Book excerpt: The purpose of this study was to develop a theory grounded in data of how diabetic COGIC members combine their Afro-Theistic faith with their knowledge of type 2 diabetes mellitus (DM), through Afro-Theistic social constructs. With grounded theory as a methodology, this study unequivocally followed Corbin and Strauss (2007) processes of developing a grounded theory. The interview of only five participants provided enough thick and rich data to saturate five categories that emerged on the way to developing a grounded theory. The five categories that emerged were: (1) Faith, the most prevalent, (2) Fix and (3) Fatalism, both actions, and (4) Fear and (5)Frustration, both emotions. A Theory in 3-D emerged as a portrait of the grounded theory developed along with the actions, interactions and the emotions relative the subculture (COGIC) of African-Americans with type 2 diabetes mellitus (DM). This Theory in 3-D captured a theory of Substantive Living. The Theory in 3-D also morphed into a model called Permeated Learning. In the process of developing a theory concerning DM, this study traces the African-American culture back to slavery. The tracing of the African-American culture is in two areas, theism and health. Theism provided the source of African-Americans subcultures through the divisions of churches. Most of the health discussions relates to diabetes in the African-American culture, but a discussed recreational exercise of the African-American ancestors is at the end of chapter 5.

But has science gone so far in racializing diabetes as to undermine the search for solutions? In a rousing indictment of the idea that notions of biological race should drive scientific inquiry, Sweetness in the Blood provides an ethnographic picture of biotechnology’s framings of Type 2 diabetes risk and race and, importantly, offers a critical examination of the assumptions behind the recruitment of African American and African-descent populations for Type 2 diabetes research. James Doucet-Battle begins with a historical overview of how diabetes has been researched and framed racially over the past century, chronicling one company’s efforts to recruit African Americans to test their new diabetes risk-score algorithm with the aim of increasing the clinical and market value of the firm’s technology. He considers African American reticence about participation in biomedical research and examines race and health disparities in light of advances in genomic sequencing technology. Doucet-Battle concludes by emphasizing that genomic research into sub-Saharan ancestry in fact underlines the importance of analyzing gender before attempting to understand the notion of race. No disease reveals this more than Type 2 diabetes. Sweetness in the Blood challenges the notion that the best approach to understanding, managing, and curing Type 2 diabetes is through the lens of race. It also transforms how we think about sugar, filling a neglected gap between the sugar- and molasses-sweetened past of the enslaved African laborer and the high-fructose corn syrup- and corporate-fed body of the contemporary consumer-laborer.

Few studies have focused solely on dietary changes (one of the foundations for diabetes self-care), particularly in the context of family and the role of AAW. The purpose of this descriptive ethnographic study was to explicate cultural influences on food practices of AAW with type 2 diabetes (T2DM) in order to inform the health care community as well as future development of culturally-tailored interventions. Specific aims were to describe typical daily food practices and identify cultural influences on food practices of AAW with T2DM. Symbolic Interactionism, a sensitizing framework for viewing AAW with T2DM as a subculture, guided this study. Purposeful sampling was used to recruit 20 AAW who: were between 35 and 70 years of age, had been diagnosed with T2DM, shopped and prepared meals for their families, and attended church functions where food was served . Data collection consisted of one-one-one interviews and participant observation of church fellowship dinners, grocery shopping, and food preparation. A social anthropological approach to content analysis was used to describe behavioral regularities in food practices. Trustworthiness was maintained by an audit trail. Findings indicate that for informants in this study, who had diabetes ranging from 2 to 30+ years, there is a constant struggle between cultural food practices and eating healthier because of diabetes, particularly within the home setting where a majority of daily food practices take place. Difficulties in making dietary modifications result from conflicts between the need to change dietary practices to control diabetes and personal food preferences, as well as the preferences of people within the participants' social network. In addition, difficulties derive from AAW's emotional dedication to the symbolism of food and traditional cultural food practices. AAW are the gatekeepers for family food practices and are the keys to healthy dietary practices. This study begins to fill the research gap regarding cultural dietary food practices of this population. With increased knowledge, researchers and health care providers will be better able to improve AAW food practices, and ultimately improve diabetes control in this high-risk population.

