Book excerpt: In recent years, the increase in knowledge about the functioning of the immune system has revealed not only its importance in the defense against external agents such as pathogens or toxins, but also in the control of tumor cells and the importance of the processes of inflammation or immunological tolerance. On the one hand, all this knowledge has allowed a better understanding of the putative pathogenic consequences of immune system dysfunction, which includes inflammatory, autoimmune and immunosuppressive diseases, among others. On the other hand, current knowledge about immunoregulation has paved the way to better prevent or control transplantation rejection. However, such mechanisms underlying immune dysregulation are highly variable depending on the type of pathology (systemic chronic inflammatory diseases, autoinflammatory diseases, autoimmune disorders, immunosuppression) and on characteristics of the host such as sex, genetics, nutritional status, etc. Given the wide variety of pathologies that are a consequence of excessive, inefficient or inadequate induction of immune responses, the study of factors involved in the dysregulation of the immune system has gained great attention during the last decades.

Phillip Allen Sharp is an American geneticist and molecular biologist who co-discovered RNA splicing. He shared the 1993 Nobel Prize in Physiology or Medicine with Richard J. Roberts for “the discovery that genes in eukaryotes are not contiguous strings but contain introns, and that the splicing of messenger RNA to delete those introns can occur in different ways, yielding different proteins from the same DNA sequence. He works in Institute Professor Koch Institute for Integrative Cancer Research, Massachusetts Institute of Technology (MIT), Cambridge, MA, US. La ciencia primitiva de la química y la microbiología fue la fuente de la mayoría de las drogas hasta la revolución de la ingeniería genética a mediados de la década de 1970. Luego, la biotecnología puso a disposición agentes proteínicos novedosos como interferones, factores sanguíneos y anticuerpos monoclonales que han cambiado la farmacia moderna. Durante el año pasado, surgió una nueva farmacia de oligonucleótidos a partir de la ciencia de la expresión génica, como el empalme de ARN y la interferencia de ARN. Con este pensamiento, he intentado escribir un libro de texto conciso, para médicos del trabajo, estudiantes de medicina y otros profesionales de la salud, que pueda explicar los conceptos básicos de la epidemiología de manera clara y sencilla. He tratado de suprimir la brecha existente en la comunicación entre el Médico del Trabajo como clínico y el epidemiólogo, describiendo algunos ejemplos clínicos a través del libro, explicando al Médico del Trabajo, por que es necesario el énfasis epidemiológico sobre el estudio de grupos de trabajadores, mas que de individuos.
