Book excerpt: This book aims to highlight the importance of the development of health conditions and demand for the application of noninvasive mechanical ventilation (NIMV) outside the intensive care units (ICUs); the diversification of possible scenarios outside the ICUs; the need to establish references that consolidate this phenomenon and the healthcare organizations models. In the last decades the extension of the use of NIMV outside of the ICUs has led to the generation of protocols and to the creation of new in-hospital care models. In this field, the main determining factors are a better knowledge of technique, technological advancement, better monitoring capacity, the creation of multidisciplinary teams adequately trained in their application, and social and health events that have overloaded ICUs. All these elements have promoted the creation of these NIMV units outside ICUs. This new reality entails the need for clarification of concepts, recommendations, and analysis of how to plan NIMV. Although the literature that clearly determine the indications and aids on the use of NIMV is considerable, this volume, pointing out the diversity of different healthcare models to define how to organize NIMV outside the ICUs, shed a light and bring a clear benefit to the scientific community involved. The book is structured in eleven main sections analyzing the epidemiology and trends for NIMV healthcare models and determining factors for these models outside ICUs. The originality of the work, its clear clinical-practical impact and the multidisciplinary approach given by all healthcare professionals involved (intensivists, pneumologists, internal medicine and emergency medicine specialists, geriatricians, chest respiratory therapists, etc.) is very relevant for the thoroughness of the book.

Book excerpt: Although diagnosis always begins with a careful history and physical examination and a physician is obligated to consider more than the diseased organ, testing of lung function has become standard practice to confirm the diagnosis, evaluate the severity of respiratory impairment, assess the therapy response and follow-up patients with various cardio-respiratory disorders. Ventilation, diffusion, blood flow and control of breathing are the major components of respiration and one or more of these functional components can be affected by any disorder. Frequently, no single pulmonary function test.

This portable text covers current and practical approaches to evaluation and management of conditions encountered in the fetus and the newborn, as practiced in high volume clinical services that include contemporary prenatal and postnatal care of infants with routine, as well as complex medical and surgical problems. Written by expert authors from the Harvard Program in Neonatology and other major neonatology programs across the United States, the manual’s outline format gives readers rapid access to large amounts of valuable information quickly. The Children’s Hospital Boston Neonatology Program at Harvard has grown to include 57 attending neonatologists and 18 fellows who care for more than 28,000 newborns delivered annually. The book also includes the popular appendices on topics such as common NICU medication guidelines, the effects of maternal drugs on the fetus, and the use of maternal medications during lactation. Plus, there are intubation/sedation guidelines and a guide to neonatal resuscitation on the inside covers that provide crucial information in a quick and easy format.

Available in PDF, EPUB and Kindle. Book excerpt: Why Critical Care Evolved METs? In early 2004, when Dr. Michael DeVita informed me that he was cons- ering a textbook on the new concept of Medical Emergency Teams (METs), I was surprised. At Presbyterian-University Hospital in Pittsburgh we int- duced this idea some 15 years ago, but did not think it was revolutionary enough to publish. This, even though, our fellows in critical care medicine training were all involved and informed about the importance of “C- dition C (Crisis),” as it was called to distinguish it from “Condition A (Arrest). ”We thought it absurd to intervene only after cardiac arrest had occurred, because most cases showed prior deterioration and cardiac arrest could be prevented with rapid team work to correct precluding problems. The above thoughts were logical in Pittsburgh, where the legendary Dr. Peter Safar had been working since the late 1950s on improving current resuscitation techniques, ?rst ventilation victims of apneic from drowning, treatment of smoke inhalation, and so on. This was followed by external cardiac compression upon demonstration of its ef?ciency in cases of unexpected sudden cardiac arrest. Dr. Safar devoted his entire professional life to improvement of cardiopulmonary resuscitation. He and many others emphasized the importance of getting the CPR team to o- of-hospital victims of cardiac arrest as quickly as possible.

Book excerpt: Written by outstanding authorities from all over the world, this comprehensive new textbook on pediatric and neonatal ventilation puts the focus on the effective delivery of respiratory support to children, infants and newborns. In the early chapters, developmental issues concerning the respiratory system are considered, physiological and mechanical principles are introduced and airway management and conventional and alternative ventilation techniques are discussed. Thereafter, the rational use of mechanical ventilation in various pediatric and neonatal pathologies is explained, with the emphasis on a practical step-by-step approach. Respiratory monitoring and safety issues in ventilated patients are considered in detail, and many other topics of interest to the bedside clinician are covered, including the ethics of withdrawal of respiratory support and educational issues. Throughout, the text is complemented by numerous illustrations and key information is clearly summarized in tables and lists.

Book excerpt: Written for all health care professionals who wish to assess the problems of their patients and clients in a systematic and effective way—whether diagnosing a problem, predicting the response to treatment or monitoring change. The editors have succeeded in making this book comprehensive in scope, covering the major areas in which most clinicians and researchers will desire ready access to measurement techniques and instruments including: screening and detection of psychiatric illness, depression, obsessions and compulsions, social dysfunctioning, mental handicaps, special problems of the elderly, drug problems, eating and sleeping disorders and sexual behavior. In addition to proposing guidelines for the selection and construction of measurement methods and listing the relevant questionnaires and techniques, the merits and limits of rating scales and other methods are considered. There is also an attempt to clarify diagnostic, definitional and other conceptual issues in relation to the problems covered.
