Book excerpt: The development of specialty skills in medical and surgical practice in the late 19th and in the 20th century transformed medical practice. For the first time, a patient could visit a doctor with the expectation of an accurate diagnosis and effective treatment. Disease prevention became a realistic proposition. Traditional practice methods became obsolescent, but a yearning for generalist medical wisdom and respect continued until the 2nd World War disrupted progress. In the 19th century, the London Hospital was remarkably open to new ideas, and the Chairman of the Board, Viscount Knutsford, was a master fundraiser. Investment in novel facilities and staff, including the establishment of special departments, and consequent changes in clinical practice led to a growing national and international reputation in clinical practice and education. Specialty skills defined innovations, both in hospital and family practice. More recently, merging St Bartholomew’s, the Royal London Hospital and other hospitals has reactivated the advance of specialism.

Book excerpt: In the sixth edition of this work John Fry has reverted to the format of the earlier editions published in 1965, 1970 and 1973 and has presented statistical data drawn widely from many sources. General practice in the United Kingdom and elsewhere throughout the world has undergone many changes in the past decade. This new edition brings the available statistical informa tion up to date and indicates the way ahead. General practice, the College and the National Health Service have been closely linked in effecting change. The membership of the College con tinues to grow and its influence in changing the face of general practice in the United Kingdom is significant. John Fry's new book presents that characteristic blend of fact and personal opinion which was so successful in earlier editions and so valuable to general practice and others in primary health care. Once again he has shown what can be achieved by one man based on his own work and those of others in general practice. John Lawson RCGP, 1983 President of the Royal College of General Practitioners vi Preface The Present State and Future Needs series had its roots in the dark days of the early 1960s when morale in general practice was low and when numbers of new entrants were actually going down. They were the days of mass emigration of doctors from Britain and the National Health Service. The fourth and fifth editions were entitled Trends in General Practice (1977 and 1979).

Available in PDF, EPUB and Kindle. Book excerpt: Change is frequent in healthcare, yet change management is often far from perfect. This book considers the complexity of change within large organisations, explores existing models of change and emphasises the vital role of emotional and cognitive readiness in successful change management. Despite the plethora of organisational change management approaches used in healthcare, the success rate of change in organisations can be as low as 30 percent. New thinking about change management is required to improve success in service development, improvement and innovation. Arguing that emotional and cognitive readiness for change requires engagement with the people involved, and a thorough understanding of areas of friction and potential challenge, this book also delves into the neglected issue of emotion, examining emotional labour and emotion and change. It investigates how human emotion can be incorporated into Change Management Models, alongside and intertwined with cognitive approaches, to support effective change. Using the NHS as a central case study, this book incorporates examples of actual change from a range of healthcare settings from acute to primary care, enabling readers to see how Change Management Models can be adapted and utilised in practice. This is an essential read for students, as future change leaders, and practitioners and managers leading and managing change in healthcare.

This book was released on 2004 with total page 392 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book explores how London society responded to the dilemma of the rampant spread of the pox among the poor. Some have asserted that public authorities turned their backs on the "foul" and only began to offer care for venereal patients in the Enlightenment. An exploration of hospitals and workhouses shows a much more impressive public health response. London hospitals established "foul wards" at least as early as the mid-sixteenth century. Reconstruction of these wards shows that, far from banning paupers with the pox, hospitals made treating them one of their primary services. Not merely present in hospitals, venereal patients were omnipresent. Yet the "foul" comprised a unique category of patient. The sexual nature of their ailment guaranteed that they would be treated quite differently than all other patients. Class and gender informed patients' experiences in crucial ways. The shameful nature of the disease, and the gendered notion of shame itself, meant that men and women faced quite different circumstances. There emerged a gendered geography of London hospitals as men predominated in fee-charging hospitals, while sick women crowded into workhouses. Patients frequently desired to conceal their infection. This generated innovative services for elite patients who could buy medical privacy by hiring their own doctor. However, the public scrutiny that hospitalization demanded forced poor patients to be creative as they sought access to medical care that they could not afford. Thus, Venereal Disease, Hospitals and the Urban Poor offers new insights on patients' experiences of illness and on London's health care system itself. Kevin Siena is Assistant Professor of History at Trent University.
