed for 146 patients pre-intervention and 96 patients post-intervention of the educational seminar. Use of the teach-back technique was the independent variable and patient satisfaction was the dependent variable. An educational seminar on discharge teaching with the use of the teach-back technique was provided to staff nurses. The study unit at the participating hospital has low patient satisfaction scores for discharge teaching related to understanding care and medication. Data analysis revealed patient satisfaction scores increased from 19.7% pre-intervention to 19.8 % post-intervention related to understanding care and 25.1% pre-intervention to 28.9% post-intervention related to understanding medications. Results demonstrated there was a positive correlation between patient teaching with the use of teach-back technique and patient satisfaction.
