Book excerpt: Excerpt from Antiseptic Surgery: Its Principles, Practice, History, and Results The interest which Antiseptic Surgery has awakened throughout the profession has led to many public debates and to the accumulation of a great mass of literature on the subject. The time seems now to have come for a detailed account and discussion of the whole matter, and it is with the view of furnishing such an account that the present volume has been published. The question might have been dealt with in two ways; either by the discussion of the present standpoint of the principles and practice of Antiseptic Surgery, with but little reference to former literature; or, as I have attempted to do, by tracing in addition the development of its theory and practice, and following out and criticising the various steps by which the present state of knowledge on the has been reached. The former mode might no doubt have produced a shorter work, but it could not have been made complete in the present transitory state of our knowledge, and, taken alone, it would have necessarily raised side issues confusing to readers unacquainted with the earlier literature. Book excerpt: Excerpt from The Evolution of Antiseptic Surgery: An Historical Sketch of the Use of Antiseptics From the Earliest Times For many years I have been engaged in researches respecting the early methods employed in the healing arts, amongst both civilised and uncivilised peoples. It has been my object, in particular, to trace the origin of the use of remedial agents. Why were certain sub stances used in the treatment of various diseases? Was their adoption the result of study and practical observa tion, or was it more usually the result of accident? Were the alleged virtues purely imaginary and due to some superstitious suggestion? A consideration of such ques tions is always of interest, and sometimes adds to our knowledge. There is a considerable amount of information scattered throughout the world in folk-lore, early manuscripts and printed books, but the difficulties of tracing out and sifting the evidence are considerable. I anticipate that the Historical Exhibition of medical, chemical and pharmaceutical objects which I am organising, to be held in London (eng), will lead to the revealing of many facts, and the elucidation of many obscure points, in connection with the origins of various medicines. Book excerpt: Excerpt from Antiseptic Surgery: The Principles, Modes of Application, and Results of the Lister Dressing Although for more than a decade it has been hardly possible to glance through a medical journal without seeing something concerning the Lister method, there are comparatively few medical men in this country who have a sufficiently good knowledge of this modern system of treating wounds to enable them to apply it with essential accuracy. The result is that it is either not used at all by most surgeons, or else is incorrectly employed with disappointing results, which are not fairly attributable to the method. This condition of affairs is probably largely due to the fact that there has been no low-priced treatise on the subject in the English language, from which one could acquire the necessary information with regard to the principles, practice, and results of Antiseptic Surgery. With the desire of supplying my fellow practitioners in America with such a work, and thus enabling them to experience the benefits of a method which will do more than any other to lead their surgical patients to recovery without delay, danger, or discomfort, I have translated this work of an eminent French surgeon. Its careful perusal will, I believe, make it plain, not only that the Listerian theory is rational and its practice wonderfully satisfactory, but also that its application is neither difficult, nor seriously expensive. Wilson Prize for Literary Science Writing Short-listed for the 2018 Wellcome Book Prize A Top 10 Science Book of Fall 2017, Publishers Weekly A Best History Book of 2017, The Guardian "Warning: She spares no detail!" —Erik Larson, bestselling author of Dead Wake In The Butchering Art, the historian Lindsey Fitzharris reveals the shocking world of nineteenth-century surgery and shows how it was transformed by advances made in germ theory and antiseptics between 1860 and 1875. She conjures up early operating theaters—no place for the squeamish—and surgeons, who, working before anesthesia, were lauded for their speed and brute strength. These pioneers knew that the aftermath of surgery was often more dangerous than patients’ afflictions, and they were baffled by the persistent infections that kept mortality rates stubbornly high. At a time when surgery couldn’t have been more hazardous, an unlikely figure stepped forward: a young, melancholy Quaker surgeon named Joseph Lister, who would solve the riddle and change the course of history. Fitzharris dramatically reconstructs Lister’s career path to his audacious claim that germs were the source of all infection and could be countered by a sterilizing agent applied to wounds. She introduces us to Lister’s contemporaries—some of them brilliant, some outright criminal—and leads us through the grimy schools and squalid hospitals where they learned their art, the dead houses where they studied, and the cemeteries they ransacked for cadavers. Eerie and illuminating, The Butchering Art celebrates the triumph of a visionary surgeon whose quest to unite science and medicine delivered us into the modern world.

