Book excerpt: The healthcare community needs to be concerned about nursing turnover intention and the effect on the projected nursing shortage. The evaluation of nursing turnover intention and the association between turnover intention and various predictors needs to be determined. Job satisfaction and perceived stress have been identified to influence the intention to turnover. In addition, environmental factors (i.e. odor, noise, and light) may influence perceived stress and job satisfaction. This study tested theoretical relationships between the dependent variables of perceived stress, job satisfaction, and turnover intention and each of the independent variables of (a) odor, (b) noise, and (c) light. The sample was comprised of 116 full-time registered nurses who worked on the medical-surgical unit of a 588 bed Level 1 trauma center located in northern New Jersey. Hypothesis testing employed correlational and regression statistical techniques. Statistically significant relationships were found between perceived stress and job satisfaction (r =.549, p =.000), job satisfaction and turnover intention (r = .740, p = .000), perceived stress and turnover intention (r =.336, p =.000), and perceived level of odor and job satisfaction (r = -.272, p =.003). The relationships between perceived stress and job satisfaction, job satisfaction and turnover intention, and perceived stress and turnover intention were anticipated due to the strong theoretical and empirical evidence supporting these relationships. Although a specific hypothesis was not proposed to combine all independent variables to explain the dependent variable of turnover intention, all independent variables were added to a regression model using backward regression. Backward regression was used because none of the independent variables were strong predictors of turnover intention. The environmental factors did not contribute significantly to the variance in turnover intention. Likewise, perceived stress offered little contribution. Finally, level of light and job satisfaction, together, explained 56% of the variance in turnover intention.

To identify the factors of job satisfaction and stress of nurses and their association with turnover intention rate in an acute hospital in Hong Kong. Background. Heavy workload, shortage of manpower and high turnover rate of nurse are the major problems in Hong Kong particularly in public hospitals even after measures have been taken to tackle them in recent years. Many studies in western countries showed that workload, turnover rate, stress, satisfaction level and turnover intention of nurse were associated. However there are few studies in Asian countries. This information is needed to provide policymakers evidence to formulate particular policy to deal with these problems. Design. This is a cross sectional study. Survey data were collected from nurses in the medical ward, operation theatre and intensive care unit of Pamela Youde Nethersole Eastern Hospital in Hong Kong. Methods. A snowball convenience sampling method was used to collect samples from the selected departments. My colleagues in the select departments referred participants to me. Packet of questionnaire was distributed to the participants and the questionnaire was self - administered .The survey questionnaires, including the Expanded Nursing Stress Scale (ENSS), Job Satisfaction Scale (JSS) and Anticipated Turnover Scale (ATS), were used to collect data from the sampled subjects. Results. Mean scores of ENSS, JSS and ATS of the respondents were 140.77 out of 228, 114.12 out of 216 and 49.48 out of 84 respectively. These indicated that respondents had high stress and high intention to leave level but mild satisfaction level. It also revealed that there was inverse relationship between stress and satisfaction level (r = -0.23, 95% CI: -0.46 to -0.02) and between satisfaction level and intention to leave level (-0.41, 95% CI: -0.62 to -0.19). Conclusion. There is some evidence of high stress level and intention to leave in nurses working in public hospital while their satisfaction level is in a moderate level. Stress level, satisfaction level and intention to leave level in nurses are associated. The management should adopt an evidence - based approach in improving the work environment for nurses. Even though the generalization of this study is weak, it is useful for the hospital to deal with the human resource problems of nurses. The work environment impacts nurses’ perception and loyalty to organizations based on satisfaction versus dissatisfaction, including quality patient care. The purpose of this study was to explore work environmental factors that affect nurse job satisfaction in a hospital setting. Marilyn Ray’s Theory of Bureaucratic Caring was the theoretical framework for this study. The research utilized the McCloskey/Mueller Satisfaction Scale (MMSS) to examine factors in an acute care hospital associated with nurse job satisfaction. The study invited 130 nurses from a hospital in rural South Carolina to participate electronically via a link provided through their work email accounts. The sample included a total of 23 participants that completed the survey, including registered and licensed practical nurses (RNs & LPNs). The study revealed no significant work environmental factors that affect nurse job satisfaction. Results of the study indicated that nurses were neither satisfied nor dissatisfied with their current jobs (M=3.35). Participants rated the factors that affect job satisfaction in the following order from very satisfied to least: scheduling, co-workers, interaction opportunities, extrinsic rewards, praise and recognition, balance and family, professional opportunities, and control and responsibility.

