Book excerpt: Cemented Total Hip Arthroplasty (THA) remains one of the most successful procedures in Orthopaedic surgery. It has become very clear that it is the surgical expertise, in particular the quality of the cementing technique, which will affect long-term outcome and success. It is the intention of this book to provide an up-to-date comprehensive assessment of the entire field of cemented THA. Special emphasis has been given to practice-relevant aspects: well-illustrated and detailed operative steps as a practical guideline, a basic science chapter and long-term outcome data are provided. Minimally invasive surgery, modern perioperative management and patient fast tracking are covered. A number of highly respected experts have contributed to this in-depth compilation of the "state of the art" in 2005. This book is written and intended for both, trainees and established arthroplasty surgeons who are dedicated to perform a well-cemented THA.

Available in PDF, EPUB and Kindle. Book excerpt: The theme of this work is the application of the engmeermg theory of frictional torque to total hip replacement. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.

Available in PDF, EPUB and Kindle. Book excerpt: Renowned philosopher and cultural theorist Kristeva (Powers of Horror: An Essay on Abjection) offers an extended consideration of artistic figurations of the severed head, the organizing theme to an exhibition she coordinated at the Louvre in 1998. Though she follows a single historical trajectory, moving from Paleolithic skull cults to antique Greek sculpture to the Surrealist drawings, Kristeva eschews the disciplinary constraints of art history, instead employing psychoanalysis to explore the intertwined problems of representation and mortality posed by the severed head. For Kristeva, the capacity to figure the life of the mind first requires a confrontation with this horrific object that stands at the boundary between life and death, registering not only the loss of corporeal form but also subjective interiority. Though this book does not engage with recent images of decapitation, it is not without contemporary political-cultural import; for Kristeva, these cruel artistic figurations offer us the capacity to contemplate the sacred within a technology-driven contemporary visual culture. Verdict While a challenging text, this beautifully written and richly layered meditation on mortality and representation will undoubtedly appeal to those readers interested in semiotic and psychoanalytically informed readings of art.-Jonathan Patkowski, CUNY Graduate Ctr.(c) Copyright 2012. Available in PDF, EPUB and Kindle. Book excerpt: Joint replacement surgery has seen remarkable progress and development in recent years both in prostheses and in surgical technique. A prime concern has been improved durability, for which a major factor is reduction of the polyethylene wear that leads to osteolysis. This book presents an update on the means by which the problems of wear and loosening are being addressed in total hip arthroplasty (THA) and total knee arthroplasty (TKA). Included are chapters on new surgical techniques for difficult cases, nonpolyethylene interfaces for THA, custom hip prostheses, and computer-assisted surgery. Arthroplasty 2000 also takes up current controversial issues such as posterior cruciate substitution versus retention in TKA. With contributions by eminent specialists in total joint replacement in Asia, Europe, and North America, this volume is a valuable reference for all orthopedic surgeons.

Book excerpt: This book presents analyses of the most commonly reported failure modes of hip stems: loosening and thigh pain; both are attributed to the relative motion and instability at the bone-implant interface due to failure to achieve sufficient primary fixation. The book investigates various factors that could affect primary stability and therefore the long-term outcome of hip arthroplasty. The results complement experimental work carried out in this area as in-vitro experiments have several limitations that could be addressed through computer simulations.

Book excerpt: Utilizing detailed case presentations from experts in the field, this book presents common clinical scenarios involving patients who have sustained displaced femoral neck fractures, discussing the injury mechanism, early treatment, potential treatment options, risks and complications involved. The style of presentation, the ease of readability, the use of appropriate and relevant cases to highlight the learning points, and the discussion of the decision-making process are all unique features of the book's approach. Using this tailored process, the each topic is reviewed and the rationale for what appears to be the optimal treatment - hemiarthroplasty or total hip replacement, for example - is provided to the reader, emphasizing that the ultimate decision requires the combination of science and judgment, both of which are continually evolving. Additional chapters present ongoing controversies in the management of these challenging fractures, as well as complications such as aseptic necrosis, symptomatic hardware and nonunions. Portable and concise, orthopedic and trauma surgeons, residents and fellows alike will find Displaced Femoral Neck Fractures a practical and user-friendly resource.
