Book excerpt: "Though 20th century has seen life expectancy largely lengthened worldwide, aging population, chronic diseases, worsening food supply with deficit nutrition and environmental problems add to the burden of healthcare systems all around the world. Data analytics, which has been seen as a significant power in other industries, is expected to contribute to the improvement of efficiency and effectiveness in healthcare. This thesis aims to identify and promote more effective and efficient strategic, operations and clinical policies in healthcare systems through descriptive, predictive and prescriptive analytics. To this end, this thesis focuses on three essays, i.e. three data-driven problems based on medium to large size of real life datasets, on: i) design of financial incentive systems for maternity care; ii) design of specialist response policies and modified triage coding to reduce waiting times in emergency departments (EDs), and iii) design of observation units for hearth failure patients. The first essay focuses on strategic level and aims to design a two-level financial incentive mechanisms to reimburse physicians, in order to reduce unnecessary C-sections while retain it for those who need it, resulting in enhanced birth quality with alleviated economic burden for overall health care system. Contributing to clinical decision-making, we first cluster the patients according to their pregnancy complexities, and characterize a threshold between spontaneous birth and medically necessary planned C-section by analyzing 12.7 million annual birth records from National Bureau of Economics Research through statistical learning methods. Then we compare payment systems analytically vis-\'{a}-vis a variety of performance measures within two-level hierarchy, (i) mainstream payment models and (ii) compensation on the top of mainstream payment, and provide insights about the effectiveness of alternative payment models in the context of maternity care. Finally, we propose optimal payment for physicians to maximize the value for patients under the principal and agent framework, from the strategic perspective.The second paper focuses on operational level and targets to reduce the length of stay in EDs by designing a systematic response policy for various specialists depending on ED clinical demands. This work is motivated by and verified with 40,000 ED visits to a local community hospital in Montreal. We first identify a class of patients who are more likely to require specialist consultation based on their clinical information available at the triage stage through statistical analysis. Then we analyze several alternative policies for specialists' response to consultation requests using queuing models with non-homogeneous Poisson arrival rates. Moreover, we examine an integrated ED decision-making by incorporating specialist consultation requests in the triage system. Finally, our proposed optimal specialist response policy and associated modified triage coding are verified through a comprehensive simulation model. We provide a feasible guideline of integrated patient streamlining to shorten length of stay and alleviate overcrowding in ED. The third paper focuses on clinical level and propose a framework to design a dedicated observation unit for acute decomposition heart failure patients, in order to provide proper treatment and reduce unnecessary hospitalization and chance of post-discharge events. To this end, we, first, use multiple analytical models to figure out the proper number of bed for this observation unit based on historical patient arrival data from a local community hospital. Based on the confined range of analytical capacity, we use simulation models to analyze different discharge and admission policies. We propose an optimal discharge-admission criteria for this dedicated observation unit to realize cost-saving and quality enhancement of treating acute decomposition heart failure patients." --

The first essay addresses issues in healthcare. Our goal is to evaluate the cost-effectiveness of bevacizumab compared to the baseline treatment with only chemotherapy in recurrent/persistent and metastatic cervical cancer using recently reported updated survival and toxicology data. We developed a Markov model with 5 patient health states for both treatments. With data based on the Gynecologic Oncology Group 240 randomized trials and the 2013 MediCare Services Drug Payment Table and Physician Fee Schedule, we present monthly transition probabilities and cost data. Our results show that chemotherapy plus bevacizumab can delay progression, but incur more complications.The second essay lies at the interface between operations management and marketing. We aim to understand the tradeoffs in offering outlet stores. In particular, we study how much differentiation should be kept between the main and outlet stores from three perspectives: price, product and location. We find that an outlet store is more likely to be opened when travel sensitivity is lower or costs associated with it are lower. Moreover, offering an outlet store encourages the rm to improve the quality of the product sold in the main store as to reduce the cannibalization effect. We also observe that location differentiation has a substitution effect on quality and price differentiation.In the third essay, we study several operational challenges for the on-demand service platforms. We consider a situation when an on-demand service platform uses earning sensitive independent providers with heterogeneous reservation prices to serve its time and price sensitive customers with heterogeneous valuation of the service. We present a queueing model with endogenous supply and endogenous demand to model this on-demand service platform. Based on our analysis, we find that it is optimal for the platform to charge a higher price, pay a higher wage, and offer a higher payout ratio when the potential customer demand increases. We use a set of actual data from a large on-demand ride-hailing platform in numerical experiments to illustrate some of our main insights.

