Book excerpt: Posttraumatic stress disorder (PTSD) and substance use disorders (SUDs) are independently and concurrently associated with emotional and interpersonal problems. Though current PTSD-SUD treatments primarily target individual factors, there is growing support for the involvement of interpersonal relationships in treatment for PTSD-SUD. It remains unclear how PTSD symptom severity combined with intrapersonal and interpersonal factors may increase risk for craving, particularly in the context of romantic conflict. The present study recruited trauma-exposed individuals (n = 82) in current or recent romantic relationships in treatment for a SUD. Additionally, we examined the relations of PTSD symptom severity to intrapersonal emotion regulation (ER), interpersonal emotion regulation (IER), communication patterns, and craving following conflict. There was a significant indirect relation of PTSD symptom severity to the desire and intention to use drugs following conflict through intrapersonal ER difficulties. Further, there was a significant negative association between PTSD symptom severity and constructive communication and significant positive associations between PTSD symptom severity and intrapersonal ER difficulties, efficacy in regulating negative emotions with others, tendency to regulate positive emotions with others, demand/withdraw communication patterns, and craving following conflict when controlling for age, gender, relationship satisfaction, and past-year substance use frequency. Results suggest the potential utility of targeting ER and interpersonal communication styles to reduce substance use among individuals with PTSD-SUD in romantic relationships.

Women are more at risk of exposure to physical and sexual violence (Hedtke et al., 2008), and women who have experienced interpersonal violence are at heightened risk of developing several mental health conditions including posttraumatic stress disorder (PTSD), and substance use disorders (SUDs; Acierno et al., 1999; McGruder-Johnson et al., 2000; Messman-Moore & Bhuptani, 2017). Treatment outcomes for individuals with co-occurring PTSD and SUDs tend to be worse than for those diagnosed with PTSD alone (Schäfer & Najavits, 2007), and outcome results for trauma-informed treatments for co-occurring PTSD and SUD are mixed. There is clearly a need for improvement in effectiveness of treatment and applying a dimensional approach to understanding PTSD symptoms could provide valuable insight. The focus of the present study was to explore interrelationships among PTSD symptoms by applying Individual Differences Scaling (INDSCAL) to data from the Women, Co-occurring Disorders, and Violence Study (WCDVS). Primary aims were to identify organizing dimensions for symptoms and evaluate changes in dimension emphasis in relation to treatment outcome for PTSD, alcohol symptoms, and drug-related symptoms. Women who responded positively to treatment such that they no longer met PTSD criteria at six-months placed less emphasis on arousal, re-experiencing trauma content, and threat vigilance. Women who successfully reduced alcohol and drug symptoms placed more emphasis on arousal, re-experiencing trauma content, and threat vigilance. These findings suggest trauma-informed care may reduce PTSD symptoms, however, individuals who experience a reduction in alcohol or drug-related symptoms may continue to have difficulties managing arousal, re-experiencing trauma, and perceiving threats in their environment. These findings underscore the need to treat co-occurring PTSD and substance use symptoms concurrently and highlight the importance of creating new treatment protocols and programs to target both disorders.

Book excerpt: The experience of trauma (e.g., interpersonal violence, sexual assault) is associated with a range of adverse outcomes, including an increased risk for misusing substances (Ullman et al., 2013), often in an attempt to cope (Asberg & Renk, 2012; Hogarth et al., 2019). However, the association between trauma symptoms and substance use consequences might be explained by other processes that could be targeted for intervention. For example, paying attention to inner body sensations (interoceptive body awareness; IBA) predicts lower Post-Traumatic Stress Disorder (PTSD) symptoms (Reinhardt et al., 2020), but IBA has not been examined in relation to adverse substance use consequences. Thus, the present study examined associations among trauma symptoms, IBA, and substance use consequences. Specifically, we hypothesized that higher levels of trauma symptoms would correspond with less IBA and, in turn, more frequent adverse consequences of substance use. Moreover, we examined the assertion that interoceptive body awareness would mediate the association between trauma symptoms and problematic substance use. Participants (N = 271) age 18-years or older were recruited from various online sources (Reddit, Facebook) and a university research participant pool (SONA). Results suggest that trauma symptom severity was associated with less interoception and more substance use consequences, while interoceptive awareness was inversely associated with substance use consequences. However, interoceptive awareness was not a significant mediator for this relationship once the effect of trauma symptoms was accounted for. This is the first study to examine IBA as a correlate of substance use consequences to the best of our knowledge. Although IBA failed to mediate the effects of trauma symptoms on substance use outcomes in this convenience sample, future studies may examine the role of body awareness on substance among survivors of interpersonal violence, specifically. Findings highlight the importance of understanding trauma correlations (i.e., body awareness and substance misuse) and may inform treatment modalities for individuals at risk of substance misuse.
