This chapter is intended to share the challenges facing existing public health advocacy and interventions against tobacco smoking in SSA countries and to make recommendations necessary to control tobacco smoking in the SSA countries.

Book excerpt: "On 27 February 2005, the WHO Framework Convention on Tobacco Control (WHO FCTC) entered into force and became legally binding for the contracting countries, referred to as the Parties to the Convention. Five of the first 40 countries that ratified the Convention are in the African Region. As of 31 December 2014, 43 of the 47 countries in the African Region had ratified or acceded to the WHO FCTC (38 ratifications and five accessions). Parties to the WHO FCTC have the obligation to report periodically to the Conference of the Parties on their progress in implementing the Convention. Parties in the African Region are therefore obliged to submit reports to the Conference of the Parties within the reporting cycles. To date, 34 Parties have submitted at least one report on their progress in implementing the Convention. In 2005, the Fifty-fifth session of the Regional Committee for the African Region reviewed the first report on implementation of the WHO FCTC in the Region and endorsed the proposed actions. The Regional Committee recommended that Member States ratify the WHO FCTC, develop and implement comprehensive tobacco control legislation and adopt national plans of action in accordance with their obligations under the Convention. In 2013, the Sixty-third session of the Regional Committee for the African Region reviewed and endorsed the second report on implementation of the WHO FCTC in the Region. The Regional Committee recommended that countries accelerate implementation of the Convention by prioritizing enforcement of existing laws, allocating adequate resources proportionate to the burden of tobacco and intensifying South-South collaboration. This 10-year report highlights the achievements made in the African Region in implementing the WHO FCTC during the period February 2005 to December 2014."--Page 1

Reducing and controlling tobacco smoking should be a primary aim for a certain population, in order to reduce harms to health caused by this important risk factor, and it seems urgent to adopt intervention tools involved in responsibility fields such as health care, education, politics, economy and media. Among health professionals the prevalence of tobacco smoke is extremely high, more than other professional categories, and this could be partly attributed to a low weight that tobacco smoking has in the medical curriculum of future physicians, that will contribute in a determinant way to healthy choices of their patients. In order to realise that, the medical students need to be adequately trained with the aim of acquire competences and skills that help patients to prevent tobacco smoking and to increase smoking cessation, through a programme oriented to specific issue related to the potential harm of tobacco products. A survey conducted by Ferry et al. in the American Schools of Medicine underlined the lack of courses related to tobacco smoking. Moreover, a randomised trial carried out by Cummings et al., the Schools of Medicine result as the ideal setting to teach smoking cessation techniques to health professionals. The National Cancer Institute in 1992 recommended that primary and secondary prevention interventions on tobacco smoking will become mandatory in the curriculum of Medical USA students. However, until now this recommendation still is far from being fully implemented. The aim of the book is to give an overview on the epidemiology of tobacco smoking among different settings and populations, but with a special focus on health professionals and medicals students, and to show available examples of smoking prevention and cessation training in different settings.

Book excerpt: The fact that tobacco ingestion can affect how people feel and think has been known for millennia, placing the plant among those used spiritually, honori?cally, and habitually (Corti 1931; Wilbert 1987). However, the conclusion that nicotine - counted for many of these psychopharmacological effects did not emerge until the nineteenth century (Langley 1905). This was elegantly described by Lewin in 1931 as follows: “The decisive factor in the effects of tobacco, desired or undesired, is nicotine. . . ”(Lewin 1998). The use of nicotine as a pharmacological probe to und- stand physiological functioning at the dawn of the twentieth century was a landmark in the birth of modern neuropharmacology (Limbird 2004; Halliwell 2007), and led the pioneering researcher John Langley to conclude that there must exist some “- ceptive substance” to explain the diverse actions of various substances, including nicotine, when applied to muscle tissue (Langley 1905). Research on tobacco and nicotine progressed throughout the twentieth century, but much of this was from a general pharmacological and toxicological rather than a psychopharmacological perspective (Larson et al. 1961). There was some attention to the effects related to addiction, such as euphoria (Johnston 1941), tolerance (Lewin 1931), and withdrawal (Finnegan et al. 1945), but outside of research supported by the tobacco industry, addiction and psychopharmacology were not major foci for research (Slade et al. 1995; Hurt and Robertson 1998; Henning?eld et al. 2006; Henning?eld and Hartel 1999; Larson et al. 1961).
