The significance of these factors is an important consideration to public health advocates and government agencies, health insurance companies, pharmaceutical firms, physician and provider associations, medical ethics researchers, patient consumers, and other stakeholders in health care. Better understanding of the factors that motivate physician prescribing can bring about more efficient use of health care resources through the improved dissemination of information and more informed decision making. This paper attempts to provide a sound and inclusive review of the influences upon physician prescribing patterns in the extant literature. This review identifies several factors as being highly influential upon physician prescribing: peer influence, financial and managed care considerations, pharmaceutical representatives and drug samples, and direct-to-consumer advertising. To date, several key areas have not been well researched; these include the perceived price of a drug, the role of new technology, and the impact of physician experience. Based upon the literature review, a theoretical model of individual physician prescribing in the outpatient setting is proposed. Further research might be directed toward determining the influence of each factor upon physician prescribing and testing the overall model and interactions among its individual components.

Book excerpt: This study evaluates the effect of a specially designed, physician-oriented handbook of antimicrobial use on the prescribing patterns of a group of fifty doctors at a university hospital. Data were evaluated over a peroid of one-and-one-half years, before and after the distribution of the handbook. For the purposes of this study, antimicrobial therapy was classified: (1) inappropriate if it violated one of a number of recognized principles of antimicrobial therapy, (2) appropriate if it agreed with specific recommendations or alternatives given in the distributed reference handbook, and (3) acceptable if it was neither inappropriate nor appropriate as defined by the handbook. An initial survey of antimicrobial prescribing patterns was made. Five months later the handbook was distributed and a two-week orientation program, consisting of the distribution and promotion of the problem-oriented, pocket-size handbook of appropriate antimicrobial therapy, was conducted. The handbook, which was developed by the authors and reviewed and approved by a panel of infectious disease specialists, presented guidelines for appropriate and efficacious usage of antimicrobial agents as most currently accepted in common clinical infections. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book excerpt: The structure of sequential behavior and its patterns have attracted the attention of researchers from various disciplines, such as game theory, human and animal behavior, evolutionary ecology, psychology, behavioral economy, and cognitive sciences. Sequential behavior seems to be involved in the processes of sequential learning, decision-making processes, and exploratory behavior. There are, however, competing hypotheses regarding the mechanisms involved in the complexity of the behavioral responses of organisms when exposed to changing environments. The behavioral response to different environmental conditions is often utilized in behavioral ecology studies, where the changes in behavior are quantified in terms of probability of dispersal, distance, and speed of movements or time budget, where the time spent on different activities (foraging, resting, explore, etc.) is registered and analyzed in terms of cost–benefit. This book represents a series of articles covering a broad spectrum of organisms and proposes the analysis of sequential behavior utilizing indices commonly applied in the estimation of developmental instability (fluctuating asymmetry, directional asymmetry, and antisymmetry) toward estimating the degree of “Behavioral Instability”. Deviations from symmetry will be interpreted in ecological and evolutionary terms, where the cost and benefits of changes of the degree of behavioral instability will be analyzed in terms of overall costs and benefits and its evolutionary significance. Presented in this collection are multidisciplinary studies (theoretical and computational as well as experimental and empirical approaches) that validate the proposed approach and pave the way for future investigation in the novel field of what is best described as behavioral instability.