Book excerpt: Introduction: African Americans are disproportionally diagnosed with Type-2 diabetes, and have been observed to have poor self-management, which increases risk of complications. Social influences are commonly associated with diabetes-related outcomes. Purpose: The purpose of the study was to assess whether and how socio-contextual factors influence self-management and emotional distress in urban African American adults living with type-2 diabetes, as well as to assess whether social support facilitate relatedness need fulfillment within this population. The study also sought to gain an understanding of the psychosocial support needs and preferences of urban African American adults living with type-2 diabetes. Methods: The descriptive study employed a mixed-methods research design. African American adults were recruited to take a one-time written survey. Hypotheses one and two were tested using hierarchical linear regression, hypothesis three was assessed using VI a simple linear model, and hypotheses four and five were evaluated using path analysis. Four gender-stratified follow-up focus groups were also conducted, to confirm quantitative findings and gain insight into the support needs and preferences of urban African American adults living with type-2 diabetes. Study Variables: Independent study variables included social support, autonomy support, basic psychological needs (autonomy, competence, relatedness), and autonomous regulation. Primary dependent variables were diabetes self-management and diabetes-related emotional distress. Results: 155 eligible individuals (67 male, 88 female) were included in the quantitative analytic sample. 23 individuals (10 male, 13 female) participated in follow-up focus groups, yielding a total of 6 hours of audio-recorded focus group data. Autonomy and competence significantly predicted self-management and emotional distress in hypotheses one and two, respectively. Competence was the strongest predictor of selfmanagement and emotional distress among sample participants, followed by autonomy, but relatedness was not observed to be a significant variable in the prediction of study outcomes. Social support was observed to significantly predict relatedness need fulfillment in a simple linear model for hypothesis three. Data did not support hypothesis four and five. Qualitative findings contextualized the quantitative study findings. Conclusions: Future support programs should facilitate basic need fulfillment in urban African American adults living with type-2 diabetes. Relatedness is especially important in this collectivistic cultural group.

Book excerpt: ABSTRACT: Diabetes is a significant public health problem and a leading cause of disability and death in the United States. The purpose of this study was to assess diabetes knowledge and self-care activities and explore experiences of African-American women with type 2 diabetes mellitus (T2DM). This mixed method study used a quantitative correlational design and a qualitative descriptive design. The data was collected using surveys and semi-structured individual interviews and analyzed using SPSS 17.0 and Atlas ti 5.2. Data were transcribed verbatim and examined for emergent themes. The sample included fifty-two African-American women between the ages of 40-64 from the North Central Florida region. The mean age for the sample was 55 years. Approximately 55.8% of the participants had higher than the average knowledge for diabetes. Results showed no associations between the respondent's level of knowledge about diabetes and certain self-care activities. However there were positive significant correlations between knowledge about diabetes and the years of education (rho=.24) and some self-care activities namely (a) specific diet (r=.28) and (b) foot care (rho=.40).

Book excerpt: Through the lens of Type 2 diabetes this dissertation considers race and problems of difference and risk with developments in treatment, genomic science, and the conduct of research and research priorities. Based primarily on fieldwork in New York and California, I interrogate public health notions of outreach with biotechnology and clinical research concepts of biomedical translation as synonymous practices. Institutional relationships and marketing drivers, I argue, reflect relatedness back onto the Type 2 diabetes patient through causal narratives of risk and inevitability. In effect, kinship--genetic, familial, racial, ethnic, and environmental--becomes the driver of both risk and emergent forms of bioliterary discipline. I present a narrative of how diabetic risk became racialized over time and how African Americans became seen as a desirable research population. Arguing against biological race, I present an ethnographic example of how one such African American population, or community, emerged from particular social and political histories. Fieldwork uncovered lingering memories of the Tuskegee Experiment combined with cultural incompetency in both public health and biotechnology sectors. Further, I consider the bioethical challenges of African (American) participation in new genomic research aimed toward reducing health disparities. However, as a racial category, genetics researchers debate the precise genetic location and definition of "Africa" in the human genome. I suggest that the search for a pathological Africa in the human genome may deepen racial stigmatization as well as author new narratives of difference. I submit that social disparity, not biological disparity, presents the ultimate challenge to successful effective clinical translation and public health outreach.
