Book excerpt: The association between impulsivity and substance abuse has important implications in the clinical substance abuse treatment field. Adolescents who are characteristically more impulsive by nature of their developmental may be more at risk for treatment failure. The inclination by which individuals devalue future rewards in favor of more immediate rewards with lower monetary values is a type of impulsivity that has received much attention in the substance abuse research field. Identified as temporal discounting (TD) or delay discounting, this phenomenon is not only a factor that influences substance abuse, but also may be a factor that leads to failure in substance abuse treatment. Another important factor that has been shown to influence treatment outcome is the therapeutic relationship between a provider and a client. The purpose of this study was to examine the relationship between TD, working alliance and treatment experiences among substance-abusing adolescents enrolled in outpatient drug abuse treatment. For this study, a descriptive within-subjects repeated measures design was used. Data was collected from a convenience sample of Hispanic adolescents 14-17 years of age meeting DSM-IV-TR criteria for substance abuse or dependence. Participants were recruited from a larger 60 month National Institute on Drug Abuse funded parent study that examined Culturally Informed and Flexible Family-based Treatment of Adolescents and Traditional Family Therapy among adolescents. Variables included participant age, working alliance, whether participants attended weekly therapy sessions for at least the first 5 weeks (retention), number of sessions in which subjects participated in therapy 8 weeks after enrollment (dosage), and the change in TD from the time of enrollment and (T1) to approximately 5 weeks after enrollment T2). Working alliance was measured at 5 weeks after initiation of therapy. It was hypothesized that TD would be inversely related to age, inversely related to working alliance, inversely related to treatment retention after 5 weeks of therapy, inversely related to the number of therapy session in the first 8 weeks of therapy, and that TD would decrease after 5 weeks of therapy. Statistical analysis including linear and logistic regressions as well as analysis of variance was used. No significant relationships were found between TD and the independent variables of age, treatment retention, dosage, the change is TD from T1 to T2, and working alliance. Although statistically significant findings were not achieved in this study, the research has clinical relevance. Analyzing treatment methodologies and differences in alliance and retention among a Hispanic adolescent population provides data that has not heretofore been reviewed. Impulsivity is linked to risk behaviors. Further research is needed to identify how treatment methodologies may be refined to address temporal discounting and impulsivity among Hispanic adolescents thereby improving treatment retention and decreasing the substance abuse to improve long term outcomes among this vulnerable population.

Hispanic adolescents in substance abuse treatment programs have been found to differ from non-Hispanic Caucasian adolescents in the relationship of their delinquent behavior to psychoactive substance involvement. This study examined differences in posttreatment substance use and delinquent behaviors in Mexican American and non-Hispanic Caucasian adolescents during the initial six months following treatment for substance abuse. In addition, the impact of acculturation and cultural specific stress on the outcomes of Mexican American substance abusing adolescents was explored. The study included 40 Mexican American and 230 non-Hispanic Caucasian teens entering inpatient adolescent alcohol and drug treatment programs. Participants were interviewed during the treatment program and at six months following discharge. A resource person was interviewed as a corroborative informant. The Customary Drinking and Drug Use Record (CDDR), the Relapse Review and the Conduct Disorder Questionnaire (CDQ) were administered to assess psychoactive substance involvement, aftercare meeting attendance and delinquent behavior over the six month period of the study. Mexican American subjects also completed the Acculturation Rating Scale for Mexican Americans (ARSMA-II) and a subscale of the Hispanic Stress Inventory (HSI) measuring cultural and family conflict. Hierarchical multiple regression was used to test the interaction between ethnicity and delinquency in predicting posttreatment substance use. The relationship between acculturation and cultural stress on posttreatment outcome was also explored using regression and correlation. While Mexican American and Caucasian adolescents demonstrated similar rates of relapse and abstinence during the six months following substance abuse treatment, Mexican American adolescents had more intense substance use patterns and more negative consequences for alcohol and drug relapse. Additionally, compared to Caucasian youth, Mexican American adolescents displayed a stronger relationship between delinquent behavior and substance use intensity during the six months after treatment. Acculturation had an influence on outcomes for Mexican American adolescents, with Anglo orientation negatively correlated with posttreatment substance use, and overall acculturation related to likelihood to attend AA/NA meetings.

In this study, two groups of participants were formed: the at risk for substance abuse (ARSA) group consisted of participants who indicated 'almost always, ' 'frequently, ' 'sometimes, ' or 'rarely' on either of two items on the Outcome Questionnaire-45.2 (OQ-45.2) (i.e., the eye-opener item: "After heavy drinking, I need a drink the next morning to get going" and the annoyed item: "I feel annoyed by people who criticize my drinking (or drug use)"). The non-ARSA group consisted of participants who indicated 'never' on both of the eye-opener and annoyed screening items on the OQ-45.2. Data available from a counselor-training center for a client participant sample (n = 68) was used. As part of the usual counselor training center procedures, clients completed questionnaires after their weekly counseling session. The measures included the Working Alliance Inventory and the OQ-45.2. Results revealed no significant differences between the ARSA and non-ARSA groups in working alliance, total outcome symptomology, or in any of the three subscales of symptomatology. Working alliance was not found to be significant in predicting outcome symptomatology in this sample and no moderation effect of substance use on the relationship between working alliance and outcome symptomatology was found. This study was a start into the exploration of the role of substance use in the relationship between working alliance and outcome symptomatology in individual psychotherapy. Further research should be conducted to better understand substance use populations in individual psychotherapy.
