Book excerpt: This volume contains the proceedings of the Fourth International Symposium on Coronary Heart Disease, held in Frankfurt/Federal Republic of Germany. In 1970, when the fIrst symposium was held, the whole range of diagnostic and therapeutic measures was covered, but since then interest in coronary heart disease has increased so considerably that the fourth symposium was restricted to only two therapeutic aspects, angioplasty and intracoronary thrombolysis. It can be noted with pleasure that European cardiologists, namely Andreas Griintzig and Peter Rentrop, have made essential contributions to the applicability of these modes of treatment. When angioplasty was fIrst discussed at the Third International Symposium (Coronary Heart Disease, Stuttgart, 1978) a great deal of astonishment was expressed. Today the procedure has gained an important place in the treatment of angina pecto ris. The proportion of candidates for coronary surgery in whom angioplasty can be attempted instead of operation is around 10%, in certain subgroups even higher. The most remarkable fmding of our 5-year experience with angioplasty are the extre mely good long-term results. Recurrences happen in about 15%-20% of cases within the fIrst 3 months; after this period of time, recurrences are very unusual and mostly due to progression of the disease at other sites. Angioplasty has thus also contributed to our understanding of the dynamics of the atherosclerotic process.

It has been the purpose of this meeting to focus the interest of scientifically and clinically interested cardiologists on the new developments in this field. We therefore chose the subtitle 'Medical-Mechanical -Surgical Approach'. The medical improvements in myocardial perfusion have been brought about by the application of streptokinase, urokinase and tissue-type plasminogen ac tivator in the first hours after the onset of an acute myocardial infarction. The different modes of application and the possibilities to evaluate and eventually to quantify the results of these treatments were addressed during the first part of the meeting. The mechanical way to improve perfusion nowadays mainly consists of the application of intracoronary balloon angioplasty. Although since 1977 the treat ment has become a routine method, several questions are still open such as the exact mode of action, the reaction of the vessel wall, the optimal pressure and balloon size as well as the long term results and the prevention of restenosis.

The identification of patients best suited for thrombolytic therapy has been a challenging task, as has the ideal adjuvant strategy. The emergence of information vital for patient care has appeared at an extraordinary pace, with hundreds of articles being published yearly. Unfortunately, a resource devoted to the area of thrombolysis does not exist, making the dissemination of information to physicians, scientists and health care providers problematic. The Modern Era of Coronary Thrombolysis is designed to bring the medical and scientific communities up to date. It will serve as a foundation for future investigation, as well as a resource which can be referred to for many years to come.

Book excerpt: Obstruction of coronary blood flow and the resultant consequences are the center stage pathophysiologic events in cardiology today. The speculations of Jenner, Burns, Heberdin, McKenzie, Prinzmetal and many others had until now been left to observations of isolated tissue and intact animal experimentation. Only with the advent of Gruentzig's technique, which allowed us to 'work safely inside the coronary arteries' are we able to observe the effects of coronary occlusion in living conscious man. PTCA provides not only a therapeutic modality for non-operatively opening coronary obstructions, but has also provided the best model for studying the effects of acute ischemia on the heart. The procedure also lead the way to all other interventional cardiology developments, including modern thrombolysis in the setting of acute myocardial infarction. In his previous works, Serruys has examined how PTCA can serve as a model for studying acute ischemia. In this book, he and his co-authors discuss the effects of balloon-induced ischemia on the electrocardiographic changes, coronary blood flow dynamics, cardiac muscle metabolism and left ventricular function, as well as measures to counter these effects and provide for reperfusion in unstable angina and acute myocardial infarction. Technology has expanded the 'eyes' of the observer of these events. The authors use many techniques including ECG recording from surface, endocardium, and intracoronary electrodes; angiographic assessment of coronary flow pattern using digital techniques, as well as doppler flow measurements; biochemical assessment of metabolic products stimulated by ischemia; and digital angiographic and echo doppler assessment of left ventricular function.

Continuous development of the procedure and equipment has made complex and multi-vessel-inter ventions possible and has led to other techniques of intravascular angioplasty. In reviewing current results, this volume pays particular attention to causes, incidence, circumstances, recognition, treatment and outcome of complications of PTCA; it also considers restenosis, which still occurs at an unsatisfactory high rate after primarily successful procedures. To deal with these topics, experts on PTCA met to discuss their experiences and insights into peculiar aspects of complications in PTCA. The discussion was subdivided into: Recognition of complications; Complications in new interventional techniques; Management of complications; and Restenosis after angioplasty. The question of whether angiographic and pathological characteristics of sclerotic lesions allow the identification of complication-prone lesions in advance is discussed in the first chapter.