The first essay examines the impact of health information exchange (HIE) on the service quality of long-term care (LTC) facilities based on a five-year (2013-2017) panel data set of the U.S. LTC facilities. Our results show that the readmission rate of an LTC facility with an operational HIE is reduced by 2% on average as compared to the rate of a facility without an operational HIE. We also find that EHR or Telemedicine do not report significant performance enhancement with the combination of HIE on reducing readmission rate since LTC facilities do not use EHR and Telemedicine to their full potential. Our findings empirically demonstrate the importance of promoting effective data exchange in LTC facilities as well as improving the rudimentary use of EHR and Telemedicine to increase the value that HIE can create. The second essay analyzes artificial intelligence (AI) patents in healthcare IT granted by the U.S. Patent and Trademark Office (USPTO). We use network analysis to examine co-patenting collaborations among organizations and analyze technological knowledge inflows and outflows in inter-organizational citation networks. The radical factors of patents are also identified to evaluate the level of the patent impact in the AI research of health IT. Our findings provide managers with actionable guidance on how they should adjust their innovation management plan with respect to AI innovation types. The third essay investigates the impact of effort cost on implementing advanced IT on an on-demand platform with heterogeneous service options. A principal-agent model is developed to find the optimal service policy of both the agent and the principal in the context of healthcare system. We also show that the incentive misalignment issue between the principal and the agent can be solved by offering extra compensation for the less preferred service type of the agent, which in turn affects the agent's strategy and the principal's rewards."--Pages ix-x.

To that end, in this dissertation we studythree important, related, and yet independent, healthcare research problems are studiedwith two viewpoints data analytics based and operations research (OR) based techniquesin healthcare. In this research, our objective is to develop, validate and integrate thedata-driven predictive models into the hospital workflow and provide decision makingsupport for medical professionals. It is our hope this research can help generate solutionsto these problems, improving the efficiency of hospital operations, patient outcomes, andpatient and staff satisfaction.The three issues addressed in this dissertation are: (1) patients return to hospitalwithin 72 hours after discharge from being an inpatient (e.g., surgical) due to adverseevents, (2) developing a provider scheduling system for outpatient clinic at a multi-clinicfacility, and (3) developing a preference based scheduling model for a healthcare providernetwork (HPN).In the first problem we develop a stacked classification model to predict short-termand long-term readmission, emergency department revisits and deaths (REDD) alongwith identifying the important factors contributing to adverse events. We proposeda stacked classification model with level one training models using logistic regression,random forest and gradient boosting method. The second level learning model was anon-negative least square method. The final outcome is a linear combination of thepredictions from the three learning models. The short-term (3-day) model had a fairc-statistic of 0.671 and 0.664 for the derivation and validation cohort, respectively. Thec-statistic of long-term (30-day) REDD model was 0.713 and 0.711 for the derivationand validation cohort, respectively. These models were implemented at Geisinger HealthSystem and are under pilot study. In the second problem, we propose a multi-objective provider scheduling modelunder demand and capacity uncertainty and patient no-shows. Though the problem isformulated as a mixed integer programming (MIP), the network structure of the problemsimplifies the computational complexity of the problem.Resource flexibility is an important criterion for any scheduling model to be practical.In the final research problem, the appointment scheduling with multiple clinic locations,multiple physicians is proposed as a network model, which gives it the necessary flexibilityto dynamically assign providers to patients in real time but also to be rigid enough so thatit can be implemented months in advance. In addition to that, we consider multi-prioritypatients with heterogeneous preference in our model. We develop theoretical boundsfor the best achievable policy as well propose a heuristic approach to solve the dynamicproblem. Using extensive numerical experiments we show the proposed scheduling policiescan outperform the current scheduling practices. Finally, to make the model closer toreality we propose two extensions to the model. In the first extension, the demand followsdoubly stochastic Poisson process. We also show that the proposed dynamic policy isrobust under the assumption of a doubly stochastic assumption. In the second extension,we propose joint capacity planning and appointment scheduling problem, which can beformulated as a mixed-integer nonlinear programming (MINLP).iv.

