Book excerpt: Children living in poverty are more likely to have mental health problems, and their conditions are more likely to be severe. Of the approximately 1.3 million children who were recipients of Supplemental Security Income (SSI) disability benefits in 2013, about 50% were disabled primarily due to a mental disorder. An increase in the number of children who are recipients of SSI benefits due to mental disorders has been observed through several decades of the program beginning in 1985 and continuing through 2010. Nevertheless, less than 1% of children in the United States are recipients of SSI disability benefits for a mental disorder. At the request of the Social Security Administration, Mental Disorders and Disability Among Low-Income Children compares national trends in the number of children with mental disorders with the trends in the number of children receiving benefits from the SSI program, and describes the possible factors that may contribute to any differences between the two groups. This report provides an overview of the current status of the diagnosis and treatment of mental disorders, and the levels of impairment in the U.S. population under age 18. The report focuses on 6 mental disorders, chosen due to their prevalence and the severity of disability attributed to those disorders within the SSI disability program: attention-deficit/hyperactivity disorder, oppositional defiant disorder/conduct disorder, autism spectrum disorder, intellectual disability, learning disabilities, and mood disorders. While this report is not a comprehensive discussion of these disorders, Mental Disorders and Disability Among Low-Income Children provides the best currently available information regarding demographics, diagnosis, treatment, and expectations for the disorder time course - both the natural course and under treatment.

The principal object of this book was to provide an accurate factual basis for the planning of future services. Originally published in 1972, this title is based on three years’ extensive research and consists primarily of statistical studies rather than case studies. There is a survey of facilities provided by hospitals for the mentally handicapped and a sample survey of three Hospital Regions indicating the characteristics and disabilities of the mentally handicapped inpatients. Also included are two cohort studies, one of the subsequent experience of patients after their first admittance to a hospital, and one of the subsequent experience of all first referrals to a Local Authority service. Both studies were carried out to determine trends in the patterns of care. This was the first time that cohort techniques had been applied in this field in this country. Since effective long-term planning depends upon the coordination of the work of Local Authorities and hospitals, it is unrealistic to divorce the need for hospital beds from community provision. This book, therefore, examined the present services for the mentally handicapped in the round. It is supplemented by an extensive bibliography and numerous tables and diagrams. As a result, it should be of particular interest to psychiatrists, clinical psychologists, hospital administrators, senior nursing staff and Local Authority social service administrators. This book is a re-issue originally published in 1972. The language used is reflective of its era and no offence is meant by the Publishers to any reader by this re-publication.

There is clear, systemic bias at all stages of the prosecution and the sentencing process – in determining who is competent to be executed, in the assessment of mitigation evidence, in the ways that counsel is assigned, in the ways that jury determinations are often contaminated by stereotyped preconceptions of persons with mental disabilities, in the ways that cynical expert testimony reflects a propensity on the part of some experts to purposely distort their testimony in order to achieve desired ends. These questions are shockingly ignored at all levels of the criminal justice system, and by society in general. Here, Michael Perlin explores the relationship between mental disabilities and the death penalty and explains why and how this state of affairs has come to be, to explore why it is necessary to identify the factors that have contributed to this scandalous and shameful policy morass, to highlight the series of policy choices that need immediate remediation, and to offer some suggestions that might meaningfully ameliorate the situation. Using real cases to illustrate the ways in which the persons with mental disabilities are unable to receive fair treatment during death penalty trials, he demonstrates the depth of the problem and the way it’s been institutionalized so as to be an accepted part of our system. He calls for a new approach, and greater attention to the issues that have gone overlooked for so long.

The U.S. Social Security Administration (SSA) provides income support and medical benefits to individuals with cognitive limitations who experience significant problems in their ability to perform work and may therefore be in need of governmental support. Addressing the concern that SSA's current procedures are consistent with current scientific and professional practices, this book evaluates the process used by SSA to determine eligibility for these benefits. It examines the adequacy of the SSA definition of mental retardation and its current procedures for assessing intellectual capabilities, discusses adaptive behavior and its assessment, advises on ways to combine intellectual and adaptive assessment to provide a complete profile of an individual's capabilities, and clarifies ways to differentiate mental retardation from other conditions.

These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. He examines the influences of gender, race, and class in the institutionalization process and relates policies in the South to those in the North and Midwest, regions that had established similar institutions much earlier. In addition, Noll creates a vivid portrait of life and work within institutions and the impact of institutionalization on patients and their families. At the center of the story is the debate between the humanitarians, who advocated institutionalization as a way of protecting and ministering to the mentally deficient, and public policy adherents, who were primarily interested in controlling and isolating perceived deviants. According to Noll, these conflicting ideologies meant that most southern institutions were founded without a clear mission or an understanding of their relationship to southern society at large.

Book excerpt: Examining representations of mental difference, this collection focuses on the ways that adaptations (including remakes, reboots, and other examples of remixed narratives) can shape and shift the social contexts and narratives we use to define mental disability. The movement of narratives across media via adaptation, or within media but across time and space in the case of remakes and reboots, is a common tactic for revitalization, allowing storytellers to breathe new life into tired narratives, remedying past inaccuracies and making them accessible and relevant for contemporary audiences. Thus, this collection argues that adaptation provides a useful tool for examining the constraints or opportunities different media impose on or afford narratives, or for measuring shifts in ideology as narratives move across cultures or through time. Further, narrative functions within this collection as a framework for examining the ways that popular media exerts rhetorical power, allowing for deeper understandings of the ways that mental disability is experienced by differently situated individuals, and revealing relationships with broader social narratives that attempt to push definitions of disability onto them.
