Book excerpt: College counseling centers are searching for innovative and short-term mental health interventions that promote wellness, especially given the growing number of college students who report serious and chronic stress. Researchers and clinicians have tested two promising interventions, self-compassion meditation and biofeedback training, in their search to find effective tools to assist people in handling stress. Some evidence shows that these interventions can help individuals to increase mindfulness and self-compassion and to decrease self-criticism and anxiety. The purpose of the study was to investigate whether HRV biofeedback training using the emWave and Self Compassion Meditation would help participants to attain a more positive state as measured by the questionnaires. Forty participants were randomly assigned to one of three conditions: Biofeedback, Self-Compassion and Control. The dependent variables and the scales used to measure them included mindfulness (5-Facet Mindfulness Questionnaire; FFMQ), self-compassion (Self-Compassion Scale; SCS), self-criticism (Levels of Self-Criticism Scale; LOSC) and anxiety (State-Trait Anxiety Inventory, STAI-S and STAI-T). The procedures took place as follows: The Biofeedback Condition participated in Biofeedback training with the emWave, the Self-Compassion Condition listened to an adapted version of the Self-Compassion/ Loving Kindness meditation, created by Kristin Neff, and the Control Condition listened to a recording of an adapted version of the history of cookies by Linda Stradley (2004). It was hypothesized that participants in the Biofeedback Condition would report significantly greater outcome means on the FFMQ as well as significantly lower outcome means on the STAI-S and STAI-T when compared to the Control Condition and the Self-Compassion Condition. Participants in the Biofeedback Condition were also expected to report significantly higher mean exit scores on the SCS as well as significantly lower mean exit scores on the LOSC when compared to the Control Condition. The participants in the Self-Compassion Condition were expected to report significantly greater post-intervention means on the SCS as well as significantly lower mean exit scores on the LOSC when compared to the Biofeedback Condition and the Control Condition. The participants in the Self-Compassion Condition were also expected to report significantly higher mean exit scores on the FFMQ as well as significantly smaller mean exit scores on the STAI-S and STAI-T when compared to the Control Condition. A one-way between-groups ANOVA with post-hoc test revealed significantly lower STAI-S mean exit scores in the Biofeedback Condition and the Self-Compassion Condition when compared to the Control Condition. However, conducting a set of ANCOVAs did not reveal any significant differences between the exit scores of the three interventions (Biofeedback training, self-compassion meditation and control) on the FFMQ, the SCS, the LOSC or the STAI-T. The present study consisted of pilot work, thus, the small number of participants in each condition may have impacted statistical power. Future studies should try to replicate the results with greater numbers of participants.

Negative health behaviors can result as well as poor mental health outcomes such as depression and anxiety. Although the relationship between perceived stress and biobehavioral outcomes is well-established, less is known about protective factors that could buffer these effects. Self-compassion and forgiveness are such protective factors that may reduce these negative health outcomes. Self-compassionate individuals possess the ability to self-soothe when facing failure and react to disappointment with self-kindness and patience (Hope et al., 2014). Higher levels of self-compassion are also associated with higher levels of forgiveness (Neff & Pommier, 2010). Using an anonynmous survey of undergraduate college students (aged 18-24 years), this study examined whether self-comapssion and forgiveness buffer against biobehavioral effects of stress in a US national sample (N=217). The survey included measures of stress, depressive symptoms (CESD-10), trait anxiety (STAI), self-compassion, tendency to forgive and body mass index. Participants reported clinically significant levels of depression levels and anxiety. Trait forgiveness was positively associated with self-compassion [r = +0.33, n = 173, p 0.05]. This dissertation used a quasi-experimental design to examine the effects of mindfulness-based cognitive therapy (MBCT; Segal & Teasdale, 2002) on 21 psychology graduate students. The correlations between mindfulness, self-compassion, and satisfaction with life were also examined. Outcome measures include Mindful Attention Awareness Scale (MAAS-Brown & Ryan, 2003), Five Factor Mindfulness Questionnaire (FFMQ, Baer, Smith, Hopkins, Krietemeyer, & Toney, 2006), Self-Compassion Scale (SCS, Neff, 2003a), and Satisfaction with Life Scale (SWLS; Diener, Emmons, Larsen, & Griffin, 1985). Contrary to expectation, the results indicate no statistically significant differences between MBCT and control group on any of the outcome variables. Additional analyses indicated that the common humanity factor of self-compassion was significantly correlated with life satisfaction. Mindfulness and self-compassion also demonstrated positive significant correlations.

