Book excerpt: EXECUTIVE SUMMARY AND RECOMMENDATIONS: The goal of this TIP is to recommend guidelines for primary care clinicians to follow in caring for patients with alcohol and other drug use disorders. These guidelines were developed by a Consensus Panel of clinicians, researchers, and educators who work on the prevention and treatment of substance use disorders. Protocols are based partly on research evidence, partly on Panel members' clinical experience. The algorithm to the left follows a patient with substance use problems who presents in a primary care setting. The chart will serve as a guide or road map through screening, brief assessment, brief intervention, assessment, referral, specialized treatment, and followup care as they are detailed in the TIP. Since substance use disorders are often chronic conditions that progress slowly over time, primary care clinicians, through their regular, long-term contact with patients, are in an ideal position to screen for alcohol and drug problems and monitor each patient's status. Futhermore, studies have found that primary care clinicians can actually help many patients decrease alcohol consumption and its harmful consequences through office-based interventions that take only 10 to 15 minutes (Kahan et al., 1995; Wallace et al., 1988). This potential, however, is largely untapped: Saitz and colleagues found that of a sample of patients seeking substance abuse treatment, 45 percent reported that their primary care physician was unaware of their substance abuse (Saitz et al., in press). Yet even though screening and limited treatment of substance use disorders do not require a large time investment, the Consensus Panel that developed this TIP recognized that many primary care clinicians are already overwhelmed by the demands imposed by expanded gatekeeper functions. The Panel realized that a practical approach to addressing patients' substance abuse problems was needed: one that recognized the time and resource limitations inherent in primary care practice and offered a series of graduates approaches that could be incorporated into a normal clinic or office routine. Biological, medical, and genetic factors as well as psychological, social, familial, cultural, and other environmental features all bear on substance abuse. Addressing the condition effectively requires a team effort, especially when it has progressed beyond the early stage. For this reason, in addition to screening and intervention treatment options, these guidelines include information about viable referral for assessment and treatment, as well as followup. Readers will notice that the TIP contains more information on alcohol use and abuse than on use of illicit drugs. This reflects both the scope of the problems and the research literature available about them. It is estimated that about 18 million people with alcohol use problems and 5 million users of illicit drugs need treatment. Although the Panel recognizes that tobacco is an addictive substance with a major public health impact, it is not included in this TIP because the topic falls outside CSAT's purview. Readers are referred to "Smoking Cessation: a Guide for Primary Care Clinicians," published by the Agency for Health Care Policy and Research (Agency for Health Care Policy and Research, 1996). The Consensus Panel's recommendations are based on a combination of clinical experience and research-based evidence. In the list below, the summary guidelines supported by the research literature are followed by (1); clinically based recommendations are marked (2). Citations supporting the former are referenced in the body of the document. Screening and assessment instruments mentioned below are reproduced and discussed in Chapters 2 and 4 and Appendix C. The guidelines are presented in more detail in Chapter 6.

Book excerpt: Series Editors: Moira Stewart, Judith Belle Brown and Thomas R Freeman Primary care clinicians are often unfamiliar with new and effective methods for detecting substance abuse problems in their earliest stages, and the majority of patients with substance abuse problems remain undiagnosed. Substance Abuse is written by primary care clinicians and focused to meet the needs of primary care providers, demonstrating how the patient-centered clinical method can assist clinicians in learning how to diagnose this complex psychosocial disorder. This book describes how to use state-of-the-art screening techniques, and how to understand and motivate patients to decrease or eliminate harmful use of alcohol and drugs. It presents the latest scientific findings and gives examples of using a patient-centered approach, as well as describing specific communication skills, with samples of dialogue illustrating their use in helping substance-abusing patients. This is essential reading for all family doctors, paediatricians, gynaecologists, psychiatrists, nurses, social workers, psychologists and all clinicians whose practices include substance abusing patients. It will also appeal to counsellors, education personnel and all professionals working with substance abusing individuals.

