GAO found that: (1) although the overall defense budget has declined, wasteful practices that inflate defense acquisition costs remain; (2) DOD continues to establish questionable requirements for its weapon systems, project unrealistic cost and performance estimates, develop duplicative weapons systems, and prematurely commit to production before completing testing; (3) DOD has supported efforts to eliminate overlapping and redundant weapon requirements and reduce high-risk acquisition strategies; (4) DOD has begun to reassess many of its most expense weapon programs to determine which systems should be terminated, reduced, or delayed; (5) Congress has established an independent commission to reevaluate the military services' roles and missions and examine whether the DOD acquisition structure is too complex; (6) DOD initiatives to improve its weapons acquisition process cannot be assessed because they are in various stages of implementation; and (7) DOD believes that substantive reforms in its acquisition process can be achieved due to fiscal constraints, reduced threats, dwindling forces, congressional support, and DOD commitment to reform.

Book excerpt: Experience has shown that new weapon systems regularly encounter great difficulties as they begin production, such as a high percentage of components that must be scrapped or reworked. These give rise to significant cost increases and schedule delays. Through an examination of six weapon system case studies, GAO found that such problems occur in programs where efforts to prepare weapons for production were insufficient. The six programs reviewed were the Army's Copperhead projectile and Black Hawk helicopter, the Navy's High Speed Anti-Radiation Missile (HARM) and Tomahawk cruise missile, and the Air Force's F-16 fighter and Air-Launched Cruise Missile (ALCM). GAO found further that technical performance concerns, program management and staff, and funding and quantity instability greatly influenced these preparations during the weapon's development. DOD has issued two directives which should improve production preparations in future programs. GAO makes recommendations on applying these directives to individual programs, to both improve production preparations and reduce the effects of other program influences on such preparations.

Although some yet-to-be deployed systems designed in the mid-1970s are likely to exhibit many of the same problems occurring in the high performance weapons deployed today, recently developed systems should benefit from emphasis on reliability, availability, and maintainability, therefore presenting a brighter future. However, a reappraisal of some may be in order. As shown by recent directives, more attention should be paid in the early design of weapons to the best mix of high performance and support characteristics, considering expected force resources and operations. Defense's emphasis on lower cost weapon systems and greater reliability, although well placed, does not appear to have been sufficient. While established inventory objectives for new weapons are high, rapidly rising unit costs make it unlikely that they can be achieved without major increases in or realignment of the defense budget.

Book excerpt: The Department of Defense (DOD) has repeatedly delivered the most capable weapon systems in the world, but with consistent schedule delays and at significant cost to taxpayers. The process used to manage the acquisition of these systems has been characterized by organizations both internal and external to DOD as one that is inefficient, cumbersome, and bureaucratic.1 A contributing factor to this inefficient process is the significant time and effort required to complete information requirements before an acquisition program can proceed through a milestone to the next phase in the weapon system acquisition process. DOD leadership has acknowledged that too much time is invested in preparing for key milestones, including the documentation and oversight of information required by statutes and policy,2 which takes time away from conducting day-to-day core program management tasks such as contractor oversight, engineering, and risk management. There is a natural tension between oversight and accountability on one hand and efficient program management on the other. Oversight and accountability add process, which is justifiable given the high cost and risk associated with major weapons and the inherent incentives for program managers and contractors to be optimistic about what they can accomplish. The additional process does come at a cost of time and other resources that a program manager could potentially employ better. If it were clearly demonstrable that program cost and schedule outcomes have improved as oversight has intensified, then the additional process would be easy to accept. But, as we have reported over the years, program outcomes have not significantly improved. Thus, it is an appropriate time to assess the value of the documents and reviews demanded of today's acquisition process. We are not suggesting that individual actions to improve oversight were not justifiable, but rather questioning whether their cumulative demand is in balance with their desired effects. According to federal internal control standards, agencies should develop effective and efficient processes to ensure that actions are taken to address requirements, such as in this case, completing the information required to aid in milestone decisions.3 DOD has embarked on some initiatives aimed at streamlining different aspects of its weapon system acquisition process, but acknowledges more needs to be done. In the National Defense Authorization Act for Fiscal Year 2014, Congress mandated that GAO review DOD's weapon systems acquisition process, with an objective of identifying processes or procedures with little or no value added.4 This report examines (1) the effort and value involved in DOD's preparation for a milestone decision, (2) the factors that influence the time needed to complete the milestone decision process, and (3) alternative processes used by some DOD programs and leading commercial firms.
