Its purpose is to unravel what has gone wrong in the past and provide guidance on preventing similar incidents in the future. Regrettably, history has shown that many incidents tend to recur after a few months, often due to people moving on and lessons being forgotten. Among the various teaching methods, experiential learning has proven the most effective, as it helps retain subject matter for an extended period.
