Book excerpt: Since 1948, the World Health Organization (WHO) has launched numerous programs aimed at improving health conditions around the globe, ranging from efforts to eradicate smallpox to education programs about the health risks of smoking. In setting global health priorities and carrying out initiatives, the WHO bureaucracy has faced the challenge of reconciling the preferences of a small minority of wealthy nations, who fund the organization, with the demands of poorer member countries, who hold the majority of votes. In The World Health Organization between North and South, Nitsan Chorev shows how the WHO bureaucracy has succeeded not only in avoiding having its agenda co-opted by either coalition of member states but also in reaching a consensus that fit the bureaucracy's own principles and interests. Chorev assesses the response of the WHO bureaucracy to member-state pressure in two particularly contentious moments: when during the 1970s and early 1980s developing countries forcefully called for a more equal international economic order, and when in the 1990s the United States and other wealthy countries demanded international organizations adopt neoliberal economic reforms. In analyzing these two periods, Chorev demonstrates how strategic maneuvering made it possible for a vulnerable bureaucracy to preserve a relatively autonomous agenda, promote a consistent set of values, and protect its interests in the face of challenges from developing and developed countries alike.

Book excerpt: Since the beginning of the World Health Organization, many of its staff members, regional offices, member states, and directors-general have grappled with the question of what a 'spiritual dimension' of health looks like, and how it might enrich the health policies advocated by their organisations. Contrary to the wide-spread perception that 'spirituality' is primarily related to palliative care and has emerged relatively recently within the organisation, this study shows that its history is considerably longer and more complex, and has been closely connected to the WHO's ethical aspirations, its quest for more holistic and equitable healthcare, and its struggle with the colonial legacy of international health organisations. While such ideals and struggles silently motivated many of the key actors and policies - such as the provision of universal primary healthcare - which for decades have embodied the organisation's loftiest aspirations, the WHO's official relationship with 'spirituality' advanced in fits, leaps, and setbacks. At times creative and interdisciplinary, at others deeply political, this process was marked by cycles of institutional forgetting and remembering. Rather than as a triumph of religious lobbyists, this book argues, the 'spiritual dimension' of health may be better understood as a 'ghost' that has haunted - and continues to haunt - the WHO as it comes to terms with its mandate of advancing health as a state of 'complete well-being' available to all.

Book excerpt: In recent public workshops and working group meetings, the Forum on Microbial Threats of the Institute of Medicine (IOM) has examined a variety of infectious disease outbreaks with pandemic potential, including those caused by influenza (IOM, 2005) and severe acute respiratory syndrome (SARS) (IOM, 2004). Particular attention has been paid to the potential pandemic threat posed by the H5N1 strain of avian influenza, which is now endemic in many Southeast Asian bird populations. Since 2003, the H5N1 subtype of avian influenza has caused 185 confirmed human deaths in 11 countries, including some cases of viral transmission from human to human (WHO, 2007). But as worrisome as these developments are, at least they are caused by known pathogens. The next pandemic could well be caused by the emergence of a microbe that is still unknown, much as happened in the 1980s with the emergence of the human immunodeficiency virus (HIV) and in 2003 with the appearance of the SARS coronavirus. Previous Forum meetings on pandemic disease have discussed the scientific and logistical challenges associated with pandemic disease recognition, identification, and response. Participants in these earlier meetings also recognized the difficulty of implementing disease control strategies effectively. The growing reliance on truly international health strategies by national and international agencies, including the more definite and extended practice of preven tive medicine, has provided the means to achieve significant gains in the quality of health in years to come. A redeeming feature of contemporary failures in science and medi cine is that-once intelligently studied, analyzed, and evaluated-even these failed efforts may provide real insights that can mold our capacity and determination. So it is that, more than in any bygone age, the past ten years have seen the implementation of a sound and systematic in frastructure for international health undertakings, thus paving the way for improved health for all.