Available in PDF, EPUB and Kindle. Book excerpt: During the last few centuries, and particularly in recent decades, problems result ing from the excessive use of drugs and alcohol have spread virtually as an epidemic to every country in the world and to almost every community. Abuse of alcohol and drugs is related to numerous other health problems, such as the spread of acquired immune deficiency syndrome (AIDS) on all continents. Health and social services today cannot afford to ignore this crisis since it affects all levels of society and manifests itself in diverse health and social problems. In recent years, the World Health Organization has received numerous requests for training material for physicians in this field. This manual therefore meets an urgent need. The availability of new data and the dearth of suitable textbooks have made its preparation mandatory. The influence of sociocultural factors on drug dependence and alcohol related problems-on their cause, development, and consequences as well as on their treatment and prevention-has been taken into account in the preparation of this manual in order to ensure that its usefulness is not limited to one country or region. It has been prepared primarily for the teaching of physicians and medical students, although much of it is relevant to the training of nurses, midwives, health educators, primary-care workers, medical social workers, counselors, and psychologists. In fact, suggestions have been included for adapting the manual for use in the training of such varied groups of students.

Book excerpt: While there is a wealth of published information on addiction medicine, the psychological aspects of alcohol abuse, and behavioral medicine with regard to addiction, virtually none of these resources were written with the primary care provider in mind. Addressing Unhealthy Alcohol Use in Primary Care is a resource for primary care clinicians who are confronted by patients with these problems daily, and who wish to successfully address these issues in their practice. It would focus on the literature and science relevant to primary care practice and cover the range of interventions appropriate for this setting. Topics include assessment, brief counseling interventions, pharmacotherapy, referrals to both specialty care and Alcoholics Anonymous (and other self-help programs), psychiatric co-morbidity and other drug use, and other information specific to the needs of the primary care provider.

Book excerpt: "Implementing Evidence-Based Practices for Treatment of Alcohol and Drug Disorders" provides managers and clinicians with results from Practice Improvement Collaboratives (PIC) that demonstrate how substance abuse treatment can be improved by increasing the exchange of knowledge between community-based service providers and the research community. The book examines improvement collaboratives and mentoring strategies for adopting and using evidence-based practices. Contributors address how to determine the best treatment processes to serve clients, how to deal with the hurdles faced in preparing and training counsellors, and how to affect the needed changes in agency activities. This unique professional resource responds to an Institute of Medicine report that found a substantial disconnect between research and practice in treatment for drug and alcohol dependence. Focusing on how to make the changes necessary to support the adoption and use of evidence-based practices, the book documents the activities of four sites to illustrate how investigators and treatment practitioners worked together to implement evidence-based practices. Contributors examine the development and early implementation of Practice Improvement Collaboratives, the investigator-provider-policymaker model, Motivational Enhancement Therapy, the use of Opinion Leaders in training, and targeted strategies that take into account the differences in clinician demographics and training. "Implementing Evidence-Based Practices for Treatment of Alcohol and Drug Disorders" is an essential tool for alcohol and drug counsellors, directors of alcohol and drug treatment clinics, and instructors in counsellor training and academic programs.

Book excerpt: This book has been written to serve as a manual for physicians practicing in a private office setting to recognize and recommend appropriate treatment for patients believed to be substance abusers. While it is not written for drug abuse specialists, it provides information regarding the diagnosis and treatments a competent, concerned physician can provide without becoming immersed in addiction treatment. Topics discussed include the degree to which a physician should become involved, when it is appropriate to refer, using other professions and volunteer groups, and useful medications. Guidelines for recognizing substance abuse, testing to confirm the abuse, confronting the patient, and motivating the patient into specific treatment are also presented. Tables and illustrations are used to summarize major points, making this an extremely useful reference tool for internists and other non-specialist private practitioners.

A Clinical Guide to Drug and Alcohol Problems provides a comprehensive overview to help diagnose and treat these problems. The first five chapters provide basic information on historical and cultural issues, plus the pharmacology of all abused drugs the physician is likely to come into contact with and the epidemiology and etiology of substance abuse problems. The author then addresses the clinical manifestions and course of addiction; diagnostic techniques; principles of clinical management, treatment, and rehabilitation of addictive and other associated medical disorders; and guidelines for public health approaches to the problem.
