The total number of bones in the skeletal system are 270 at birth and gradually reduce in number to 206 at adulthood due to fusion of few bones. Hip joint is amongst the largest of the weight-bearing joints after the knee and also one of the strongest joints. Usually, the average male femur is about 480mm in length with a diameter of 23.4mm. The hip joint is a classic example of a ball and socket joint where the head of the femur articulates in the acetabulum of the pelvis. The head of the femur and acetabulum are covered by a 63.5mm thick shiny white hyaline which acts as a smooth cushion to the joint. This joint has synovial fluid articulated in the mating surface which allows the joint to flex under different pressures without causing wear and tear. The synovial fluid and the hyaline help the bones move each other at different degrees of freedom without causing pain to the human. Usually, hip problems are associated with aged people due to worn-out hip joints. However, hip joint disorders can be associated with any person irrespective of age without proper care. Hip replacement surgery is considered to be amongst the greatest advances in health care of the twentieth century. In 1925 the first replacement made out of glass was used for the surgery by an American surgeon Marius Smith-Petersen. An American surgeon named Dr. Austin T. Moore at the Columbia Hospital, South Carolina performed the first metallic hip replacement surgery on September 28, 1940. Later, Sir John Charnley, who is considered the father of modern total hip arthroplasty, modeled a low friction arthroplasty in the early 1960s. This design was identical, in principle, to the prosthesis used in the present day. It consisted of a stem and head of the femur and an acetabular cup with a backing cup.

The survival of a total hip replacement depends principally on the enduring integrity of the fixation interfaces and of the articular interface. The design of the stem and the material properties of cement largely determine the state of the component-cement inter face, while the bone-cement interface is significantly influenced by both mechanical and biological factors. The surface finish and shape of cementless implants are designed to preserve the integrity of biological fixation (osseo-integration) at the bone-component interface. Once again, both mechanical and biological factors have to be considered, while bioactive coatings accelerate bone ongrowth. Metal-on-polyethylene is the most widely used articular interface. However, it has been suggested that wear of polyethylene is one of the major factors contributing to failure of total hip replacements. The increasing prevalence of total hip replacement in younger patients has stimulated the investigation of alternative, more durable couples -including ceramic-polyethylene, ceramic-ceramic and metal-on-metal. Modularity provides greater intra-operative flexibility, but each new modular interface introduces new mechanisms of failure. These need to be anticipated and appropriate measures taken to avoid them. Hopefully this book will provide a better understanding of the factors that contribute to stable interfaces and long-term survival of total hip arthroplasty.

We investigated variances in trunnion design, materials, and manufacturing processes in addition to what it takes, for the average surgeon, to obtain such information. We designed a product questionnaire intended for the manufacturers to investigate the most important characteristics of currently utilized trunnions. We inquired about size of the cone, angle of engagement, length, and postmanufacturing processing. We carefully reviewed the manufacturer's website, phoned their customer service, and attempted contact with the hip engineering team. Last, we e-mailed the questionnaire to their hip product specialist. A total of eleven companies were contacted. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.
