Book excerpt: Abstract: Caucasians and African-Americans with type 2 diabetes continue to experience preventable diabetes-related life-threatening complications despite medical advances and health care availability. African-Americans are more likely to suffer from diabetes-related complications and experience a higher diabetes-related mortality rate than Caucasians. There is a definite need to better understand the relationship of psychosocial variables to diabetes control in these two racial groups. The purpose of this two-group, comparative, descriptive study was to examine the relationships of social support, self-efficacy, and outcome expectations (beliefs that diabetes-related behaviors will lead to certain outcomes) to self-care behaviors and glycemic control in Caucasian and African-American adults with type 2 diabetes. A convenience sample of 91 Caucasian and African-American subjects receiving health care at three outpatient facilities participated in the study. All subjects completed four self-report measures: Social Support Questionnaire, Self-Efficacy Questionnaire, Outcome Expectancy Questionnaire, and the Diabetes Activities Questionnaire at the time of the outpatient visit. Long-term glycemic control was assessed by glycosylated hemoglobin analyses at the time of the outpatient visit. Two-sample t-tests revealed no significant differences between the two racial groups in regards to age (p = .81), duration of diabetes (p = .69), and glycosylated hemoglobin (p = .85). Book excerpt: Self-care is the key to living a long and healthy life for people with diabetes. Yet numerous studies show that self-care is far from optimal. This has resulted in attempts to understand the progress underlying self-care behavior in the efforts of mediating more effectively. While there are an abundance of studies focusing on African-American women and diabetes management, there is a considerable gap in health education literature regarding the self-care management experiences of African-American men living with type 2 diabetes. The management and impact of type 2 diabetes on the sense of self, lifestyle, and significant others of the African-American man are not clearly comprehended. Therefore, the purpose of the study was to explore the knowledge, beliefs, and self-care management practices of African American men living with type 2 diabetes. This was an exploratory study utilizing qualitative methodology to understand the knowledge, beliefs, and existing self-care management practices of up to 50 African-American men, ages 18-70, living with type 2 diabetes. Participants had one-on-one semi structured interviews with the primary investigator. The instruments that were used consisted of two parts: one was a self-administered paper/pencil questionnaire to collect socio-demographic information. The second was a semi-structured interview. The study significance was to identify and understand the barriers to type 2 diabetes management and how they had an adverse influence on self-care. A combination of high barriers, poor daily self-care, management, and knowledge possibly put those with type 2 diabetes at an elevated risk for subordinate self-care management. Barriers had a role in reality and diabetes researchers and educators are charged with recognizing the complications of what individuals with type 2 diabetes need to do improve and enhance their health. The study findings revealed that the participants labored with managing their diabetes and some counted on their own potential or performance, as well as the encouragement of their family, extended family, neighbors, and friends. Also, many of the participants had a unified theme in that regarding self-care management, having type 2 diabetes affected those in their surrounding environment, not just the person living with the illness. In conclusion, the study findings suggest that future diabetes research and education among African-American men should give attention to male masculinity and the powerful influence it has on utilizing preventive health services. The limited amount of African-American men included in empirical type 2 diabetes research could help explain the under addressed barriers and complexities to positive self-care management. Book excerpt: This study was conducted to explore the barriers socioeconomically disadvantaged African American adults encounter in managing type 2 diabetes and the specific day-to-day strategies they employ to surmount them. Theoretical Framework. Four perspectives informed all aspects of this study, from crafting the research questions through interpretation of results: The agency vis-a-vis structure tension; Marxist/Conflict Theory; Constructivist Grounded Theory as explicated by Charmaz (2006), and several Social Ecological health models. Methods. Analysis was based on direct observation of 15 doctor/patient office visits and subsequent personal interviews. The patients were scheduled for routine follow-up visits and were selected from the panels of four physicians practicing at an exemplar (relatively high diabetes quality of care scores), safety-net clinic (>50% of patients on Medicaid or uninsured) in Northeast Ohio. Results. The most salient barriers patients faced in managing diabetes were economic, job, and insurance-related in nature; and these obstacles influenced all their health care decisions and behaviors. Other barriers included: managing comorbidities, food insecurity, scarcity of affordable exercise options, neighborhood crime, transportation11issues. Management strategies included: conferring regularly with their physicians; seeking the support of family, friends, and religion; going to a safety-net clinic; "listening to my body"; using trial and error to make self-management decisions; becoming informed; using available community resources; setting goals; and routinizing a diabetes regimen.Conclusion. Socioeconomically disadvantaged African American adults can successfully manage diabetes given the requisite resources: culturally competent, caring physicians and staff; the services of a safety-net healthcare system; public financial aid; information and education; and family and community support.Implications. This study is an exemplar for health stakeholders who seek to foster productive doctor/patient relationships and to improve diabetes outcomes of socioeconomically disadvantaged populations. Study findings suggest several policy implications: increasing the time for routine visits with complex patients; incentivizing caring and meaningful doctor/patient relationships over quantitative "throughput" measures; incentivizing integrative whole person care; systematically assessing patients' social environmental concerns; and offering medical students mentored experiences in international health to foster altruistic caring (Bolen, Sage, Perzynski, & Stange, 2015; Ferrer & Carrasco, 2010; Epstein et al, 2010; Berenson & Rich, 2010; Hong et al., 2010).