Job satisfaction has long been correlated with retention of RNs and the work environment has more recently emerged as an important factor in retention of RNs. Positive work ability, perceived health and psychological work-related factors, including reward incentives, work environment, job autonomy, and job satisfaction are significantly associated with nurse intentions to continue working, instead of seeking alternative employment or retiring but these factors have not been studied among older RNs. Successful aging has been influential in the retention of workers in the business arena. The combination of these three concepts, job satisfaction, work environment, and successful aging, and how they relate to retirement is particularly significant in face of the current and continued nursing shortage in the United States and around the world. As nurses age there is a suggested difference between older and younger nurses' ability to work and this difference could affect decisions made to remain on the job. Common stereotypes specific to older workers may lead to an overall disinterest about retaining older workers by human resource personnel and possible discrimination when hiring, workplace education and layoffs of older nurses. One clear priority towards older nurses is to redress employer attitudes on the subject of older workers and their ability to work. This research sought to find ways that change rather than entrench seemingly inappropriate stereotypes of older workers. Understanding older RN's decisions on retirement in terms of the multi-faceted topics of job satisfaction, work environment, and successful aging contributes to the development of strategies important to the decision to stay or delay retirement of older nurses for human resource departments.

Book excerpt: Background: Public health nurses (PHN) work long hours, carry high caseloads, manage with fewer resources, and must deal with increasing technology while ensuring the provision of quality care. Since the time of Lillian Wald, public health nursing has undergone a major transformation in the types of services provided in the community due to the complexity and acuity of the current patient population. PHNs play a significant role in helping patients to effectively manage their illness in order to prevent unplanned hospitalizations. Illness, absenteeism, turnover, and poor job satisfaction are end products of stress. The purpose of this DNP project was to examine the impact of care coordination and caseload on job satisfaction and stress on the PHNs who are providing care to patients in the community. Methods: Descriptive study using mixed methods of data collection and analysis. A purposive sample of nurses who have had experience working in home care and who have provided skilled nursing care for a minimum of one year completed self-reported surveys. Perceived stress was measured with the Expanded Nursing Stress Scale. Job satisfaction was assessed using the Measurement of Job Satisfaction Tool. Analysis of the data collected from the surveys included descriptive and inferential statistics. Correlation coefficients, two-sample t-tests and content analysis were used to summarize the data and compare group differences between nurses with different caseloads. Results: However, due to a small sample size, the correlation and t test results showed inconsistency with content analysis. Content analysis provided more insight and represented more accurately the characteristics of a small sample size. Conclusion/ Clinical relevance: There exists a gap in the research regarding the impact of these factors on PHN and additional research is needed to explore the relationships between work conditions and PHN stress and impact on patient care. The results of this study support the idea that high caseloads impact job satisfaction and stress levels of nurses working in public health. Implementing changes to improve work conditions would provide PHNs with the time needed to focus on quality patient care which would ultimately lead to improved patient outcomes. In addition, the data collected from this project could influence change as it relates to the current model of care delivery in public health.

Book excerpt: ABSTRACT: The purpose of this study was to explore the relationship of the Clinical Nurse Leader (CNL) (AACN) role with the variables of work related stress, quality of life, job satisfaction and anticipated turnover of acute care nurses. Participants included registered nurses (RNs) (N= 94) in Florida recruited from 3 (not for profit) Magnet hospitals in the Tampa Bay Florida area. An ex post facto design was used to test the hypotheses of this study; independent t-tests compared RN's responses on survey tools measuring work-related stress, quality of life, job satisfaction, and anticipated turnover. Multiple regression analysis was used to examine the interrelationships among these variables. RNs (N=94) completed five survey instruments, including a researcher-developed demographic form. The results of the study showed Aim1 which explored work- related stress did not show any statistical difference between the two groups. Aim 2 which explored job satisfaction and quality of life did not show a difference in the two groups when total scores were analyzed. However, the mental health subscale of the Sf-36(quality of life) was significant (p=.021), and the general health subscale of the Sf-36 trended toward the CNL group reporting better general health (p=.080). This study revealed that Aim 3 which explored anticipated turnover was statistically significant (p=.047). Standard multiple regression showed a significant relationship existed between CNLs, work related stress and anticipated turnover. The significance of implementation of the CNL role in decreasing turnover through a relationship with these variables may have an important impact on the nursing profession. Specifically, economic implications vii in reducing turnover that bear further exploration and improving the nursing work environment. This research is the first study to explore the CNL role in relation to these variables.

