It is not clear what impact these changes will have on medical and public health preparedness programs around the country. Although there has been tremendous progress since 2005 and Hurricane Katrina, there is still a long way to go to ensure the health security of the Country. There is a commonly held notion that preparedness is separate and distinct from everyday operations, and that it only affects emergency departments. But time and time again, catastrophic events challenge the entire health care system, from acute care and emergency medical services down to the public health and community clinic level, and the lack of preparedness of one part of the system places preventable stress on other components. The implementation of the ACA provides the opportunity to consider how to incorporate preparedness into all aspects of the health care system. The Impacts of the Affordable Care Act on Preparedness Resources and Programs is the summary of a workshop convened by the Institute of Medicine's Forum on Medical and Public Health Preparedness for Catastrophic Events in November 2013 to discuss how changes to the health system as a result of the ACA might impact medical and public health preparedness programs across the nation. This report discusses challenges and benefits of the Affordable Care Act to disaster preparedness and response efforts around the country and considers how changes to payment and reimbursement models will present opportunities and challenges to strengthen disaster preparedness and response capacities.

Book excerpt: In this study, we find that Medicaid expansion under the Affordable Care Act (ACA) is generally associated in expansion states with a shift of program financial resources away from children toward nonaged nondisabled adults. We examine Medicaid expenditures per capita for beneficiary categories including children, aged adults, and the disabled, as well as for nonaged nondisabled adults, who were the focus of the ACA's Medicaid expansion. We compare the growth of per capita expenditures on these groups from fiscal year (FY) 2013 to FY 2019 between states that chose to expand Medicaid on January 1, 2014, and states that had not expanded at the time data were collected. Nonexpansion states exhibited a remarkable stability in the distribution of program resources between these categories, with each category receiving nearly the same percentage of Medicaid financial resources in FY 2019 as in FY 2013. After controlling for changes in the beneficiary population within expansion states, we find that per capita spending growth on Medicaid's previously eligible population was largely similar for expansion states and nonexpansion states but that this overarching similarity masked significant shifts of program financial resources between more specific beneficiary categories within expansion states. The most striking of these shifts was a dramatic shift of financial resources away from children in expansion states. States that expanded Medicaid per the terms of the ACA spent only 5.9% more per capita on children in FY 2019 than they did in FY 2013 compared with growth of 22.7% in per capita spending on children in nonexpansion states and of 27.0% in average healthcare spending per capita for the US population as a whole.

Book excerpt: Medicaid’s federal-state matching system of financing is the nation’s largest example of fiscal federalism. Using generous federal subsidies, the Affordable Care Act incentivized states to expand Medicaid, which became a state option in the aftermath of a 2012 Supreme Court ruling. As of early 2020, 14 states had not yet expanded, with concerns over state budgetary effects described as a key barrier. We use an event-study approach to analyze state budget data from 2010-2018 and assess the effects of state Medicaid expansion decisions. We find that Medicaid expansion increased total spending in expansion states by 6% to 9%, compared to non-expansion states. By source of funds, federal spending via the states increased by 10% in the first year of Medicaid expansion, rising to 27% in 2018. Changes in spending from state funding were modest and non-significant, with less than a 1% change from baseline annually in the most recent years, 2017 and 2018. Meanwhile, we find no evidence that increased Medicaid spending from expansion produced any reductions in spending on education, corrections, transportation, or public assistance. Changes in Medicaid spending tracked closely with the baseline pre-ACA (2013) uninsured rate in each states, with expansion leading to roughly $2680 in added annual spending per uninsured adult. As a result, we estimate states that didn’t expand Medicaid passed up $43 billion in federally-subsidized program funds in 2018. Finally, state projections in the aggregate were reasonably accurate, with expansion states projecting average Medicaid spending from 2014-2018 within 2 percent of the actual amounts, and in fact overestimating Medicaid spending in most years.