Book excerpt: The topic of this dissertation is the development of data-driven methods and tools which may be used to aid policy makers and consumers in key healthcare decisions. Two specific problems are studied: one problem is in the hospital operations management, the other one is faced by consumers in the Marketplace. In both settings, new model and computational methods are developed to assist the decision maker in making an optimal decision. Chapter 2 presents a forecasting mechanism of operating room workload and dynamic adjustment policy on the staffing level decisions, which may be used by hospitals to better schedule the operating rooms. Staffing levels are usually determined months in advance when little information about the surgeries and their durations is known. This lack of information leads to a mismatch between the staffing level and the actual surgical duration, which results in excessive labor costs. This paper proposes a new dynamic staffing policy according to which hospital administrators adjust nurse staffing levels as information on the different types of surgeries arrives sequentially. The staffing level adjustments are accompanied with cost in order to incorporate the unwillingness to change. Using this dynamic framework, it is shown that a threshold policy defined by two adjustment levels is optimal. Numerical results using data from a large academic center suggest that the practical appeal of the dynamic policies will be greater where they will face the largest resistance: in hospitals where management considers dynamic labor adjustments to be impractical due to their complexity and thus implied costs. Chapter 3 begins the research on the impact of the Health Insurance Marketplace (the Marketplace) on consumers. The aim is to study the motivation behind the consumer choice and the roles that insurers and government play. It also provide insights in improving the efficiency in the Marketplace. This chapter introduces a cost point of view in modeling consumer decision. Consumers make rational choice by optimizing the objective function. The objective function includes the actual costs and the cost equivalent of health utilization. It is driven by the evidence that consumers value their costs most importantly. Moreover, this chapter presents a conceptual framework of dividing health utilization into regions. This captures the practical features such as over utilization in healthcare. Bounded rationality is also discussed and three potential directions are provided in this chapter. Chapter 4 builds a model to predict medical expenditure which consists of a regression, a transition, and an evolution of the parameters. This model is the first to use the number of service visits as predictors. This is consistent with the collecting process of data which is at an service event base. Moreover, it incorporates the behavior considerations of the different insurance status of consumers. Aside from this model, two alternatives models are considered for comparison. They reflect two existing approaches: regression on personal characteristics and log transformation for nonzero expenditures. The predictors are selected and their coefficients are evaluated using the longitudinal data from Medical Expenditure Panel Survey (MEPS). The two alternatives are compared with the model and it turns out that the model outperforms them in both prediction accuracy and variability explanation. Chapter 5 presents the simulation of consumer choice. The results suggest that the cost saver better matches the statistics from the federal government than the utility maximizer. Moreover, this chapter provides implications to the share between insurers and consumers. The actuarial values (the share of expenditure paid by insurers) is calculated based on simulated participation. They demonstrates inconsistency with the standard ones. Therefore inefficiency exits in the actuarial value calculation. This chapter also evaluates the government involvements in the Marketplace. The results suggest that its involvements, especially the government subsidies, effectively encourage the broader participation of insurance plans.

Book excerpt: The healthcare system in the US is rapidly changing and reshaping to adopt continuously evolving demand for improved operational efficiency and treatment effectiveness from patients and providers in critical health services. Healthcare service systems and clinical treatment operations need to be more predictable to increase operational efficiency through proactive operations management. This research contributes to the literature by discovering clinical processes and calibrating discrete-event simulation models in healthcare service systems using data-driven and process-driven predictive models. Unlike the data-driven predictive approaches such as machine learning and statistical methods, the proposed methodologies in this thesis leverages and focuses on process-based methods and analysis in healthcare service systems. Our first contribution is an integrated framework for process-driven multi-variate change point detection by coupling change point detection models with machine learning and process-driven simulation modeling in healthcare service systems. Initial development and succeeding calibration of discrete-event simulation models for complex healthcare systems require precise identification of dynamically changing process characteristics. Existing data-driven change point methods assume that changes are extrinsic to the system and cannot utilize available process knowledge. Our framework leverages simulation models to generate system-level outputs that are then used to predict system characteristics and change points using neural networks. The framework’s optimization layer iterates the change points by repeating simulation and predictive model building steps until the simulated system characteristics conforms to that of the actual process data. Using an emergency department case study, we demonstrate that the developed approach significantly improves change point detection accuracy over data-driven methods’ estimates and is able to detect actual change points. Our second contribution is a time-to-event prediction approach for clinical care operations in intensive care units. By focusing on the sepsis treatment in intensive care units, we predict time-to-event for antibiotic administration at critical vital states of the sepsis-risk patients. Our approach’s most salient aspects are the feature engineering specific to sepsis care and timing and labeling of the predictions. Using a real dataset, MIMIC-III (Medical Information Mart for Intensive Care-III) dataset, we demonstrate that the approach is able to accurately predict a practical time-window for antibiotic administration. Through predicted antibiotics administration time interval, the providers can make informed decisions and the operations staff can proactively coordinate activities to ensure meeting service standards for quality of care.

In the first part of the thesis, we investigate the impact of inventory control on the availability of drugs to patients at public health facilities in Zambia. We present consistent empirical data and simulation results showing that, because of its failure to properly anticipate seasonal variations in demand and supply lead-times, this system leads to predictable patient-level stock-outs even when there is ample inventory available in the central warehouse. Secondly, we propose an alternative inventory control system relying on mobile devices and mathematical optimization, and present results from a validated simulation model suggesting that its implementation would lead to a substantial improvement of patient access to drugs relative to the current system. In the second part of the thesis, we investigate the impact of returning customers on pricing for fashion Internet retailers. Our analysis of clickstream data from an online fashion retailer shows that a significant proportion of sales is due to returning customers, i.e. customers who first visit an item at a particular price, but purchase the item in a later visit at a lower price. We propose a markdown pricing model that explicitly incorporates returning customers. We propose a model for quantifying the value of the returning pricing model relative to a pricing model that does not distinguish between first-time and returning customers, and determine the value of returning pricing both exactly and through developing bounds. Based on real data from a fashion Internet retailer, we estimate the parameters of the returning demand model and determine the value of the returning pricing model. Lastly, we study the promotion optimization problem faced by grocery retailers, i.e. deciding which items to promote and at what price. Our formulation includes several business rules that arise in practice. We build demand models from data in order to capture the stockpiling behavior through dependence on past prices. This gives rise to a hard problem. For general additive and multiplicative demand structures, we propose efficient LP based methods, show theoretical performance guarantees and validate our results using real data.
