Book excerpt: Excerpt from The Journal of Cutaneous Diseases, Vol. 26: March, 1908 Owing to the striking and peculiar symptoms of the disease, all observers are of one mind as to its clinical characteristics, but there is considerable divergence of opinion as to its histopathology, and more especially as to its place in nosology. While most authors agree with the view of Kaposi that the affection is a form of sar coma, a not inconsiderable minority regard its sarcomatous nature as more or less doubtful, while a.few deny it absolutely. Most of those who have studied its histopathology found the lesions composed of spindle-celled elements, either entirely or in large part; a few found only round or oval cells, while Kaposi and Perrin6 found both types of cell. Book excerpt: Excerpt from The Journal of Cutaneous Diseases, Including Syphilis, Vol. 26: May, 1908 Elliot 11 believes his case to be the same as Tilbury Fox's hydroa simplex, in which there was little crusting and no scarring. A bi opsy of a freshly occurring lesion was made. In this section work the stratum corneum was broad and well defined, with loosened and separated layers, especially near the vesicles, but marked around that portion of the sweat ducts which passed through it, and over some of these latter it was raised, forming vesicles. The nuclei were retained almost to the surface. The stratum lucidum was scarcely demonstrable, and the stratum granulosum was seen limited to a single layer. The rete near the vesicle became acanthotic, more than doubling itself. The cells became long and narrow, slightly granular and somewhat loosened, nuclei occasionally absent, but as a whole they were well stained. He speaks of its being an inflammatory reaction, but does not speak of the marked stream of leucocytes into the vesicles and sur rounding areas. In his summary he concludes that the point of origin of these lesions is primarily in the epithelia of the sweat ducts just below the horny layers of the epidermis, extending from there to the rete; and that the secondary symptoms are those of inflammation seated especially in the papillary layer. Among those, we found 80 (10 boys and 20 girls) presenting all the features of heredo - syphilis. Ten were under treatment for Pott's disease, 15 for unilateral, either right or left, hip disease, 1 for double hip disease, 1 for white swelling of the elbow, 2 for white swelling of the wrist, and 2 for suppurative cervical adenitis.
