Book excerpt: Post-exercise recovery strategies are commonly used with elite athletes to minimise fatigue experienced post-training. Sleep is widely considered to be the most important period for achieving optimal recovery post-exercise, however, sleep research in athletes is limited. One of the most popular post-exercise recovery techniques is cold water immersion (CWI). While there has been some research into the subsequent performance benefits (~60 min to 72 h) of post-exercise cold water immersion, there has been limited research into the physiological responses in the post-immersion (~90 min) period. As athletes often perform CWI in the evening post-exercise, research is needed to investigate the effects of CWI on subsequent sleep quality and quantity. The purpose of this thesis was to investigate the effect of evening high-intensity exercise, plus any effect of CWI performed post-exercise, on subsequent sleep quality and quantity. Specifically, study 1 investigated the core temperature and salivary melatonin responses to 15 min of either cold (14°C) or warm water immersion (WWI: 34°C) after a high-intensity cycling protocol in the evening. Study 2 investigated the effect of early evening high-intensity training sessions (over a training phase) on the sleep of elite youth soccer players in their normal sleeping environment using wrist actigraphy, by comparing high-intensity and low-intensity/no training nights. Additionally, as the players regularly engaged in CWI post-training (15 min in 14°C) it was possible to explore any effects of CWI on subsequent sleep. Study 3 combined elements of study 1 and study 2 together, by investigating the effect of evening highintensity cycling with and without the addition of CWI (15 min in 14°C) performed post-exercise compared to a control (no-exercise, no CWI) condition on the sleep of healthy male athletes. Sleep was measured using polysomnography (PSG), the gold standard of sleep measurement. Core temperature, skin temperature and salivary melatonin were also measured as these have all been linked to sleep mechanisms and there has been limited research surrounding their response to evening exercise and CWI. The results from these studies show that performing CWI post-exercise will decrease core temperature at a greater rate than both WWI and a control condition, with the effect lasting for ~2 h post-exercise. In addition, salivary melatonin is unaffected by both high-intensity exercise and CWI, even when controlling for light exposure, which usually inhibits the secretion of melatonin. However, most importantly, high-intensity exercise performed in the evening with or without the addition of CWI performed postexercise does not affect whole night sleep quality and quantity of athletes as measured by both actigraphy and PSG. Therefore, athletes and coaches can be confident that performing CWI post-exercise in the evening does not negatively affect the subsequent sleep of athletes.

Book excerpt: Victims of exertional heat stroke (EHS) in whom treatment is delayed have higher rates of multi-organ failure and a greater number of fatalities. Death related to EHS is preventable, through immediate treatment via cold-water immersion (CWI). To date little is known about the influence of treatment delays on core cooling following EHS. Thus we sought to examine the effects of treatment delays on cardiovascular and thermal responses prior to, during, and following CWI treatment in individuals with exercise-induced hyperthermia. Our findings demonstrate that treatment delays resulted in a sustained level of hyperthermia and cardiovascular strain that significantly increased the time an individual is at risk to the potential lethal effects of EHS. Moreover, we report that cold water immersion treatment is powerful enough to overcome the adverse effects of treatment delays and rapidly reduce core temperatures while facilitating the re-establishment of blood pressure towards normal resting levels.

Ten active, non-competitive male volunteers performed twenty minute treadmill exercise at equal relative intensities during two separate trials where: (1) gradual hypohydration took place, and (2) euhydration was induced by water ingestion in order to compensate for the body water volume loss demonstrated in the hypohydrated condition. Core temperature, represented by both esophageal and rectal temperatures, was recorded, along with surface temperatures (forearm, finger, chest thigh, and calf) and heart rate during a twenty-one minute adaptation period, throughout exercise and for forty minutes of recovery. Hematocrit samples were taken immediately prior to and following the exercise protocol and subject weights were measured before and after the testing session. The interaction between trials and time for esophageal and rectal temperatures and the difference between the hypohydration and euhydration trials for esophageal and rectal temperatures were all non-significant, and demonstrated that core temperature values did not differ between the hypohydrated and euhydrated conditions. However, core temperature was significantly different across time for both esophageal and rectal temperatures, due to a post-exercise elevated plateau from pre-exercise values. This elevation corresponded to the core temperature at which skin vessel dilation occurred during e exercise for both trials. The correlation between the difference in the hypohydration and euhydration esophageal elevations and the absolute and relative percentage weight loss of each subject after hypohydration further demonstrated a weak role for water intake in post-exercise hyperthermia. Several hypotheses are proposed to explain the post-exercise core temperature elevations in both the hypohydrated and euhydrated trials and the lack of significant difference between the two trials. Nine endurance trained males (35.9 ÷ 7.8 years old) completed 2 trials consisting of 8 repetitions of 1200 m at a running pace equal to 75% ofVO2peak. Design and Setting: A 2x2x5x5 crossover design with repeated measures on time and treatment directed data collection. The independent variables were gender, treatment, time (pretreatment, and post 15 seconds, 3 minutes 6 minutes and 9 minutes) and trial (5 trials for each time group). Response time (Tresp), reaction time (Treac), trial and surface temperature were measurement variables. Subjects: Thirty-six subjects, 18 females and 18 males were recruited from a physically active volunteer college student population. Measurements: Fractioned response time was tested following a 20 minute treatment. Response time and Treac were recorded by the reaction timer, and Tmov was calculated by taking the difference between Tresp and Treac. For each time/subject the high and low Tresp were discarded and the middle three trials were averaged and used for statistical analysis. A 2x2x5 ANOVA was used to determine overall differences between gender, treatment and time followed by Newman-Keuls multiple comparison tests. Results: Males were faster than females for Tresp, Treac and Tmov. Movement time and Tresp were slower with cold water immersion, but Treac was unaffected. Movement time and Tresp were fastest pretreatment, and slowest during the post 15-second time group. Though both Tmov and Tresp progressively sped up from the post 15-second through the post 9-minute time group, they did not return to pretreatment values when data collection discontinued. Conclusions: Immersing the dominant ankle in cold water for 20 minutes increases Tmov of the dominant lower limb; thereby increasing fractioned response time (Tresp).

During this grant period, the CMNR has met from three to six times each year in response to issues that are brought to the committee through the Military Nutrition and Biochemistry Division of the U.S. Army Research Institute of Environmental Medicine at Natick, Massachusetts, and the Military Operational Medicine Program of USAMRMC at Fort Detrick, Maryland. The CMNR has submitted five workshop reports (plus two preliminary reports), including one that is a joint project with the Subcommittee on Body Composition, Nutrition, and Health of Military Women; three letter reports, and one brief report, all with recommendations, to the Commander, U.S. Army Medical Research and Materiel Command, since September 1995 and has a brief report currently in preparation.