At baseline (Time 1), participants completed measures of self-critical (SC) and personal standards (PS) higher-order dimensions of perfectionism. Participants completed mindfulness and self-compassion measures one year later (Time 2), and depressive and anxious symptom measures at Time 1, Time 2, and two years after baseline (Time 3). In contrast to PS, SC perfectionism was moderately to strongly associated with lower levels of mindfulness and self-compassion domains and facets. Hierarchical regression analyses of moderator effects demonstrated that, for individuals higher in SC perfectionism compared to those lower, both domain mindfulness and self-compassion predicted decreases in depressive and anxious symptoms over two years, adjusting for the effects of Time 1 and Time 2 symptoms. For high SC perfectionistic individuals, specific mindfulness facets of observing, describing, nonjudging of inner experience, and nonreactivity to inner experience predicted lower depressive and anxious symptoms at Time 3. Specific self-compassion facets of self-kindness, common humanity, and mindfulness predicted decreases, while self-judgment and over-identification predicted increases, in depressive and anxious symptoms for high SC perfectionists at Time 3. Previous research has demonstrated that a variety of mindfulness and self-compassion interventions can improve stress, burnout, and self-compassion in this population. However, these programs are not consistently implemented and, given the costly nature of program implementation, it is imperative to improve the effectiveness of interventions on psychological and behavioral health outcomes through systematic development. The specific aims were to: 1) assess feasibility and acceptability of the Self-compassion, Yoga, and Mindfulness for Burnout: Integrating Online Sessions and Interpersonal Support (SYMBIOSIS) program, and 2) evaluate preliminary psychological and behavioral health outcomes. A mixed-methods approach was utilized to investigate feasibility, acceptability, and effects of the program using validated measures and open-ended questions. Participants were 20 first- and second-year medical students at one U.S. mid-western medical college who participated between October 2020 to December 2020. Participants were enrolled in one of two groups for the virtual 8-week program via Webex. Participants completed surveys at pre, post, and four weeks after the intervention. They also completed weekly home practice assessments. Of the 19 participants who completed the program (95% retention rate), all participants attended 6 or more sessions. Repeated measures ANOVAs revealed that participants had significant improvements in burnout, self-compassion, perceived stress, and health promoting behaviors across time. These results were supported by qualitative themes of increased social support, wellness skills, and overall positive experiences. Findings suggest that the SYMBIOSIS program was feasible and acceptable to medical students, and improved psychological and behavioral health outcomes.

Book excerpt: At its core, Social Anxiety Disorder (SAD) is an intense fear where an individual is afraid of being rejected, humiliated, embarrassed, or negatively judged by others in social situations. Due to these feelings, those affected by SAD avoid interpersonal situations, which maintain and worsen the disorder. SAD affects 15 million adults and impairs daily functioning in countless aspects. Through various research studies, evidence has shown that individuals who suffer from SAD have difficulty managing their emotional states such as fear and anxiety and are less willing to accept and forgive themselves than their healthy peers. Willingness to accept, be kind, and forgive one's self is known as self-compassion. It is not clear in what way self-compassion effects the anxious and fear emotional states that define SAD. As fear is an immediate response to manifest danger, it is likely that self-compassion is more closely tied to anxiety that is prospective in nature. Therefore, this study examines if effects of self-compassion are more pronounced for anxiety rather than fear in a distressing task. Undergraduate students (N=130) completed the self-compassion measure on a computer and participated in a Free Breathing task (measure baseline) and the Trier Social Stress Test (measure distress) where they prepared (anticipatory state) and delivered (fear state) a speech in front of researchers. Participant's negative affect (nervous and scared) ratings were obtained following each task. Results concluded that self-compassion had a trend effect in decreasing negative affect equally for both the anticipatory and fear tasks relative to baseline.

Book excerpt: Health care professionals are at higher risk for work-related stress, burnout and compassion fatigue than other professionals. One coping strategy that has attracted considerable attention is the practice of self-compassion. Self-compassion training is associated with psychological benefits such as decreased self-criticism and rumination and increased connectedness, mindfulness, and life satisfaction. (Smeets et al., 2014). While there is preliminary evidence that online self-compassion training is as effective as in-person training (Finlay-Jones et al., 2016), length and duration of online programs may affect adherence. Accordingly, this study assessed the feasibility of a short, 4-week online self-compassion program tailored for students in health care professions. Since this was an exploratory study, there was no control group. A convenience sample of 31 students who were registered in health care profession programs at the University of Ottawa and other Canadian universities were recruited. Key feasibility indicators included ease of recruitment, retention, completion of outcome measures, and acceptability of the program (adherence, satisfaction, ease of use). The study also obtained preliminary data on the effects of the intervention on levels of self-compassion, mindfulness, depression, anxiety and stress. Recruitment was easier than expected, with 31 participants enrolled in the study within a 2-month period. Of these, 21 started the self-compassion training. Adherence to the daily self-compassion meditations was poor, with only four participants meditating at least 50% of the expected number of days. Participants rated the self-compassion meditations as easy to follow and satisfaction ratings were acceptable for three of the four meditations. However, only two of the meditations were perceived to be relevant to healthcare professions. Intent-to-treat analyses revealed that the mindfulness facet Nonjudge and levels of stress improved from pre-to- post study. Separate analyses on program starters revealed that the mindfulness facets Describe, Nonjudge, and Act with Awareness, and levels of stress and anxiety improved. However, it is unlikely that these changes were attributed to the meditation practice given the poor rate of adherence. Overall, results suggest that this brief online self-compassion meditation training may not be feasible for healthcare trainees at this time without certain modifications to the program.

