Book excerpt: Abstract: The Mental Health Services Act (MHSA) of 2004 was analysed using David Gil’s (1992) analytic framework. The strengths and weaknesses of the Act were assessed and special attention was placed on transitional age youth (TAY) who are consumers of Full Service Parternship (FSP) services in Los Angeles County. This analysis found that there have been some improvements in the provision of services to TAY as a result of the MHSA (2004). However, TAY continue to be a group that remains underserved despite the availability of MHSA (2004) funds. The lack of infrastructure of Department of Mental Health to oversee MHSA (2004) funded projects is likely one of the main reasons why new programs for TAY have not been developed. This analysis did find that TAY, who have been served under MHSA (2004) funded programs, have lower rates of incarceration and hospitalization. The recommendations for social work practice, policy and research are discussed.

The proposed program was written to operate under the Orange County branch of Aspiranet (an agency tasked with assisting foster care and former foster care TAY with housing). The California Wellness Foundation was selected as an appropriate funding source as it supported the development of mental health services for foster care TAY. Book excerpt: This dissertation examines the children's public mental health Full Service Partnership (FSP) program established by the California's Mental Health Services Act of 2004 in order to provide mental health and social services and supports not otherwise available to the underserved. Utilizing a California statewide dataset of all public county mental health system provisions linked from two sources of data, including FSP program exposure data (Data Collection and Report dataset) and mental health service data (Client Services Information dataset) from the California Department of Health Care Services (CDHCS), analyses compare the FSP program to usual care within county mental health systems. Synthesizing records summarizing each month a child was served, the final dataset encompasses 36 counties (61%) inclusive of 623,031 (70%) children (ages 618) served within county mental health systems over 102 months between July 1, 2004 and December 31, 2012, reflecting 15,723 children served by the FSP program. To evaluate whether the FSP program was meeting its goals as established in statute and guidelines, three research questions were addressed: 1) Do FSP programs reach underserved children, as intended? 2) Do FSP programs provide a different array of services as compared to usual care, as intended? 3) Does participation in the FSP program result in positive outcomes as intended, measured by a decrease in the use of mental health emergency service? Adjusting for demographics, indicators of clinical severity, and county of residence, three respective analytical models were developed to compare FSP served children to those in usual care including: 1) hierarchical logistic regression modeling for the odds of a child enrolling into an FSP versus remaining in the pool of those served by usual care in each county, 2) t-tests contrasting service provision arrays for FSP served children as compared to those in intensive (service on = four days per month) usual care, and 3) longitudinal hierarchical Poisson regression modeling the pre-post matched change in incidence rate of mental health emergency services use before, after and without FSP enrollment. As intended by policy, results indicate that FSPs reach the underserved, provide a different array of service intensity and result in reductions of crisis care for the children and families it served. The grant was developed to identify funding sources to implement the evidence-based practice of family integrated transitions for youth with co-occurring substance abuse and mental health disorders transitioning from secure juvenile facilities back to their homes. The analysis intended to explore the benefits and consequences of the tax increase, and how mental health services are impacted under the MHSA. This study scrutinized a historical review of mental health services in the United States, information of political trends on national and state level, mental health reform, and enactment of policies related to mental health. The analysis concluded that the MHSA legislation has potentially increased mental health service programs for people with mental illness and their families. However, the study also found that the state has failed to provide proper guidance to counties for effective program evaluation.

Book excerpt: Despite two decades of significant advancements in the development of psychotherapeutic and pharmacological approaches for children and adolescents with mental health problems, the most difficult problem continues to be engagement. However, there is a dearth of research examining factors beyond individual and family level factors. This study addresses this gap by honing in on the organizational level factors to further understand engagement of transition age youth. Engagement in services among transition age youth is particularly precarious, as they are less influenced by their guardians. Ecological perspective posits that there are multiple barriers to service engagement that exist on varying levels of the ecosystem. Building on the Gateway Provider Model, socio-technical theory, and organizational culture theory, the specific aims of this study are 1) to explore how practitioner-level characteristics (a. socio-demographic characteristics, born professional characteristics) are related to youth service engagement; 2) to explore how organization-level characteristics (a. organization characteristics, born organizational culture) are related to youth service engagement; and 3) to explore the moderating role of organizational culture on practitioner-level characteristics that affect youth service engagement. A cross-sectional survey research design was used to address the research questions. The data was collected from 279 practitioners from 27 mental health service organizations which represent three major metropolitan areas (Cleveland, Columbus, Cincinnati) in Ohio. Hierarchical linear modeling (HLM) was used to address a nested (clustered) structure. Findings from the study revealed that a practitioner who had lower case loads and lower concerns of service barriers, and provided more service provision perceived higher level of service engagement. None of the practitioner demographic predictors had a significant effect on perceived service engagement. The practitioners who provided service to the urban population and worked at a mental health clinic setting (vs. outreach setting) perceived a higher level of service engagement. In an organization where the level of professional support and trust was higher and concerns in hierarchy problems were fewer, practitioners perceived that their transition age youth (TAY) clients were more engaged in mental health services. In addition, there were cross-level interaction effects between the practitioners' professional characteristics and organizational culture. That is, in an organization which has a higher level of professional values, the relationship between more frequent coordination with other organizations and service engagement became stronger. Also, the fact that practitioners who perceive more controls of hierarchy in the organization and have a stronger relationship between resource knowledge and service engagement supports the conclusion that resource knowledge is a better predictor of service engagement in the organization with more hierarchical control. These findings are critical for policy makers, practitioners, administrators, and other allied human service professionals who are responsible for working across systems with youth in transition

Book excerpt: In 2004, California voters passed Proposition 63, a ballot initiative to increase funding for mental health care by taxing 1 percent of personal income over $1 million. Now called the Mental Health Services Act (MHSA), the goal was to reform California's mental health system to provide community based services and encourage an innovative, recoveryfocused, and culturally competent approach to care. This study attempts to examine if MHSA and its components have the desired impact on the target population. To investigate the question, two programs are analyzed that served similar clients using the same service model with data collected April 2007 through June 2008. Both programs used Assertive Community Treatment (ACT) as their recovery model in serving clients with mental health issues; one of the programs was funded under MHSA and the other was not. The hypothesis of this study is that the program that received MHSA funding and therefore required to follow guidelines set under MHSA will have higher fidelity to the ACT model and also have better client outcomes than the program that is funded through the larger traditional mental health department. Outcomes were measured by comparing client scores in a variety of outcome measures collected monthly by program staff, and by analyzing fidelity assessments conducted on each program. A Student's t-test was used to compare percentages of clients with improved or stable scores between the programs in several outcome areas (including mental health recovery stage, substance abuse, residential, employment, and educational status). Only one of the five outcome measures, the mental health recovery stage, showed a significant difference in the percentage of clients with "stable or improved" scores (p=.011) with a larger percent of clients in the MHSA-funded program improving or remaining stable. However, the traditionally funded ACT program scored significantly higher on the Dartmouth Assertive Community Treatment Scale, which measures program fidelity to the evidence-based model. These are mixed results and do not strongly prove or disprove the hypothesis. These mixed results raise more questions and reflect the need for further study and improved data measures. Analyzing outcomes of this unique legislation is essential to understanding the impact it has on individuals and families, their communities and the service delivery system.
