The long-term human and fiscal consequences of child abuse and neglect are not relegated to the victims themselves-they also impact their families, future relationships, and society. In 1993, the National Research Council (NRC) issued the report, Under-standing Child Abuse and Neglect, which provided an overview of the research on child abuse and neglect. New Directions in Child Abuse and Neglect Research updates the 1993 report and provides new recommendations to respond to this public health challenge. According to this report, while there has been great progress in child abuse and neglect research, a coordinated, national research infrastructure with high-level federal support needs to be established and implemented immediately. New Directions in Child Abuse and Neglect Research recommends an actionable framework to guide and support future child abuse and neglect research. This report calls for a comprehensive, multidisciplinary approach to child abuse and neglect research that examines factors related to both children and adults across physical, mental, and behavioral health domains-including those in child welfare, economic support, criminal justice, education, and health care systems-and assesses the needs of a variety of subpopulations. It should also clarify the causal pathways related to child abuse and neglect and, more importantly, assess efforts to interrupt these pathways. Book excerpt: Child abuse policy in the United States contains dangerous contradictions, which have only intensified as the public slowly accepted it as a middle class problem. One contradiction is the rapidly expanding child abuse industry (made up of enterprising psychotherapists and attorneys) which is consuming enormous resources, while thousands of poor children are seriously injured or killed, many while being "protected" by public agencies. This "rediscovery" has also led to the frenzied pursuit of offenders, resulting in the sacrifice of some innocent people. Moreover, the media's focus on the sensational details of high-visibility sexual abuse cases has helped to trivialize, if not commercialize, the child abuse problem. As such, child abuse has gone from a social problem to a social spectacle. By the 1980s the child welfare system had become a virtual "nonsystem," marked by a staggering turnover of staff, unmanageable caseloads, a severe shortage of funding, and caseloads composed of highly dysfunctional families (many with drug-related problems). To make room for these families, public agencies rationed services by increasingly screening-out child abuse reports which contained little likelihood of serious bodily harm. In The Politics of Child Abuse in America, the authors argue that child abuse must be viewed as a public safety problem. This redefinition would make it congruent with other family-based social trends, including the crackdown on domestic violence. Children must have the same legal protection currently extended to physically and sexually abused women. This can be done by creating a "Children's Authority," which would have the overall charge for protecting children. Specifically, Children's Authorities would have the responsibility for providing the six main functions of child protection: investigation, enforcement, placement services, prevention and education, family support, and research and development. Offering a unique perspective on the cold reality of this crisis, The Politics of Child Abuse in America will be a provocative work for social workers and human service personnel, as well as the general reader concerned with this timely issue.

Book excerpt: In 2016, the National Child Abuse and Neglect Data System (NCANDS) documented that 1,750 children died nationwide due to child maltreatment and 676,000 suffered maltreatment that was substantiated by authorities (U.S. Department of Health & Human Services, Administration for Children and Families, Administration on Children, Youth and Families, Children's Bureau, 2018). During the same period in Alaska, NCANDS reported 1 child maltreatment fatality and 3,142 children suffered maltreatment that was substantiated by authorities (USDHHS, 2018). By applying two cost estimate methodologies of child maltreatment calculated by Peterson, Florence, & Klevens (2018) and Fang, Brown, Florence, & Mercy (2012) we derived Alaska estimates and compare these to those of the United States. Calculating costs using investigated cases and the most up-to-date inclusive cost estimation methodology we find that child maltreatment in 2016 resulted in an annual Alaska lifetime economic burden of $7.1 billion (2016 USD). This compares to an estimate of $2.08 trillion nationwide. This estimation methodology quantifies direct costs and those perceived from the societal perspective which includes all costs regardless of payer or when they accrue. Although nationwide total costs are almost 300 times that of Alaska, when looking at costs on a per capita basis (using child population aged 0-18), Alaskans have a burden of $37,700 per child compared to $28,300 per US child. NCANDS reported that in 2016 Alaska had 3.38 children under 18 for every 10 adults compared to the US having 2.95 (USDHHS, 2018). These high costs associated with child maltreatment in Alaska, which are 1.33 times that of the per capita costs of the nation, emphasize the need for population based and targeted prevention investments.

In this occasional paper, we provide an overview of the ways researchers have defined and estimated the scope of child sexual abuse, focusing on agency tabulations and large-scale surveys conducted over the last several decades. More precise estimates of the number of children affected by child sexual abuse would improve the ability of the public health, child welfare, pediatrics, and other communities to prevent and respond to the problem. We recommend using a comprehensive surveillance system to assess and track the scope of child sexual abuse. This system should be grounded by common definitional elements and draw from multiple indicators and sources to estimate the prevalence of a range of sexually abusive experiences.