The purpose of this study was to assess diabetes knowledge and self-care activities and explore experiences of African-American women with type 2 diabetes mellitus (T2DM). This mixed method study used a quantitative correlational design and a qualitative descriptive design. The data was collected using surveys and semi-structured individual interviews and analyzed using SPSS 17.0 and Atlas ti 5.2. Data were transcribed verbatim and examined for emergent themes. The sample included fifty-two African-American women between the ages of 40-64 from the North Central Florida region. The mean age for the sample was 55 years. Approximately 55.8% of the participants had higher than the average knowledge for diabetes. Results showed no associations between the respondent's level of knowledge about diabetes and certain self-care activities. However there were positive significant correlations between knowledge about diabetes and the years of education (rho=.24) and some self-care activities namely (a) specific diet (r=.28) and (b) foot care (rho=.40).

The Denyes Self-Care Agency Instrument (DSCAI-90) and the Denyes Self-Care Practice Instrument (DSCPI-90) were used to survey a convenience sample of 40 adult African Americans with type 2 diabetes, living in the community. Findings revealed that self-care agency and self-care practice were present, and there was a positive correlation between the two concepts. The results of this study supported the usefulness of Orem's Self-Care Deficit Nursing Theory (SCDNT) with adult African American diabetic populations and identified the need for further research to examine self-care in other African American populations.

Book excerpt: Type 2 diabetes mellitus represents a daunting self-management challenge due to its complicated daily treatment regimen which includes adhering to diet, exercise, and medication recommendations. Little research has evaluated perceived barriers to diabetes self-care in a group that is at particular risk for diabetic complications: U.S. Latinos. This study had two aims. The first was to provide evidence that experimental and validated self-report instruments selected for this study functioned in a comparable manner in both Spanish and English in a sample of low income, predominantly Spanish-speaking diabetic patients (n = 77). The second aim was to examine hypothesized relationships of selected predictors to two criterion variables measuring health status (i.e., glycosylated hemoglobin A1c and Diabetes Impact) using a method described by Baron & Kenny (1986). Predictors included age, education, and income as covariates, and perceived environmental barriers to self-care, self-efficacy for treatment adherence, fatalistic beliefs (external health locus of control), and Latino cultural diabetes beliefs. Potential moderator and mediator variables of the relationship between perceived barriers to self care and criterion variables included self-efficacy for treatment adherence, fatalistic beliefs and Latino cultural diabetes beliefs. Scales used in this study had coefficient alpha values ranging from .82-.96 in Spanish and .74-.97 in English and also demonstrated strong factor structure integrity. Social influences are commonly associated with diabetes-related outcomes. Purpose: The purpose of the study was to assess whether and how socio-contextual factors influence self-management and emotional distress in urban African American adults living with type-2 diabetes, as well as to assess whether social support facilitate relatedness need fulfillment within this population. The study also sought to gain an understanding of the psychosocial support needs and preferences of urban African American adults living with type-2 diabetes. Methods: The descriptive study employed a mixed-methods research design. African American adults were recruited to take a one-time written survey. Hypotheses one and two were tested using hierarchical linear regression, hypothesis three was assessed using VI a simple linear model, and hypotheses four and five were evaluated using path analysis. Four gender-stratified follow-up focus groups were also conducted, to confirm quantitative findings and gain insight into the support needs and preferences of urban African American adults living with type-2 diabetes. Study Variables: Independent study variables included social support, autonomy support, basic psychological needs (autonomy, competence, relatedness), and autonomous regulation. Primary dependent variables were diabetes self-management and diabetes-related emotional distress. Results: 155 eligible individuals (67 male, 88 female) were included in the quantitative analytic sample. 23 individuals (10 male, 13 female) participated in follow-up focus groups, yielding a total of 6 hours of audio-recorded focus group data. Autonomy and competence significantly predicted self-management and emotional distress in hypotheses one and two, respectively. Competence was the strongest predictor of selfmanagement and emotional distress among sample participants, followed by autonomy, but relatedness was not observed to be a significant variable in the prediction of study outcomes. Social support was observed to significantly predict relatedness need fulfillment in a simple linear model for hypothesis three. Data did not support hypothesis four and five. Qualitative findings contextualized the quantitative study findings. Conclusions: Future support programs should facilitate basic need fulfillment in urban African American adults living with type-2 diabetes. Relatedness is especially important in this collectivistic cultural group.

Few studies have focused solely on dietary changes (one of the foundations for diabetes self-care), particularly in the context of family and the role of AAW. The purpose of this descriptive ethnographic study was to explicate cultural influences on food practices of AAW with type 2 diabetes (T2DM) in order to inform the health care community as well as future development of culturally-tailored interventions. Specific aims were to describe typical daily food practices and identify cultural influences on food practices of AAW with T2DM. Symbolic Interactionism, a sensitizing framework for viewing AAW with T2DM as a subculture, guided this study. Purposeful sampling was used to recruit 20 AAW who: were between 35 and 70 years of age, had been diagnosed with T2DM, shopped and prepared meals for their families, and attended church functions where food was served . Data collection consisted of one-one-one interviews and participant observation of church fellowship dinners, grocery shopping, and food preparation. A social anthropological approach to content analysis was used to describe behavioral regularities in food practices. Trustworthiness was maintained by an audit trail. Findings indicate that for informants in this study, who had diabetes ranging from 2 to 30+ years, there is a constant struggle between cultural food practices and eating healthier because of diabetes, particularly within the home setting where a majority of daily food practices take place. Difficulties in making dietary modifications result from conflicts between the need to change dietary practices to control diabetes and personal food preferences, as well as the preferences of people within the participants' social network. In addition, difficulties derive from AAW's emotional dedication to the symbolism of food and traditional cultural food practices. AAW are the gatekeepers for family food practices and are the keys to healthy dietary practices. This study begins to fill the research gap regarding cultural dietary food practices of this population. With increased knowledge, researchers and health care providers will be better able to improve AAW food practices, and ultimately improve diabetes control in this high-risk population.
