Book excerpt: The major legislation, the Patient Protection and Affordable Care Act, is attempting to overhaul the health care system in the United States. Health educators need to understand how this health care policy will impact the profession. Forecasted with change, this study's goal was to provide preliminary insights into the perceived impact of the Affordable Care Act and changes that could occur within the health education profession as a result of this major health care reform legislation. Seven knowledgeable, experienced, and well-respected leaders of the health education profession participated in this qualitative research study. Semi-structured, exploratory interviews were conducted with six participants and one participant provided written responses to the interview protocol questions. After each interview, a thematic analysis was conducted on the participants' responses. Five themes emerged from the interviews: (1) a fragmented sick-care system, (2) ACA becomes law: the participants' reactions, (3) ACA becomes law: the profession's reactions, (4) impact on the profession, and (5) health education in 2020. The changes the Affordable Care Act is attempting to make to the health care system are no secret. There is potential for health educators to do something they have never been able to do before because of the Affordable Care Act, but action must be taken by these professionals. The positive elements of this legislation need to be protected, strengthened and verified, and further action needs to be taken to assure all critical components for creating a truly reformed health care system are incorporated into future legislation. Future research focused on investigating the impact the Affordable Care Act has on the health education profession should be conducted on a regular basis. As more mandates within the law are enacted over time, the impact on the profession, more than likely, will shift. It is also recommended future research seek to quantify the impact the legislation has on the profession. The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care. These recommendations support the creation of a diverse workforce that is competent, compensated, and authorized to serve vulnerable and underserved populations across the life cycle. Specifically, it was aimed at reducing the exorbitant costs of medical care, which prohibit many Americans from obtaining routine care. This study systematically reviews 20 peer reviewed articles that were selected based on whether they compared access to care data before and after the ACAs implementation. A review of the literature reveals that the ACA's provisions which include Medicaid expansion and regulation that prohibits insurance carriers from denying care to subscribers with pre-existing conditions has indeed lowered the number of uninsured and increased access to care. From increasing access to primary care among the general population to ameliorating inequalities among socioeconomically disadvantage groups, the ACA has significantly increased American's access to healthcare. However, the study also finds that there are still gaps in coverage within subgroups of the Hispanic/Latino population, and this can be attributed to variances in citizenship status and comprehension of the English language. While access to coverage has increased, additional research is required on the number of providers who are accepting Medicaid or have terminated their contract with Medicaid due to its low reimbursement rates and the influx of Medicaid beneficiaries.

Book excerpt: Since its creation by the Institute of Medicine (IOM) in 2007, the Roundtable on the Promotion of Health Equity and the Elimination of Health Disparities has been fostering dialogue on racial and ethnic disparities in health and health care, examining the development of programs and strategies to reduce disparities, and encouraging the emergence of new leadership focused on health equity. For the past several years, a prominent topic of discussion within the roundtable has been the Patient Protection and Affordable Care Act (ACA). The ACA has multiple provisions specific to race, ethnicity, and language and other provisions with significant implications for racially and ethnically diverse populations. In April 2013, the roundtable held a workshop to address many issues surrounding the ACA, including expansion of coverage, delivery systems, and access points, service delivery and payment reform, public-private partnerships, and challenges to the safety net. This report summarizes the presentations and discussions from the workshop.
