Book excerpt: With increasing scrutiny on the use of seclusion and restraint on America's troubled youth, it is important for psychiatric hospital providers to find ways to minimize the frequency and length of use of seclusion and restraint. The present study took advantage of a unique opportunity to investigate the effects of policy implementation in August 1999 by the Centers for Medicare and Medicaid Services (CMS) and subsequent policy implementation in January 2000 by a non-profit child and adolescent psychiatric hospital. A Univariate ANOVA was completed that showed significant reductions in the length of time of seclusions and restraints following implementation of CMS policy and the use of mechanical restraints were eliminated. Additional decreases were noted with the next policy implementation. Significant relationships were also noted pertaining to type of special measure, diagnosis, and age. These results indicated the importance of administrative leadership in shaping seclusion and restraint practices and show the lack of psychological technology to address violence prediction and prevention. Recommendations for future study are included.

Mental health hospitals and health care monitoring organizations have placed great emphasis on its reduction or total eradication. Restraint and seclusion, when not properly conducted may be very dangerous and injurious to the individual. The use of seclusion or restraint causes significant risks to the physical and psychological wellbeing of the individual and that of the staff. Psychiatric Nursing Assistants (PNA's) have little training about mental health, because most of them are high school graduates, with sometimes no experience except the trainings given to them on employment. Some of them have been working at the facility for two or three decades. This lack of education or insufficient training with old age leads sometimes to inappropriate response when a patient is in a state of mental health crisis and physical aggression resulting in restraint and seclusion. Further training, among other measures, for psychiatric nurse's assistants, such as verbal de-escalation techniques, continuing education on mental health issues, consumer behavior when in crisis and appropriate ways to respond to a consumer when in crisis will help reduce seclusion and restraint. Providing more training for PNA's will complement the efforts of the psychiatric nurses to reduce restraint and seclusion. In a benchmark exercise conducted in psychiatric intensive care units at two Western Australian hospitals to assess staff confidence in coping with patient aggression, research shows that the staff at the hospital where staff had training had more confidence in dealing with aggression than the hospital without staff training (McGowan, Wynaden, Harding, Yassine, and Parker, 1999).