It is critical therefore that effective strategies be put into place if both current and future health and long-term care reform efforts are to have their greatest chances at success. Rhode Island is the first state to receive permission to operate its entire Medicaid program under a global cap. As a consequence, it has entered the national consciousness as a key data point potentially supporting the block grant approach to Medicaid reform. In this book, Edward Alan Miller identifies factors that either facilitated or impeded the design and implementation of Rhode Island’s Global Consumer Choice Compact Medicaid Waiver in order to draw broader lessons for the Medicaid block grant debate and health and long-term care reform more generally. Book excerpt: The Patient Protection and Affordable Care Act (PPACA), signed into law on March 23, 2010, made significant changes to the way eligibility for the Medicaid program will be determined and who the program will cover. Under PPACA, eligibility for Medicaid--a joint federal-state program that finances health care for certain categories of low-income individuals--must be expanded to non-elderly individuals with incomes at or below 133 percent of the federal poverty level (FPL) beginning on January 1, 2014. Through this expansion, states will provide Medicaid coverage to eligible low-income parents and childless adults. PPACA also requires the establishment of American Health Benefit Exchanges (referred to as exchanges)--marketplaces where eligible individuals can purchase private health insurance in each state. The Centers for Medicare & Medicaid Services' (CMS) Office of the Actuary has estimated that, as a result of the expansion, the number of Medicaid enrollees will increase by 14.9 million in 2014 and by 25.9 million in 2020. State governments will play a key role in implementing many aspects of this reform, which must be in place by the beginning of 2014. Specifically, states will need to make major changes to the way they conduct Medicaid eligibility determinations for individuals and families. States also will need to develop streamlined eligibility and enrollment systems that allow for the coordination of enrollment across Medicaid, the Children's Health Insurance Program (CHIP), and exchanges. At the same time, states will need to address the financial implications of implementing this Medicaid expansion and accompanying enrollment systems. The federal government will initially provide states with full funding to cover the cost of adults who are newly eligible for Medicaid due to the expansion. Congress asked us to report on the actions states are taking to implement the Medicaid expansion. This report addresses the following questions: 1. What are states' responsibilities for implementing the Medicaid expansion provisions under PPACA? 2. What actions have selected states taken to prepare for the Medicaid expansion provisions of PPACA and what challenges have they encountered? 3. What are states' views on the fiscal implications of the Medicaid expansion on state budget planning?

Book excerpt: It is well documented that lack of health insurance negatively affects access to care and health outcomes. Uninsured people are less likely, compared to those with health insurance, to have usual source of care and receive necessary care primarily due to the cost, leading to detrimental health consequences. The Medicaid expansions under the Patient Protection and Affordable Care Act (ACA) were intended to provide access to health insurance coverage for many of the more than 45 million uninsured Americans. This major policy change originally required all states to expand the eligibility of their Medicaid programs to those younger than 65 years with incomes up to 138% of the federal poverty level (FPL), based solely on income without regard to categorical eligibility status. To date, there is ample evidence that the percentage of the uninsured has been significantly reduced nationally despite the fact that 14 states have not adopted the ACA Medicaid expansions as of August 2019 due to the 2012 Supreme Court ruling making those expansions voluntary rather than mandatory. This dissertation assessed the further effects of the ACA Medicaid expansions on out-of-pocket spending, healthcare utilization, and health outcomes for chronic conditions using a nationally representative sample of the low-income non-elderly population from the 2010-2016 Medical Expenditure Panel Survey and 2005-2016 National Health and Nutrition Examination Survey. It took advantage of the natural experiment that allowed a comparison of changes in outcomes between the expansion and non-expansion states. The three studies found that the ACA Medicaid expansions were associated with reduced out-of-pocket spending and improved financial risk protection, a modest increase in primary care physician visits without any meaningful change in emergency department visits, and improved clinical measures for hypertension and diabetes (but no improvement in outcomes for hyperlipidemia and depression), during the three years after the policy implementation. The findings suggest that the ACA has been successful in achieving its goals of removing financial barriers, promoting access to primary care, and improving population health among low-income uninsured Americans. It has important implications for state decisions on adopting the ACA Medicaid expansions and for the ongoing national debate over the repeal of the ACA, including the Medicaid expansions.