The descriptive method of research was used in this study. To gather primary data, the researcher administered the Maslach Burnout Inventory Survey (MBI-HSS) and the Work Environment Scale inventory survey questionnaires to thirteen nurses in the area. The perceptions of the respondents on dimensions in their work environment are positive with respect to involvement in their jobs, peer or coworker cohesion, supervisor support, autonomy, work pressure, clarity of rules and policies, and managerial control. Their perceptions are equivocal with respect to task orientation, innovation, and physical comfort. The respondents report a moderate degree of burnout feelings in terms of emotional exhaustion; they have not experienced burnout feelings in terms of depersonalization; and they report having moderate level of feelings of professional accomplishment. There is no significant difference in the perceptions of the respondents when grouped according to age, marital status, educational attainment, and years of experience as a registered nurse. Book excerpt: The purpose of the study was to perform a quantitative occupational stress risk assessment (OSRA) to evaluate the impact of health promotion (HP) lifestyle and perceived nature contact on the consequences of perceived stress from exposure to nursing stress risk factors. Perceived stress is defined as the employee’s personal interpretation of his or her work circumstance and any discrepancy between workplace demands (stressors) and individual capacities (resources). Registered nurses (RNs) in patient care settings are prone to high levels of perceived stress, which can cause errors in clinical care giving. High perceived stress is a consequence of risk factors from a nurse’s daily work routine including: workload, death and dying, inadequate preparation for role, staff support, uncertainty concerning treatment, communication with physicians, and communication with other nurses. Mitigating perceived stress improves nursing job performance, job satisfaction, and overall employee health, while reducing mental fatigue, absenteeism, and burnout. Methods performed included: Aim 1) a systematic literature and meta-analysis to establish benchmarks for perceived stress in nursing professionals for comparison of United States (US) and International healthcare settings; Aim 2) a cross-sectional study using a 125-item survey instrument administered online to an RN study population to evaluate relationships between perceived stress, HP lifestyle, and perceived nature contact; and Aim 3) a quantitative OSRA to rank nursing stress risk factors and examine differences by gender, ethnicity, and race. Results were: Aim 1) Perceived Stress Scale 14-item (PSS14) value for International nursing studies was 25.41 (95% CI 21.1 to 29.7), while US PSS14 nursing studies yielded a value of 23.8 (95% CI 19.8 to 27.7). Book excerpt: The health care dilemma of increasing patient acuity coupled with decreasing reimbursement, is forcing acute care agencies to explore new ways to increase productivity. Work redesign in the hospital setting is one strategic response to this dilemma. The restructuring of health care delivery systems and the redesign of nursing roles is creating dramatic changes in the work environment for nurses. Empirical evidence supports a relationship between work environment and job satisfaction of nurses. A successfully redesigned system is one that meets its' refined goals and objectives and makes its visions a reality (Spitzer-Lehman & Flarey, 1995). Therefore, the purpose of this study was to measure perceived RN satisfaction with the work environment pre and post work redesign. This study used a non-experimental, descriptive design. Registered nurses who work on a 40-bed medical telemetry unit in a large midwestern teaching hospital were identified for this study. A 38-item questionnaire was given prior to implementation of work redesign by another researcher and repeated 18 months after implementation by this researcher. The Work Quality Index (Whitley & Putzier,1994) contained six subscales: Professional work environment, autonomy, work worth, professional relationships, role enactment, and benefits. Demographic profiles were also obtained. Measures of central tendency and t-tests were employed to answer the research questions. Mean scores for the defined variables of RN satisfaction revealed the absence of statistically significant results, but did reflect a positive perception of work environment satisfaction pre and post work redesign. Discussion of subscales is included along with percentile rankings. The findings support empowerment/autonomy as being a positive marker for satisfaction. Work relationships with peers was also identified as being highly satisfying. Professional work relationships with physicians was identified as an area for further investigation. The results of this study provided objective data for benchmarking and for addressing unit based issues, as well as areas for improvement. Replication of the study using a multi-unit design would allow greater generalizability of the findings as well as give a more accurate measure of outcomes of the organizational redesign.
