Book excerpt: BACKGROUND: Demineralisation around orthodontic brackets is a considerable risk of orthodontic treatment. It can occur very early during treatment and involve a large number of teeth with aesthetic and dental health consequences. Early detection of demineralisation is very important in terms of treatment and prevention. The presence of bacteria is one of the requirements for demineralisation to take place and it is unclear whether the type of bacteria in the plaque or the quantity of certain types of bacteria are significant influences on demineralisation. AIM: To identify any general changes in the bacterial composition of dental plaque in adolescents undergoing fixed orthodontic appliance therapy. A secondary aim is to determine the incidence of white spot lesion development and if this was related to the identified red fluorescent plaque. STUDY DESIGN: Prospective longitudinal cohort study. METHODS: Fourteen 11 to 23 year old consecutive patients attending the Orthodontic Department of Liverpool University Dental Hospital were recruited for this study. ToothcareTM and QLF were used to identify red fluorescent plaque and enamel demineralisation on the labial surfaces of the anterior teeth before and after the placement of fixed orthodontic appliances. The bacterial composition of the red fluorescent supragingival plaque was determined by DNA extraction, polymerase chain reaction amplification of the 16r rRNA gene and denaturing gradient gel electrophoresis. 13 RESULTS: The incidence of white spot lesions was recorded as 4.2% of the total surfaces of the teeth included as detected with QLF, the development of white spot lesions was not associated with the presence of red fluorescent plaque. There were differences in the bacterial composition of red fluorescent supragingival plaque in terms of P. gingivalis, S. mutans and S. gordonii between different participants, although changes in the plaque composition between visits for the same participant was not significantly associated with the development of white spot lesions in adolescents. CONCLUSIONS: With the limitations of this study, the placement of fixed orthodontic brackets in adolescents does not significantly change the bacterial composition of red fluorescent supragingival plaque around the brackets and that the development of white spot lesions cannot be correlated to red fluorescent plaque alone.

Book excerpt: Introduction: The purpose of this study was to use a chair-side saliva immunoassay to determine the overall prevalence of high Streptococcus mutans levels in orthodontic patients and to determine the prevalence of high S. mutans levels as a function of dental crowding and bracket type at four time-points throughout orthodontic treatment. Methods: 100 patients undergoing orthodontic treatment were selected, among which 35 used conventional brackets and 65 used self-ligated brackets. The sample population was comprised of 60 females and 40 males with overall mean age of 17.2 years. The chairside saliva assay Saliva-Check MUTANSTM was used to measure each subject at four time-points: immediately prior to bonding (T0), 3 months (T1), 6 months (T2) and 12 months (T3) into treatment. Bacteria levels as well as the amount of crowding were recorded at each time-point. Of the 400 anticipated data collection points, 8 were not recorded due to 6 patients being lost to follow-up. A repeated measures model was used to investigate the relationship between bracket type, crowding, and the risk of high bacteria levels. Specifically, a generalized linear mixed-effects model (GLMM) was used to account for the fact that the risk of high bacterial levels intrinsically varies between patients. A p-value ≤ 0.05 was significant. Results: The overall prevalence of high S. mutans levels was found to be 81% at T0, 78% at T1, 68% at T2 and 47% at T3. Bracket and crowding effects on bacteria levels were found to be non-significant (p>0.05). Only the effect of time was found iii to be significant, specifically, that T3 was different from T0, T1 and T2 (p0.0007). An incidental finding in our study showed males had a greater prevalence of high S. mutans throughout orthodontic treatment, however this gender effect was not statistically significant (p0.05). Conclusions: The Saliva-Check MUTANSTM assays' high sensitivity and specificity make it an effective tool to measure S. mutans levels and its use may facilitate further research in the field of oral health in general and the etiology of WSLs specifically.

However, these appliances can inadvertently accumulate plaque shortly after their application, which can lead to an imbalance in the bacterial population. This bacterial dysbiosis could increase the risk of several health issues, including dental caries, white spot lesions, periodontal diseases, and even metabolic diseases. This research investigates the changes in the gingival and plaque indices and the bacterial composition following the removal of orthodontic devices. Moreover, this work explores the ambiguity surrounding whether microbiological and clinical changes experienced by patients during orthodontic treatment are reversible post-therapy and compares the changes for fixed and clear aligner Orthodontic appliances Objective: To analyze the initial changes in clinical parameters and plaque levels of periodontal pathogens after orthodontic treatment with fixed or removable appliances. Materials and Methods: The subjects consisted of 14 patients completing orthodontic treatment ready for debonding. The Plaque Index and Gingival Index were measured as periodontal parameters. Both the plaque and gingival indexes were obtained from the central incisors, lateral incisors, canines, and first molars of both arches. Plaque and periodontal parameters were obtained at the following three-time points: at debonding (T0), 1 month (T1), and 3 months (T2) after debonding. Deep 16S rRNA sequencing was performed for the supra gingival plaques to analyze the correlation with the clinical indexes and to see the microbial community composition after debonding at 3-time points (T0, T1, and T2).
