Book excerpt: Purpose: of this study was to test the effect of low to moderate intensity exercise on fatigue and physical functioning in women who have completed treatment for breast cancer. Sample: Twenty-two women, ages 43-79, who had completed treatment for breast cancer, were randomly assigned to an exercise (n= 12) or control group (n = 10). The exercise group participated in a low-moderate intensity (30-50% heart rate reserve) aerobic exercise program 3 times a week for ten weeks. The control group did not participate in the exercise program. Physical functioning was measured by assessing peak aerobic capacity with a treadmill protocol. Both groups recorded their weekly level of fatigue using a Linear Analogue Self-Assessment Scale (LASA) and the Schwartz Cancer Fatigue Scale (SCFS). Results: Peak aerobic capacity increased significantly (25%, p = .005) in the exercise group. The control group showed a decrease of 5.2%. Fatigue reported with the Schwartz scale decreased in the exercise group and increased in the control group. This change was statistically significant between the groups (p = .0003) and within the groups (p =. 01). Fatigue reported with the LASA scale decreased in the exercise group and increased in the control group but the differences were not statistically significant between (p =. 17) or within (p = .10) the groups. Discussion: This aerobic exercise program was effective in improving peak aerobic capacity and reducing fatigue (SCFS) in this group of breast cancer survivors. Guidelines for an exercise intervention are already in place for chronic diseases (diabetes, heart disease), but no guidelines are in place for cancer patients and survivors. Lower intensity aerobic exercise should be considered for this population. Low to moderate intensity exercise is a safe, beneficial, efficient, and cost-effective tool for improving fatigue in breast cancer survivors.

The purpose of this study was to examine the effect of aerobic exercise on physiological and psychological function in patients rehabilitating from cancer treatment. A second purpose was to evaluate the differential effects of low and moderate intensity exercise on these variables. Eighteen survivors of breast or colon cancer (15 female and 3 male, 40-65 years of age) served as subjects. The subjects were matched by aerobic capacity and scores on a Qua1ity of Life questionnaire, and then randomly assigned to a control, low (25-35% heart rate reserve (HRR)) or a moderate (40-50% HRR) intensity exercise group. The exercise groups performed lower body aerobic exercise three times a week for ten weeks. Subject attendance for the exercise sessions was 95%. Following the exercise training, there were no statistically significant differences between the two exercise groups on any of the physiological variables. Book excerpt: We appraised seven articles examining the effects of a walking program on quality of life (QOL), fatigue, and aerobic functional capacity in individuals receiving chemotherapy or radiotherapy for cancer. Five of the reviewed studies had moderate to good methodological quality and contributed to our overall clinical bottom line. In these studies, individuals who walked (total of 3 to ¡©+24 hours) clinically improved their QOL and decreased their cancer-related fatigue, and statistically improved their aerobic functional capacity (or effectively mitigated significant decreases in aerobic functional capacity) compared to individuals who received usual cancer care. Three of the five articles specified that the aerobic walking program was performed at moderate intensity (50-70% of max heart rate). In one study with a population of women one to three years post breast cancer treatment, a two-month moderate intensity aerobic exercise program increased fatigue levels compared to the usual care group. This may have been because the intensity was too vigorous and exercise-related fatigue may have been misinterpreted as cancer-related fatigue. Therefore, we recommend close monitoring of exercise intensity and post-exercise fatigue levels to minimize the potential of causing exercise-induced fatigue in previously inactive individuals. Based on this evidence, we recommend an average of ¡©+60 minutes per week of light to moderate intensity walking during cancer treatment to increase QOL, decrease cancer-related fatigue, and improve overall aerobic functional capacity. These recommendations can apply to adults undergoing chemotherapy or radiotherapy for cancer (with treatment regimens and courses similar to those prescribed to study participants for each respective type of cancer) for whom safe exercise intensities can be established. Since the interventions were walking programs, we cannot generalize our recommendations to a non-ambulatory population or to those who have other co-morbidities that prevent walking at the required heart rates. Physical therapists can effectively screen patients for safe aerobic exercise by using an assessment or screening tool such as the Duke Activity Status Index. Future studies need to investigate the role of the intensity component aerobic of exercise to determine the appropriate aerobic exercise intensity to best improve QOL, fatigue, and aerobic functioning during cancer treatment.

However, few studies have focused on assessing the effect of activity on these symptoms after treatment is complete. Using a correlational design, this study aimed to explore associations between physical activity, sleep, fatigue and mood in women who had completed treatment for breast cancer and to evaluate the reliability and validity of the short-version International Physical Activity Questionnaire. Methods: Twenty-eight women (aged 43 to 75 years) with stage I and II breast cancer were recruited at 6-months post-diagnosis and after completion of active treatment. Respondents completed measures of activity, sleep, fatigue, depression and anxiety. Six participants also undertook actigraphic monitoring to obtain objective activity levels. Results: Descriptive analyses suggest the sample was relatively active with 50% of participants engaging in moderate-intensity activities. Despite this, however, only 18% were sufficiently active to meet national guidelines. No significant relationships were found between total physical activity, sleep, fatigue or depression, whereas, anxiety and activity were significantly correlated. Reliability of the IPAQ was low, however, comparison with objective actigraphy data suggests high criterion validity. Conclusion: These findings have implications for designing interventions to reduce anxiety among breast cancer patients returning to physical activity after treatment. However, the choice of assessment instrument may have a significant impact on research results.