Curricula and the availability of compassionate role models may also have a role in promoting compassion.CONCLUSIONSSelf-compassion has been shown to relate to positive personal and academic outcomes in students, and studies demonstrated that it can be improved using brief structured interventions. Further ways of enhancing medical studentsu2019 compassion during training, and its later impact on health care related outcomes, are to be explored.

Book excerpt: AIMS: This study aimed to investigate the effect of a compassion-focused imagery intervention and a compassionate psycho-education intervention on levels of self-compassion, self-criticism and state mood compared with a control condition in healthy adolescents. // METHOD: Fifty-one participants were randomly allocated to a compassionate imagery and psychoeducation group, a compassionate psychoeducation group or a no-intervention control. The interventions were delivered in three secondary schools in London. State measures of self-compassion, self-criticism, and mood were collected at pre- and post-intervention, and at a two-week follow up. Participants were asked to complete daily homework tasks related to each intervention. // RESULTS: Self-compassion was found to increase with a large effect size following the psychoeducation group but not in the imagery condition. Contrasting patterns were found across groups for different elements of self-compassion. Compassionate behaviour increased in the imagery group with a large effect size but not in the psychoeducation group, and uncompassionate behaviour decreased only in the psychoeducation condition with a medium-to-large effect size. Higher levels of self-compassion were associated with lower levels of self-criticism at baseline, however, no effects were found on self-criticism and state mood following the interventions. // CONCLUSIONS: This randomised-controlled study provided evidence for the potential of a brief compassionate psychoeducation intervention to increase self-compassion in adolescents. The results were inconclusive on the impact of compassionate mental imagery but it may be effective at enhancing the 'positive elements' of self-compassion, including self-kindness, mindfulness and common humanity. The absence of an effect on self-criticism suggests the potential existence of different pathways to enhancing self-compassion and reducing self-criticism. Future research is needed to address the shortfalls of this exploratory study, which was limited by a small sample size.

Book excerpt: College counseling centers are tasked with intervening in student personal problems in order to facilitate academic success (Sharkin, 2004). Student psychopathology, of which anxiety and depression are the two most common clinical presentations (American College Health Association, 2013; CCMH, 2014), has been shown to significantly interfere with academic performance in indirect ways (Brackney & Karabenick, 1995). Therefore, identifying factors that support healthy normal development expected in college students, yet are effective at intervening with depression and anxiety could be helpful in guiding therapists' goals in psychotherapy. Self-compassion is a mindfulness-based positive self-attitude that has been touted as an important component of psychological well-being (Neff, 2012). Although there is a significant body of research linking self-compassion to indicators of psychological well-being (Neely, Schallert, Mohammed, Roberts, & Chen, 2009; Neff, 2003a, 2011, 2012; Yarnell & Neff, 2013) and it is associated with less psychological pathology (Lockard, Hayes, Neff, & Locke, in press; MacBeth & Gumley, 2012; Neff, 2003a, 2012; Neff, Kirkpatrick, & Rude, 2007; Van Dam, Sheppard, Forsyth, & Earleywine, 2011), research has yet to determine its importance in a psychotherapeutic context. The current study evaluated whether self-compassion predicts psychotherapy change and can moderate the relationship between initial distress and change in psychotherapy. Results supported previous research suggesting self-compassion is inversely related to distress at pre-treatment. Self-compassion predicted changes on most CCAPS change scores when used as the only predictor in the model; however, when initial distress was added, the self-compassion effect size dropped below significance. Interestingly, there was a significant interaction between initial distress levels of anxiety and eating concerns on change scores; self-compassion facilitated gains in therapy for those low in initial distress, but was associated with less gains in therapy for those high in initial distress. Results are discussed relevant to the literature.
