Book excerpt: Abstract: Cold water immersion (CWI) is commonly used by endurance athletes to speed recovery and maintain performance during subsequent exercise bouts. The purpose of the present study was to investigate the effects of CWI following an acute bout of intense aerobic exercise on 5 km run performance, perceived muscle soreness (PMS) , range of motion (ROM), thigh circumference, and perceived exertion. Nine endurance trained males (35.9 ÷ 7.8 years old) completed 2 trials consisting of 8 repetitions of 1200 m at a running pace equal to 75% ofVO2peak. Following exercise, subjects underwent CWI (12 mm at 12°C) on the lower body or no treatment. A timed 5 km run was completed 24 hr post exercise. Exercise induced muscle damage (EIDM) is the micro trauma that occurs during strenuous exercise. Recovery is the physiologic response period following intense exercise where the body repairs EIMD, adapts to exercise, and refuels tissue for performance. There are many physiological and psychological factors involved in recovery. For example, active recovery may be advantageous because it promotes greater blood circulation to the damaged tissue. CWI may be advantageous because it serves to limit inflammation. By manipulating these factors it was hypothesized that CWI would be the most effective method of recovery. The Hanover College Institutional Review Board approved this study.

Book excerpt: Achieving adequate and appropriate recovery from exercise is essential in ensuring optimal performance during repeated bouts of exercise. The use of various recovery interventions has become popular in an attempt to enhance subsequent performance and accelerate post-exercise recovery. The application of various post-exercise hydrotherapy interventions has become increasingly popular, however, the majority of current recovery practices appear to be based largely on anecdotal evidence as opposed to rigorous scientific research or evidence based findings. Physiologically, various hydrotherapy protocols have been shown to affect the body via fluid shifts (interstitial to intravascular space), changes in blood flow and cardiovascular function, and reductions in oedema. The possible psychological effects of water immersion must also be considered, with athletes commonly reporting reduced sensations of fatigue and soreness following immersion. Current literature suggests both hydrostatic pressure and water temperature to be important factors influencing the success of hydrotherapy. The overall aim of the present thesis was to enhance current knowledge and understanding with regards to the physiological and performance effects of various forms of hydrotherapy, used as a post-exercise recovery intervention. Initially, four cold water immersion interventions were compared to active recovery, performed between two bouts of high intensity cycling in hot environmental conditions. Effectiveness of recovery was determined via performance in a subsequent exercise bout; in addition, core body temperature, lactate, and heart rate were recorded. The remaining studies were designed to investigate the effects of cold water immersion, hot water immersion, contrast water therapy, and passive recovery 4 (control) following exercise-induced fatigue and exercise-induced muscle damage. Rate of recovery was assessed through changes in performance, core body temperature, thigh girths, blood markers, and perceived exertion/soreness. The results of the combined studies indicate cold water immersion to be more effective than active recovery when performed immediately post-exercise between two bouts of high intensity cycling in hot environmental conditions. Additionally, both cold water immersion and contrast water therapy were effective in aiding recovery from exercise-induced fatigue and exercise-induced muscle damage. Performance variables indicated an improved maintenance or return of performance following these recovery protocols. The present studies have provided additional information to the limited knowledge base regarding the effect of post-exercise hydrotherapy interventions, specifically, the effect of such interventions on subsequent athletic performance. In conclusion, cold water immersion and contrast water therapy appear to be superior to hot water immersion, active recovery, and passive recovery following fatiguing and muscle damaging exercise. Functional and physiological recovery was enhanced following the use of these two recovery protocols.

Book excerpt: Physiological responses after maximal and submaximal exercise are routinely monitored in a plethora of diseases (e.g. cardiovascular diseases, cancer, diabetes, asthma, neuromuscular disorders), and normal populations (e.g. athletes, youth, elderly), while slower or irregular post-exercise recovery usually indicates poor health and/or low fitness level. Abnormal post-exercise recovery (as assessed via blunted post-exercise heart rate dynamics) helps to predict the presence and severity of coronary artery disease, while differences in recovery outcomes in athletes might discriminate between fit and unfit individuals. Disturbances in post-exercise recovery might be due to acute or persistent changes in: (1) adaptive responses mediated by the autonomic nervous system and vasodilator substances, (2) cellular bioenergetics, and/or (3) muscular plasticity. Preliminary evidence suggests possible role of time-dependent modulation of nitric oxide synthase and adenosine receptors during post-exercise recovery, yet no molecular attributes of post-exercise recovery are revealed so far. Currently several markers of post-exercise recovery are used (e.g. heart rate measures, hormone profiles, biochemical and hematological indices); however none of them meets all criteria to make its use generally accepted as the gold standard. In addition, recent studies suggest that different pharmacological agents and dietary interventions, or manipulative actions (e.g. massage, cold-water immersion, compression garments, athletic training) administered before, during or immediately after exercise could positively affect post-exercise recovery. There is a growing interest to provide more evidence-based data concerning the effectiveness and safety of traditional and novel interventions to affect post-exercise recovery. The goals of this research topic are to critically evaluate the current advances on mechanisms and clinical implications of post-exercise recovery, and to summarize recent experimental data from interventional studies. This knowledge may help to identify the hierarchy of key mechanisms, and recognize methods to monitor and improve post-exercise recovery in both health and disease.

