Book excerpt: The purpose of this study was to determine if the level of Nurse Manager (NM) emotional intelligence (EI) predicted registered nurse (RN) job satisfaction and RN perceptions of the practice environment. In addition, relationships to patient, nursing, and hospital outcomes were explored. Participants included RNs (N=659) and NMs (N=38) from 53 nursing units at eight hospitals located in the southeast region of the United States. A cross-sectional, correlational research design was used to test the hypotheses. Pearson product-moment correlation coefficients, simple linear and multiple regression statistics were conducted to analyze the data. Level of NM EI had a positive, not significant relationshp to RN job satisfaction and RN perceptions of the practice environment. A direct, positive significant relationship was observed between the variables NM EI and patient satisfaction with nursing care. There was a positive, significant relationship noted between the variables RN job satisfaction and RN perceptions of the practice environment. The indirect relationships between the level of NM EI and patient, nursing and hospital outcomes were not significant. There was a direct significant, positive relationship noted between the variables RN perceptions of the practice environment and patient satisfaction with nursing care. In addition, the interaction between RN job satisfaction and RN hours of care had a positive, significant relationship with unit level pressure ulcer rates. This study indicated that units with higher RN hours of care have increased pressure ulcer rates. In addition, results illustrate a marked increase in pressure ulcer rates on those units with higher levels of job satisfaction. In this study, pressure ulcer rates depended on the level of RN job satisfaction.

Book excerpt: There is evidence that the increase in patients per nurse ratio and job dissatisfaction are leading causes of nurse burnout in the United States. Current research mostly focuses on the hospital setting which limits generalization of findings. Therefore, more research is needed beyond the hospital environment. The purpose of this MSN thesis was to examine the relationship between job satisfaction and nurse-patient ratio with nurse burnout among home health nurses. The hypothesis is that home health nurses who experience a higher nurse to patient ratio and greater job dissatisfaction will experience increased nurse burnout. A review of literature helped to gain an appreciation for the current research that has been conducted in this area of interest, and also revealed the need for further research in the home health setting. This study utilized convenience sampling and a descriptive correlational design among a group of home health nurses in the Piedmont Region of North Carolina. Results from the MJS survey and nurse-patient ratio were analyzed with the results from the Maslach Burnout Inventory-Human Services Survey (MBI-HSS) to determine if a relationship exists. The results revealed that some of the Measure of Job Satisfaction (MJS) subscales appeared to be better predictors of overall job satisfaction, and there does appear to be a positive association between nurse-patient ratio and job satisfaction. In regards to nurse burnout, only one out of the 10 subjects showed results that suggested high nurse burnout. Conclusive relationships cannot be established between nurse-patient ratio and burnout or job satisfaction and burnout among home health nurses. Areas of improvement in this type of research have been reviewed to further the knowledge available to those who will conduct research in the future on this imperative issue.

Book excerpt: Nurses are leaving the profession due to high levels of job dissatisfaction arising from current working conditions characterized by heavy workloads, limited participation in decision making and lack of development opportunities (Canadian Health Services Research Foundation [CHSRF], 2006a). To gain organizational support for workplace improvements and thereby improve nursing retention, evidence is needed to demonstrate the impact of the work environment on patient care. The purpose of this study was to determine the relationship between nurses' perceptions of their work environment and the quality and risk outcomes for both the patient and the nurse. Kanter's (1977, 1993) theory of structural empowerment guided the study. Empowering work environments for nurses were hypothesized to impact group processes and thereby work effectiveness as reflected in patient outcomes (patient satisfaction, therapeutic self care, falls and nurse-assessed risks). Empowering workplaces were also hypothesized to enhance the nurse's psychological empowerment and, in turn, engagement in empowering behaviours that lead to quality care and job satisfaction. A multi-level cross-sectional design was used to test the study model. Self-report surveys were used for a sample of nurses (n=679) and discharged patients (n=1005) affiliated with medical and surgical units from 21 hospitals in Ontario. Unit characteristics and falls data were obtained from existing hospital databases. Using multilevel structural equation modeling, the hypothesized model fit well with the data (2=21.074, df=10, CFI=.985, TLI=.921, RMSEA=.041, SRMR .002[within] and .054[between]). Empowering workplaces had positive effects on nurse-assessed quality of care and predicted fewer falls and nurse-assessed risks as mediated through group processes. These conditions positively impacted individual psychological empowerment which, in turn, had significant direct effects on empowered behaviour, job satisfaction and care quality. Theoretically, evidence supported the further evolution of structural empowerment theory to include group processes and empowered behaviour as mediators to various nurse and patient outcomes. The evidence from this study also reinforced the critical need to invest in improving nursing work environments for the benefit of patients and nurses. Theory-informed strategies for changes to the workplace have the potential to mitigate against projected nursing shortages and ensure a sustainable workforce to meet future demands for care.

Book excerpt: "This study examined the intercorrelations among nursing home healthcare providers' stress, job distress, job satisfaction, and mood, as well as the relationships between these factors and patient outcomes. Additionally, this study investigated the impact Butler University's Music First! program had on nurse job perceptions. A total of 63 nurses from 9 long-term care facilities completed questionnaires asking about stress, job distress, and global job satisfaction. Nurses' depression, anxiety and hostility were also assessed with a 15-item Profile of Mood States questionnaire. Of these 63 nurses, 20 completed the questionnaires at three different time points during the implementation of the Music First! intervention. Patient outcomes included standardized assessments of agitation, psychiatric symptoms, cognitive functioning and depression, as well as documented instances of patient falls and reportable behaviors. Intercorrelations among nurse stress, job distress, job satisfaction, and affect ranged from r = -0.565 to r = 0.627. All correlations except one reached statistical significance. Similarly, correlations between patient outcomes and nurses' job-related self-reports documented strong relationships between nurses' affective responses to their jobs and patient outcomes. For example, the patients of nurses reporting greater stress at work were more agitated (r = 0.335) and exhibited more severe dementia (SLUMS r = -0.291), more severe psychiatric symptoms (r = 0.293), and more frequent reportable behaviors (r = 0.436) than patients of nurses reporting less stress. Finally, the Music First! program was associated with a near significant decrease in nurses' job-related stress (p = 0.054). Although these results are correlational and cannot determine the direction of these relationships, these findings do suggest that improving nurses' feelings about their jobs, improving patient behaviors or implementing programs designed to improve patients' quality of life can have a positive impact on the nursing home environment"--Leaves 2-3.
