Initially, this family consisted of ligands (insulin, IGF-I and IGF-m and receptors (the insulin receptor, the type I or IGF-I receptor and the type II or IGF-IYM-6-P receptor). Subsequently, it was discovered that six specific binding proteips (lGFBPs 1-6) playa major role in the actions of this growth factor family. In addition, there are now more potential receptors when one considers the possible roles of the insulin-receptor related receptor (IRR) and hybrid receptor dimers composed of insulin and IGF-I receptor (half-receptors). Another important aspect of this area of research is the realization that the IGFs are not only essential for normal growth and development but, in addition play an important role in the normal specialized function(s) of all tissues of the body, including the nervous system, skeleton, reproductive system, kidney, and the immune system, to name but a few. The development of recombi!tant human IGF-I for clinical testing has been a major breakthrough for investigators. Potential uses include wound healing, reversal of catabolic states, diabetes, bone remodeling, recovery from acute renal failure and many others. will determine both its use and its potential hazards.

Book excerpt: "The insulin-like growth factor-II/mannose-6-phosphate (IGF-II/M6P) receptor is a multifunctional single transmembrane glycoprotein which mediates the trafficking of M6P-containing lysosomal enzymes from the trans-Golgi network to endosomes-lysosomes (EL). IGF-II/M6P receptors located at the cell surface also function in the internalization and subsequent degradation or activation of extracellular IGF-II and other M6P-bearing ligands. However, very little is currently known about the significance of the receptor in the function of the central nervous system. Results from this thesis project indicate that IGF-II/M6P receptors are widely, but selectively distributed throughout the adult rat brain and spinal cord, and that a subset of receptors is found to be expressed in cholinergic neurons/fibers. We have also found that the IGF-II/M6P receptor in the rat brain couples to a G protein and that its activation by Leu27IGF-II, an analog which binds preferentially to the IGF-II/M6P receptor, potentiates acetylcholine release from the adult rat hippocampal formation. Additionally, we have shown that Leu27IGF-II can cause a significant reduction in whole-cell currents and depolarization of dissociated basal forebrain cholinergic neurons. These effects are mediated by a pertussis toxin-sensitive, protein kinase Ca-dependent pathway. Furthermore, we have also demonstrated that selective in vivo degeneration of basal forebrain cholinergic neurons by 192 IgG-saporin, which is mediated in part by an increase in glycogen sythase kinase-3beta activity, results in an up-regulation of IGF-II/M6P receptor levels in surviving neurons of the basal forebrain and frontal cortex. This is accompanied by a parallel time-dependent increase in other EL proteins, including Rab5, LAMP2 and the lysosomal hydrolase cathepsin D. Given the critical role of the EL system in regulating cell viability, it is likely that the increase in the level of the IGF-II/M6P receptor and other components of the EL system in surviving neurons represents an adaptive mechanism to restore the metabolic and structural abnormalities which follow 192 IgG-saporin-induced toxicity. We also report that IGF-II/M6P receptors levels are decreased in the hippocampus of Alzheimer disease (AD) brains as a function of apolipoprotein epsilon4-allele number. Moreover, immunoreactive IGF-II/M6P receptors are found to colocalize with a subset of beta-amyloid-positive neuritic plaques and tau-positive neurofibrillary tangles in the frontal cortex and hippocampus of the AD brain. Taken together, these results suggest that IGF-II/M6P receptors which are widely expressed in the adult brain, can serve a multifunctional role, including the maintenance of cellular homeostasis, modulation of neurotransmitter release and possibly in the restoration of metabolic and structural abnormalities in neurons which survive toxicity/injury." --

It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.
