Constant movement is necessary to preserve its function, but regular and well is also essential. The high frequency of spinal disorders result positioned rest ing from misuse is easily explained by day-to-day reality. Among the disorders that result from misuse of the spine, herniated disk, leading to radicular compression, is one of the most frequent. New tech niques, less invasive and yielding more precise information, have been pro gressively developed for the diagnosis of this disease and at the same time new methods of treatment have appeared, giving us a much broader range of choices and decisions to make. In the face of this evolving, complex situation, a multidisciplinary team from Strasbourg decided to clarify the topic. A single man's experience, what ever his qualities, would certainly have been insufficient and the necessarily limited views of a single speciality would also have been a handicap. This re markable work is thus the result of collaboration between clinical and inter ventional radiologists and a neurosurgeon.

Book excerpt: Considering the numerous works dealing with the angiography of the human brain, the book presented by SZIKLA et al. might seem to some to be devoted to superfluous precision, especially as it is inspired by "stereotactic" thinking. The large arterial trunks and their branches were described by anatomists for a long time, then were restudied by neuroradiologists for recognition in a more and more detailed manner on arteriograms. However, until now no encompassing work has been done to specify precisely the relationship of the blood vessels to that large and important organ, the human brain cortex, thereby permitting the recognition of the sulci and gyri as a function of the successive curves imposed on the various vessels by the deep infoldings of the cortex. Insofar as the radiologic evaluation of the cerebral cortex is concerned, fractional pneumoencephalography allows the injection of a number of sulci and fissures via the subarachnoid spaces. It should be pointed out, however, that sufficiently complete and interpretable images are obtained only under favorable circumstances (successful technique, cerebral atrophy, absence of cerebral edema, absence of arachnoid symphysis, etc. ). In addition a large number of sulci cannot be made visible by pneumography for strictly anatomic reasons such as the level of their opening into cisternal spaces.

The greatest impetus to this pro gress was given by the introduction of selective and superselective spinal angiography, microsurgical technique, and embolization. These sophisticated techniques and the skill required for their use are far from being routine in the neurosurgical and neuroradiologic departments. In spite of the rarity of spinal angiomas, the application of the above-mentioned procedures is the prerequisite for improving early diagnosis and giving timely adequate treat ment. Delay in diagnosis and treatment are still the main cause of unsatisfac tory results. In the last 10 -20 years, several groups in Europe and the USA have done important and fundamental work in introducing and developing the diganos tic and therapeutic armamentarium. Based on the pioneering work of their teachers and the classic contribution of Wyburn - Mason in 1943, they simultaneously improved the morphologic, physiologic, and clinical basis of our knowledge. Although progress is going on and many problems have to be solved, the general principles of clinical diagnosis, operative treatment, and embolization have been laid down and are to be published in a special monograph on this topic.

International and national meetings, symposia and seminars, workshops, and newsletters success fully bring to the attention of physicians within developing specialties what is occurring, but generally only in demonstration form without providing historical perspective, pathoanatomical correlates, or extensive discussion. Page and time limitations oblige the authors to present only the essence of their material. Pediatric neurosurgery is an example of a specialty that has developed during the past 15 years. Over this period, neurosurgeons have obtained special train ing in pediatric neurosurgery, and then dedicated themselves primarily to its practice. Centers, Chairs, and educational programs have been established as groups of neurosurgeons in different countries throughout the world organized themselves respectively into national and international societies for pediatric neurosurgery. These events were both preceded and followed by specialized courses, national and international journals, and ever-increasing clinical and investigative studies into all aspects of surgically treatable diseases of the child's nervous system.
