Book excerpt: Deep brain stimulation (DBS) is an established surgical therapy for movement disorders such as Parkinson’s disease (PD) and essential tremor (ET). A thin electrode is implanted in a predefined area of the brain with the use of stereotactic neurosurgery. In the last few years new DBS electrodes and systems have been developed with possibilities for using more parameters for control of the stimulation volume. In this thesis, simulations using the finite element method (FEM) have been developed and used for investigation of the electric field (EF) extension around different types of DBS lead designs (symmetric, steering) and stimulation modes (voltage, current). The electrode surrounding was represented either with a homogeneous model or a patient-specific model based on individual preoperative magnetic resonance imaging (MRI). The EF was visualized and compared for different lead designs and operating modes. In Paper I, the EF was quantitatively investigated around two lead designs (3389 and 6148) simulated to operate in voltage and current mode under acute and chronic time points following implantation.Simulations showed a major impact on the EF extension between postoperative time points which may explain the clinical decisions to change the stimulation amplitude weeks after implantation. In Paper II, the simulations were expanded to include two leads having steering function (6180, Surestim1) and patient-specific FEM simulations in the zona incerta. It was found that both the heterogeneity of the tissue and the operating mode, influence the EF distribution and that equivalent contact configurations of the leads result in similar EF. The steering mode presented larger volumes in current mode when using equivalent amplitudes. Simulations comparing DBS and intraoperative stimulation test using a microelectrode recording (MER) system (Paper III), showed that several parallel MER leads and the presence of the non-active DBS contacts influence the EF distribution and that the DBS EF volume can cover, but also extend to, other anatomical areas. Paper IV introduces a method for an objective exploitation of intraoperative stimulation test data in order to identify the optimal implant position in the thalamus of the chronic DBS lead. Patient-specific EF simulations were related to the anatomy with the help of brain atlases and the clinical effects which were quantified by accelerometers. The first results indicate that the good clinical effect in ET is due to several structures around the ventral intermediate nucleus of the thalamus.

Available in PDF, EPUB and Kindle. Book excerpt: The assassin's bullet misses, the Archduke's carriage moves forward, and a catastrophic war is avoided. So too with the history of life. Re-run the tape of life, as Stephen J. Gould claimed, and the outcome must be entirely different: an alien world, without humans and maybe not even intelligence. The history of life is littered with accidents: any twist or turn may lead to a completely different world. Now this view is being challenged. Simon Conway Morris explores the evidence demonstrating life's almost eerie ability to navigate to a single solution, repeatedly. Eyes, brains, tools, even culture: all are very much on the cards. So if these are all evolutionary inevitabilities, where are our counterparts across the galaxy? The tape of life can only run on a suitable planet, and it seems that such Earth-like planets may be much rarer than hoped. Inevitable humans, yes, but in a lonely Universe.

Therapists, coaches, healthcare professionals, and educators have known for decades that expressive writing is a powerful tool for better living, learning, and healing. But until now, few have had access to practical applications that have proven successful. In this groundbreaking collection, you’ll discover: how expressive writing can call us into healing community exciting new discoveries about how writing can support neuroplasticity and actually help change our brains—and thus our thinking and behavior new research on the role of expressive writing for prevention of compassion fatigue in RNs how transformative writing can create art from the ashes of trauma the role of journal writing for emotional balance sensible ideas about the synergy of expressive writing and play therapy for children, teens, and adults interventions and strategies for the use of expressive writing in acute psychiatric care how interactive expressive writing helps deaf teens communicate inarticulate feelings and thoughts how cancer survivors can use expressive writing to reclaim identity and strength post-treatment the role of expressive writing in developing the roots of resilience for practitioners

Book excerpt: The electric utility industry and its stakeholders in the.United States appear to be at a critical juncture in time. Powerful forces of global proportions are propelling the industry instinctively and in a secular fashion towards restructuring. That the industry will change is a fait accomplii. The nature and timing of the change is still a matter of intense debate, however. Because of the evolution of the industry into its present-day form, i.e. regulated local monopolies in their designated franchise service territories, the relative roles and expectations of various institutions would have to change to conform to the new state in the future. In either encouraging, or allowing this change to happen, society is essentially saying that future societal welfare would be better served by the changed structure contemplated. What that assumption translates into in more direct terms is that creation of future wealth would be better accomplished through redistribution of wealth today. Thoughtful individuals recognize the enormous responsibility placed upon the various entities empowered with jurisdiction over the timing and nature of the structural change. They are trying hard to bring analytical rigor to bear on the debate. One very critical element of this debate on restructuring is the issue of the treatment of transmission. The issue has been variously labeled transmission access, or pricing. Volumes have been written and spoken on this topic.

Beginning with background material on the neuron, the book then examines specific portions of the nerve cell, and of the various supporting cells. Structure is first described in a general fashion, followed by detailed coverage of the fine structure of each component, with full discussion of how the structural features relate to their functions. Extensively revised and rewritten, this book will bring readers up to date with the many important developments that have taken place since publication of the previous edition. It includes over 130 electron micrographs and line drawings, many of which are new to this edition.
