Book excerpt: Family planning programs have been highly successful over the past 30 years in providing women in developing countries with desired access to contraceptive services and helping to reduce fertility rates. Notwithstanding this success, there is still an urgent need for these programs. The world's population is increasing, with annual population growth still approximately 80 million people. Nearly all of this growth is occurring in developing nations, where fertility rates remain relatively high. This high fertility runs counter to the preferences expressed by millions of women, who actually want to have smaller families. Family planning programs are also desirable because they are associated with a range of other benefits, most notably improvements in women's and children's health. Host countries provide about 60 to 75 percent of funding for family planning. However, funding and technical assistance from donor nations, especially the United States, have been crucial to the past success of family planning programs and are equally important for strengthening and expanding program efforts to meet future challenges.

Book excerpt: This open access book provides an overview and analysis of the causes and consequences of the massive and highly consequential transition in reproductive behaviour that occurred in Asia, Latin America, and Africa since the mid-20th century. In the 1950s contraceptive use was rare and women typically spend most of their reproductive years bearing and rearing children. By 2020 fertility and contraceptive use in Asia and Latin America reached levels commonly observed in the developed world. Africa’s fertility is still high, but transitions have started in all countries. This monograph is the first to provide a comprehensive analysis of these trends and their determinants, covering changes in reproductive behaviour (e.g., use of contraception and abortion), preferences (e.g., desire to limit and space births) and the role of socioeconomic development (e.g., education). The role of government policies and in particular family planning programs is discussed in depth. Particular attention is given to provide a balanced assessment of several political and scientific controversies that have beset the field. As such this book provides an interesting read for a wide audience of undergraduate and graduate students, researchers, and public health policy makers.

This paper reviews that controversy over the role these programs have played in reducing fertility. Opposing views on a number of contentious issues (for example, the significance of unmet need and unwanted fertility) are summarized and a consensus position is presented. Surprisingly, recent estimates of the fertility reductions achievable through the implementation of strong family planning programs differ only modestly--from 1 to 1.4 births per woman. Since only a small proportion of countries have implemented strong programs, the (unweighted) average impact of programs in 1985-90 is estimated at only 0.33 births per woman. However, the (weighted) average, which gives the program impact for the developing world as a whole, is estimated at 0.96 births per woman in the late 1980s. This suggests that programs have been responsible for about 43% of the fertility decline in the developing world between 1960-65 and 1985-90.

Book excerpt: The 1960s saw the start of a sustained process of declining fertility in Latin America resulting from radical social, attitudinal, and economic changes. There has been a clear trend towards more or less generalized behaviour of regulating fertility, coincident with a rise in the availability and use of methods of contraception. There are, however, important differences between and within the countries of the region, which are analysed in full in this volume. Whether one stratifies by demographic factors, place of residence, education, or social status, from the beginning of the transition, it seems that a variety of reproductive patterns were in existence. It is also clear that the process of transition is not yet complete and that in some important social groups, fertility is still high. This volume studies the process of transition from high to low fertility as it has occurred and is occurring in Latin America. It provides a general comparative overview of transition in the region in which the link between socio-economic development and declining fertility is explored. There are sections on the process through which the transition occurs, social determinants of fertility change, and the consequences of fertility decline. Large data sets from census and survey results for many countries and points in time are presented in over 150 tables and figures. The comparative analyses are complemented by five individual country studies in the final section.
