Available in PDF, EPUB and Kindle. Book excerpt: Dementia in the adult U.S. population is a devastating disorder that is often unrecognized or misdiagnosed in its early stages. Despite the current lack of unequivocally effective treatment, recognition of early-stage dementia may offer substantial benefits. These include avoidance of inappropriate treatment related to misdiagnosis and time for the patient and family to address issues of financial, legal, and medical care planning. This Clinical Practice Guideline is intended to help primary care providers recognize and assess Alzheimer's disease and related dementias in the early stages. Differential diagnosis is beyond the scope of the guideline; however, the guidline contains a list of resources for further clinical evaluation once probable dementia has been identified.

Book excerpt: The management of Alzheimer's Disease and the related dementias is one of the major challenges to health care professionals and American society-at-large for the coming decade and the coming millennium. The rapid growth of the over-eighty-five population, the group which, as recent studies have confirmed and as many of us clinicians have long suspected, has an even higher prevalence than previously quoted of dementing disorders, is the major cause of this. We are thus challenged by, as Bernard Issacs used to call it, "the survival of the unfittest," as well as the oPtimistic approach of "bringing life to years," as John F. Kennedy said. The fact is that we, as a society, tend to confuse "treatment" and "cure" (and "prevention"). As the proceedings of the conference which this book represents emphasize, there is considerable work going on about the potential prevention of, or at least the reduction of, symptomatology in these illnesses by interventions genetically, chemIcally, and so forth. However, the more we find out, the more complicated it becomes, and the more heterogeneous Alzheimer's and the related disorders appear to be, not only in their manifestations (as clinicians have long recognized) but also in the individual initiating and underlying processes. For these reasons, absolute preventive techniques or the likelihood of an intervention which will reverse the process in a high proportion of patients, do not appear to be just around the corner.

Available in PDF, EPUB and Kindle. Book excerpt: The dementia challenge is the largest health effort of the times we live in. The whole society has to move to a realization of the significance of prioritization to make an attempt in the direction of mental health promotion and dementia risk reduction. New priorities for research are needed to go far beyond the usual goal of constructing a disease course-modifying medication. Moreover, a full empowerment and engagement of men and women living with dementia and their caregivers, overcoming stigma and discrimination should be promoted. The common efforts and the final aim will have to be the progress of a ''dementia-constructive'' world, where people with dementia can take advantage of equal opportunities.

Book excerpt: Alzheimer disease (AD) is a neurodegenerative disorder characterized by significant cognitive deficits, behavioral changes, sleep disorders and loss of functional autonomy. AD represents the main cause of dementia and has become a major public health issue. In addition, the number of patients suffering from AD is growing rapidly as the population ages worldwide. Memory impairment is usually the earliest clinical and core symptom of this disease. The diagnosis at a late clinical stage is relatively easy. However, a delay in the diagnosis is damageable for the handling of patients in terms of optimal medical and social care. The actual interest of the scientific head-ways is to optimize the diagnosis in prodromal stage of the disease and to propose personalized therapeutic solutions to individual patients. New revised AD diagnostic criteria include early alteration of cerebrospinal fluid (CSF) biomarkers: decrease of amyloïd peptides (Aβ42), and increase in tau and phosphorylated-tau (p-tau) protein concentration. This recognition of CSF biological biomarkers for the diagnosis of AD is a major step towards the “molecular” diagnosis and follow-up of the disease. Many issues are however still subject of debate. This e-book provides a comprehensive overview of the state of the art of fluid biomarkers for AD, e.g. which novel biomarkers should be implemented in clinical practice for diagnosis or for monitoring treatment or side effects, which ones are new for AD or related dementias or what is the potential of peripheral blood markers. Moreover, the e-Book provides practical guidelines how to optimally and efficiently develop and validate novel biomarker assays, and to document and control pre-analytical variation.

This book was released on 2009-03-02 with total page 590 pages. Available in PDF, EPUB and Kindle. Book excerpt: The American Psychiatric Publishing Textbook of Alzheimer Disease and Other Dementias is an up-to-date and comprehensive overview of dementia for psychiatrists and other health care practitioners who deal with cognitively impaired adults in outpatient, inpatient, and long-term care settings. With content ranging from clinical guidance to basic research, it contains information on nearly every subject related to dementing conditions or illnesses -- not only providing extensive coverage of clinical management issues but also enabling a deeper understanding of the causes of dementia. Designed to assist the practitioner faced with everyday dilemmas, from dosages of antipsychotic drugs to legal and ethical issues, this textbook describes in detail the most common conditions and diseases leading to dementia and covers pharmacologic, behavioral, and environmental treatments. It also considers a broader range of cognitive disorders and impairment in order to help practitioners recognize and treat primary brain diseases and systemic disorders affecting the brain before they reach the stage of dementia. Building on the editors' earlier work The Dementias: Diagnosis, Treatment, and Research, this new book expands on its scope, with nearly twice the number of contributors -- all clinicians or researchers at the vanguard of the field. New to this edition are chapters on epidemiology, history of dementia, biomarkers for Alzheimer disease, care of the late-stage dementia patient, prevention of dementia, and chapters devoted to: Vascular cognitive impairment, emphasizing the importance of early detection with development of appropriate treatments and risk factor control Dementia with Lewy bodies and other synucleinopathies, describing differences in cognitive profile between synucleinopathies and Alzheimer disease Frontotemporal dementias, including behavioral and language variants Traumatic brain injury, distinguishing between proximal and distal effects and risk factors for dementia later in life An abundance of charts and illustrations, extensive references and additional readings, and chapter-end key points make this a practical volume for learning, while appendixes include easily administered instruments useful in daily practice for grading cognition, day-to-day function, neuropsychiatric symptoms, and quality of life. Whether used as a clinical guide or as a sourcebook on technical and scientific developments, The American Psychiatric Publishing Textbook of Alzheimer Disease and Other Dementias is an important reference for psychiatrists, neurologists, geriatricians, primary care physicians, and other health professionals who deal with cognitively impaired adults.
