Book excerpt: The effect of participant-selected music listening on pain perception, pain tolerance, and attention to pain was evaluated during a cold-pressor pain protocol. Participants (N=50) underwent two, ice-water hand immersions in a music condition and a silence condition. Participants were asked to engage in active music listening of their choice during the cold-pressor music condition. The researcher collected a pain tolerance hand-immersion time, while participants self-reported on pain perception and attention to pain. Results showed that participants reported significantly less pain perception and attention to pain, and greater pain tolerance time under the music condition than in the silence condition. The positive results are attributed to the ability of participant-selected music to effectively engage attention. The participant-selected music choices were analyzed for musical attributes that served as possible predictors for influencing attention to pain. Of the five attributes analyzed including activation, tempo, mood, lyrics, and preference, only preference yielded significant results in diminishing attention to pain. Conclusions are made as to the influence of music on attention, pain tolerance, and pain perception, and support is given for preferred music as an effective means to engage attention for pain management.

With the multitude of pain-related consequences that can be experienced, the biopsychosocial perspective examines the chronic pain experience using a multifactorial approach that examines all aspects of life impacted by the chronic pain experience (Gatchel et al., 2007). In examining aspects that have impacted the chronic pain experience, religion has been found to be a factor. Previous research has shown that religious practitioners experience a high likelihood of turning towards religion in an effort to seek an understanding regarding what is happening to them (Pargament & Raiya, 2007). With music being a common pain intervention, being able to choose what music to listen to has been found to be related to positive outcomes (Mitchell et al., 2007). For religious individuals, religious music has been found to be a coping mechanism in response to stressful situations (Safara & Samanesadatsadidpoor, 2014). The exact mechanism for the relationship is unknown, however, when both music and religion are important, the effect of music as a coping mechanism may be amplified (Hilliard, 2005). The main goal of this study is to examine how religious and non-religious music impacts the chronic pain experience. In addition, religion and the chronic pain experience will be examined. Participants were recruited through Amazon's Mechanical Turk (MTurk) in order to gather an adult sample of individuals with pain. Statistical analyses found that individuals who scored high on measures of religiousness also reported listening to music that reminded them of their religion. It was also found that there is a strong cognitive associative context in which music is used for pain management. Implications from this research include providing resources for religious individuals experiencing chronic pain, best practices for studying religion in a psychological context, and tailoring non-pharmacological pain interventions.

Book excerpt: The purpose of this project was to investigate the impact of differentiated onset of self-selected music on pain perception and pain tolerance during a cold pressor test. Subjects participated in four trials during which music was presented at different points of time in relation to their exposure to the cold pressor test. Results indicated that listening to music prior to and concurrently with the onset of the pain resulted in lower self-reported pain. Behavioral results indicated that subjects were able to tolerate an average of 25 seconds longer when music was presented after the onset of painful stimuli. Both results have positive implications for the clinical use of music as a non-pharmalogic analgesic method of reducing pain perception and increasing pain tolerance. The onset of music as a pain mediation stimulus may be differentially indicated based on the nature of the procedure. Future research could examine the amount of pre-procedure time indicated to be most effective toward pain perception and tolerance. It is currently unknown if a longer induction period would differentially impact any of the outcome measures and if listening to a song in its entirety prior to exposure to adverse stimuli would have an impact on the outcome measures. Following controlled laboratory studies, translational research would be required to examine clinical efficacy.

There is abundant behavioural evidence to support music's pain relieving properties, however, studies to date have yet to investigate music-induced analgesia via imaging. Our first imaging study used thermal stimulation just below pain threshold in combination with various music stimuli, to determine whether music can affect neural activity in response to heat stimuli within brainstem and spinal cord regions. Differential responses to music stimuli were found within regions known for descending modulation, and familiar classical music had a unique effect on neural activity in these regions compared to unpleasant music, reverse music, and no music. This study confirmed that the emotional valence of music affects neural activity in the brainstem and spinal cord. The second study used a well-defined pain paradigm applied with or without favorite music to study the neural activity responses in the brain, brainstem, and spinal cord using imaging. Subjective pain ratings were significantly lower when painful stimuli were administered with music than without music. The pain condition alone elicited neural activity in brain regions consistently activated during similar pain studies. Brain regions associated with pleasurable music listening were activated including limbic, frontal, and auditory regions when comparing music to non-music pain conditions. In addition, neural regions showed activity responses indicative of descending modulation when contrasting the two conditions. These regions include the spinothalamic tract, dorsolateral prefrontal cortex (DLPFC), periaqueductal grey (PAG), rostral ventromedial medulla (RVM), and the dorsal gray matter of the spinal cord. The data suggest that music seems to engage mesolimbic and mesocortical brain regions to activate the descending pain modulation pathway. Lower subjective pain ratings corresponded to a greater suppression in the dorsal gray matter when listening to music. This is the first imaging study to characterize the neural response of pain and how it is mitigated by music listening, and brain and spinal fMRI are appropriate means to study pain processing and its modulation in the central nervous system.