Book excerpt: BACKGROUND: Breast cancer patients typically present with impaired glucose metabolism at diagnosis. Given that increased fat mass and decreased lean mass develop throughout the treatment trajectory and into survivorship, these impairments in glucose metabolism may worsen and may increase the risk of breast cancer patients developing secondary diseases such as diabetes in survivorship. Aerobic and resistance exercise training are known to improve glucose regulation in various clinical populations, but this has not been comprehensively explored in breast cancer patients. OBJECTIVES AND HYPOTHESES: The primary objectives of this study were to: 1) examine the differences in glucose and related markers, as well as potential explanatory measures such as body composition, inflammatory markers, aerobic capacity, muscular strength, and dietary intake between a heterogeneous group of breast cancer patients and a group of young healthy females, and 2) evaluate the potential changes in these collective measures in breast cancer participants following 24 one hour sessions of a combined cardiovascular and strength training program. Secondary objectives were to: 1) examine potential effects of the time to complete 24 one hour sessions on metabolic parameters and 2) evaluate baseline measures of the breast cancer group that dropped out to those that completed the exercise program to identify potential bias in the group that completed the exercise program. We hypothesized that breast cancer participants, prior to exercise training, would present with poorer glucose concentrations and related markers, body composition, inflammatory markers, aerobic capacity, muscular strength, and dietary intake in comparison to the healthy young reference group. We also hypothesized that all measures would improve with the exercise program and that dietary intake would ! iii! remain the same. Additionally, we anticipated that patients who completed the 24 exercise sessions in a shorter time-frame would exhibit greater improvements in glucose- related parameters compared with those who took longer to complete the program. Baseline measures between breast cancer exercise completers were expected to be similar to those that dropped out of the program. METHODS: Breast cancer patients, at any stage of disease, any phase of treatment and any treatment time, were recruited to this study. Participants underwent a supervised exercise program consisting of 24 1-hour sessions of combined aerobic and resistance exercise (usually aerobic training to start followed by resistance training). Attendance of two exercise sessions per week for one hour was prescribed. Assessments were done at baseline and following the exercise program for the breast cancer patients Baseline assessments from the patients were compared with similar measures collected, at a single time point, in the young healthy female reference group. We evaluated glucose and related markers by biochemical analysis on fasting blood. Body composition was assessed by dual energy x-ray absorptiometry (DXA) scans, body mass index (BMI) and waist circumference (WC). Cardiovascular capacity in breast cancer patients was measured by a graded exercise test and strength was assessed using a predictive one repetition maximum (1RM) test on the upper and lower limbs. Dietary intake was assessed using 3-day food diaries. RESULTS: Prior to exercise training, breast cancer patients presented with higher measures of fasting plasma glucose, glycated hemoglobin, IGF-1, insulin and a tendency for higher c-peptide when compared to the young healthy reference group; however, there were no differences in inflammatory markers between groups. Furthermore, ! iv! percentage of fat, total fat mass, BMI and WC were elevated in breast cancer patients compared with the reference group. Aerobic exercise tests, resting heart rate and muscular strength demonstrated poorer results relative to the reference group. Daily caloric intake was higher in the reference group but macronutrient distribution was not significantly different. Following 24 sessions of combined aerobic and resistance training, there were significant reductions in fasting plasma glucose (5.4 ± 0.7 vs. 5.0± 0.4 mM, p=0.008) and IGF-1:IGFBP-3 (0.33 ± 0.12 vs 0.27 ± 0.09 ng/mL, p= 0.018). Homeostatic model assessment for insulin resistance (HOMA-IR) (2.9 ± 2.4 vs. 2.1 ± 1.7 ng/mL, p= 0.06) tended to improve. IGF-1 and lactate tended to decrease following the exercise program as well. Inflammatory markers primarily remained unchanged but a significant increase in TNF-[alpha] as well as a tendency for increased IL-1[beta] were observed. Increasing delta TNF-[alpha] (from baseline to post-exercise program) was correlated with increasing delta of total fat mass (r= 0.908, p=0.0124, n= 6). Cardiovascular capacity, resting blood pressure as well as lower limb muscular strength improved following the exercise program. There were no changes in dietary intake during the exercise program. No differences were identified between the breast cancer dropout group and the breast cancer group that completed the exercise program. DISCUSSION AND CONCLUSIONS: Breast cancer patients presented with various metabolic, body composition, and fitness parameters that were poorer than the young healthy reference group. The exercise intervention was associated with improved plasma glucose, HOMA-IR and IGF-1:IGFBP-3, suggesting improvements in glucose metabolism. Future work needs to further evaluate these improvements and compare them with a usual care group that is undergoing treatment but no exercise intervention.