Sleep is widely considered to be the most important period for achieving optimal recovery post-exercise, however, sleep research in athletes is limited. One of the most popular post-exercise recovery techniques is cold water immersion (CWI). While there has been some research into the subsequent performance benefits (~60 min to 72 h) of post-exercise cold water immersion, there has been limited research into the physiological responses in the post-immersion (~90 min) period. As athletes often perform CWI in the evening post-exercise, research is needed to investigate the effects of CWI on subsequent sleep quality and quantity. The purpose of this thesis was to investigate the effect of evening high-intensity exercise, plus any effect of CWI performed post-exercise, on subsequent sleep quality and quantity. Specifically, study 1 investigated the core temperature and salivary melatonin responses to 15 min of either cold (14°C) or warm water immersion (WWI: 34°C) after a high-intensity cycling protocol in the evening. Study 2 investigated the effect of early evening high-intensity training sessions (over a training phase) on the sleep of elite youth soccer players in their normal sleeping environment using wrist actigraphy, by comparing high-intensity and low-intensity/no training nights. Additionally, as the players regularly engaged in CWI post-training (15 min in 14°C) it was possible to explore any effects of CWI on subsequent sleep. Study 3 combined elements of study 1 and study 2 together, by investigating the effect of evening highintensity cycling with and without the addition of CWI (15 min in 14°C) performed post-exercise compared to a control (no-exercise, no CWI) condition on the sleep of healthy male athletes. Sleep was measured using polysomnography (PSG), the gold standard of sleep measurement. Core temperature, skin temperature and salivary melatonin were also measured as these have all been linked to sleep mechanisms and there has been limited research surrounding their response to evening exercise and CWI. The results from these studies show that performing CWI post-exercise will decrease core temperature at a greater rate than both WWI and a control condition, with the effect lasting for ~2 h post-exercise. In addition, salivary melatonin is unaffected by both high-intensity exercise and CWI, even when controlling for light exposure, which usually inhibits the secretion of melatonin. However, most importantly, high-intensity exercise performed in the evening with or without the addition of CWI performed postexercise does not affect whole night sleep quality and quantity of athletes as measured by both actigraphy and PSG. Therefore, athletes and coaches can be confident that performing CWI post-exercise in the evening does not negatively affect the subsequent sleep of athletes.

Previous evidence has proposed that acute increases in oxidative stress induced by SIE cause a long-lasting depression in submaximal force generation called prolonged low-frequency force depression (PLFFD). The focus of this thesis was to test the hypothesis as to whether CWI can enhance recovery of neuromuscular function by decreasing oxidative stress and mitigating the extent of PLFFD. PLFFD was examined by measuring torque from evoked stimulation at low- and high-frequencies before, immediately after and 24h post-exercise. Results showed that PLFFD was observed at 24h post-exercise but no difference was seen in the extent of PLFFD between the cooled and control leg. These results do not support any benefit of acute 30 min post-exercise CWI on the recovery of skeletal muscle function at 24h following SIE.

In recent years, post-exercise hydrotherapy techniques have been increasingly used by athletes to accelerate recovery between training sessions and competitions. The published literature suggests that both cold water immersion (CWI) and contrast water therapy (CWT) are more likely to assist recovery of performance compared with hot water immersion (HWI) or thermoneutral water immersion (TWI). However, conflicting findings exist, possibly due to the methodology used. Hydrotherapy techniques may affect recovery of performance through their influence on circulation, body temperatures, and both the musculoskeletal and neural systems. Despite the widespread use of CWT by athletes, limited literature is available on its ability to influence the recovery of performance. Furthermore, the mechanisms by which hydrotherapy techniques can assist recovery are uncertain. Therefore, the purposes of the present series of studies were to investigate the effects of post-exercise CWT on recovery of high-intensity fatiguing exercise performance, determine whether a dose-response relationship exists, and to gain a greater understanding to the mechanisms by which hydrotherapy techniques can assist recovery of performance. The first two studies of the present thesis examined the effects of performing post-exercise CWT (full body excluding head and neck, alternating 1 min in 38{486}C and 1 min in 15{486}C water) for 6, 12 and 18 min duration on recovery of high-intensity cycling and running performance 2 h later. Both studies used a randomised, counterbalanced crossover controlled design. The collective results suggested that post-exercise CWT can assist recovery of both high-intensity cycling and running performance compared with seated rest control. Specifically, performing CWT for 6 min substantially accelerated recovery of both cycling and running performance, while CWT for 12 min was only substantially beneficial for cycling performance, possibly due to the cold environmental conditions experienced by participants while running outdoors. Furthermore, CWT for 18 min had no substantial effect on recovery from either mode of exercise compared with control. The purpose of the study was to compare the effect of cold-water immersion (CWI) and static stretching (SS) on individual heart rate recovery and overall muscle soreness, utilizing the Short Form McGill Pain Questionnaire 2, following high intensity exercise. The study included 10 collegiate male (n = 9) and female (n = 1) Division III collegiate soccer athletes who averaged 5.60 hrs of physical activity per week. Baseline heart rate values were obtained prior to participants completing 15-minute bout of cycling at 75-85% max heart rate. Heart rate and soreness scores were recorded post exercise and post intervention (CWI or SS) with a final soreness score recorded 24 hrs post intervention. Participants completed both recovery protocols, in a randomized order, separated by a minimum of 48 hrs. All heart rate and soreness values were run through a repeated measures factorial ANOVA for statistical analysis. No significant interaction of condition and timing on heart rate was present (F(2, 18) = 1.940, p = .173). A significant interaction of condition and timing on soreness was found (F(2, 18) = 6.915, p = .006). Both SS and CWI positively impacted soreness levels with SS causing a significant change in soreness scores. The findings of the conducted study show a benefit in overall muscle soreness via SS following high intensity exercise.
