The goal of the current study was to determine whether a mediational model, based on Lazarus and Folkman's transactional model of stress and coping, or a mode rational model, based on Terry & Conway's goodness-of-fit hypothesis, was more appropriate to describe the relationships among perceived control, coping, and health. Regression analyses revealed that (a) emotion-focused coping mediated the relationships between perceived control, (b) perceived control also acted through an indirect effect on emotion-focused coping and health behavior to impact self-rated physical health, and (c) perceived control moderated the relationship between emotion-focused coping and psychological distress. These results provide partial support for both the transactional model and the goodness-of-fit hypothesis. Results are discussed in terms of the transactional model, goodness-of-fit hypothesis, optimal coping, and the importance of perceived control in psychological adjustments. Limitations and future research directions are discussed. Especially the potential role of patient education and its impact on perceptions of control are considered.

Book excerpt: Type 1 Diabetes (T1DM) is a life-long chronic illness often diagnosed during childhood or adolescence which requires individuals to adhere to a complex treatment regimen. Unfortunately, adherence is especially poor during adolescence. One factor which may influence adherence is treatment responsibility, or determining whether the parent or adolescent assumes treatment responsibility. Although there are no clear guidelines regarding when to transition responsibility, knowing whether the adolescent has the cognitive capacity or executive functions (EF) to successfully manage their treatment is essential. Parental stress has also been associated with treatment responsibility, suggesting that greater levels of parent responsibility are associated with greater levels of reported parental stress. However, most studies have failed to examine the effects of multiple forms of stress (i.e., general, parenting, and disease-related) on treatment responsibility. Further, little is known regarding how parent perception of adolescent general and disease related EF and various forms of parent stress are related to treatment responsibility. The present aimed to assess the contribution of various forms of stress and parent perception of general and diabetes related executive functioning on the transition of responsibility from parent to adolescent in adolescents with T1DM. The findings from the study underscore the role of parent reported diabetes distress as it relates to the transition of responsibility. Specifically, diabetes distress was a significant predictor of greater parent responsibility above and beyond the role of general and disease related executive functioning. Nonetheless, results suggest that parents transition greater responsibility to their adolescent when they perceive their adolescent to have a greater ability to remember tasks associated with their illness. General and perceived stress did not influence the transition of responsibility from parent to adolescent. The present study expanded previous research by examining the role that various forms of stress and parent perception of adolescent's executive function play in their decision to transition responsibility for diabetes management among adolescents with T1DM. Despite limitations within the present study, the findings suggest clinical implications and future directions for research. Specifically, future interventions should target decreasing parent diabetes distress, while also focusing interventions on improving adolescent's diabetes related memory for managing their disease. Future research should include a smaller age range and should be longitudinal in nature to better understand how the dynamic nature of the associations among various forms stress, executive functioning, and treatment responsibility may have an impact on treatment adherence and glycemic control.

Diabetes is the most common chronic illness among children in the United States (LaPorte, Matsushima, & Chang, 1995). Serious health complications can be avoided only by following complex treatment protocols involving a difficult balance of diet, medication, glucose testing, and exercise (Diabetes Control and Complications Trial Research Group, 1993). Prevalence of diabetes and poor outcomes are disproportionately higher among Latinos than other populations (Walsh, Katz, & Sechrest, 2002).Each aspect of diabetes treatment adherence is affected by psychosocial variables (Schwartz, 1982). Previous research on adherence has focused on the knowledge, attitudes, and beliefs of patients, rather than on exploring patients' direct experience (Vermeire, Hearnshaw, Van Royen, & Denekens, 2001). Cultural barriers to treatment adherence have been identified for pregnant and postpartum Latinas (Oomen, Owen, & Suggs, 1999), but studies of ecocultural variables relevant to adherence in adolescent males is lacking. The proposed is a phenomenological inquiry (Creswell, 1998) into the psychosocial barriers to treatment adherence through Latino adolescents' own voices and perceptions of their treatment for diabetes.The experience of 12 adolescent male Mexican-Americans diagnosed with diabetes will be analyzed based on semi-structured interviews. Participants will review the findings to provide a validity check.Greater knowledge of the barriers to adherence for Latino youth will provide a guide for tailoring interventions to increase their utilization. A deeper understanding of age and culturally specific variables that enhance or block medical treatment could also be used to improve the healthcare delivery system and to provide improved, culturally competent health care for these young people and their families.In order to administer and complete the study, funding is being sought from the W. T. Grant Foundation. The foundation requires applicants to complete and submit an application for the proposed study, including a narrative outlining the questions guiding the proposal, the study rationale and its relevance to the Foundation's mission, a literature review, the specific questions to be addressed, research methods and design, a data analysis plan, anticipated products, and a communication plan.