Book excerpt: Excerpt from Antiseptic Surgery: The Principles, Modes of Application, and Results of the Lister Dressing The first edition of this book was published not long ago, and now the progress of the antiseptic method is so well known that there is no need of long preliminaries to tell how profoundly it has modified, even transformed, the surgical practice of our time. The reception with which it has met has undoubtedly varied: even in England it has difficulty in acquiring the position which is its due; in Denmark it reigns supreme; in Germany it has its enthusiasts, since it has undergone changes; in Austria, Switzerland, Holland, Russia, Italy, and America, we meet eminent representatives of Listerian sur gery and, indeed, we believe there is hardly a country where there are not some ardent disciples who proclaim its merits. Book excerpt: Infections that occur in the wound created by an invasive surgical procedure are generally referred to as surgical site infections (SSIs). SSIs are one of the most important causes of healthcare-associated infections (HCAIs). A prevalence survey undertaken in 2006 suggested that approximately 8% of patients in hospital in the UK have an HCAI. SSIs accounted for 14% of these infections and nearly 5% of patients who had undergone a surgical procedure were found to have developed an SSI. However, prevalence studies tend to underestimate SSI because many of these infections occur after the patient has been discharged from hospital. SSIs are associated with considerable morbidity and it has been reported that over one-third of postoperative deaths are related, at least in part, to SSI. However, it is important to recognise that SSIs can range from a relatively trivial wound discharge with no other complications to a life-threatening condition. Other clinical outcomes of SSIs include poor scars that are cosmetically unacceptable, such as those that are spreading, hypertrophic or keloid, persistent pain and itching, restriction of movement, particularly when over joints, and a significant impact on emotional wellbeing. SSI can double the length of time a patient stays in hospital and thereby increase the costs of health care. Additional costs attributable to SSI of between £814 and £6626 have been reported depending on the type of surgery and the severity of the infection. The main additional costs are related to re-operation, extra nursing care and interventions, and drug treatment costs. The indirect costs, due to loss of productivity, patient dissatisfaction and litigation, and reduced quality of life, have been studied less extensively.

This book was released on 2017-01-27 with total page 184 pages. Available in PDF, EPUB and Kindle. Book excerpt: Surgical site infections are caused by bacteria that get in through incisions made during surgery. They threaten the lives of millions of patients each year and contribute to the spread of antibiotic resistance. In low- and middle-income countries, 11% of patients who undergo surgery are infected in the process. In Africa, up to 20% of women who have a caesarean section contract a wound infection, compromising their own health and their ability to care for their babies. But surgical site infections are not just a problem for poor countries. In the United States, they contribute to patients spending more than 400 000 extra days in hospital at a cost of an additional US $10 billion per year. No international evidence-based guidelines had previously been available before WHO launched its global guidelines on the prevention of surgical site infection on 3 November 2016, and there are inconsistencies in the interpretation of evidence and recommendations in existing national guidelines. These new WHO guidelines are valid for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.

Available in PDF, EPUB and Kindle. Book excerpt: The book covers the basic concept of surgical and oncosurgical disciplines as a whole, as well as the management of surgical patients from pre-op preparation to discharge, i.e., all the basics needed for a successful outcome for oncosurgical patients. It covers surgical safety, the consumer protection act, medico-legal aspects, the importance of documentation, research and publications, and managing complications. The respective chapters cover pre-operative, intra-operative, and ICU management of cancer patients, based on a multi-disciplinary approach. Additionally, they highlight recent advances in surgical oncology and so-called incurable cancers. Edited and written by an interdisciplinary team of experts in oncology and palliative care, the book is intended as a clinically useful guide to the overlapping topics of pain management in cancer patients and the treatment of cancer in patients with multiple co-morbidities like cardiovascular, respiratory disease, hypertension and diabetes mellitus. Given its scope, it will benefit multi-disciplinary oncologists, pain, palliative and intensive care experts, as well as students of surgical disciplines, from MBBS, MS and DNB, to MRCS, MCh and FRCS.
