Book excerpt: Excerpt from Syllabus of the Obstetrical Lectures in the Medical Department of the University of Pennsylvania With the approval of Professor Hirst this syllabus has been pre pared to meet the difficulty of accurate note-taking, which most medical students encounter. The subject matter has been so arranged that uninterrupted attendance upon lectures is essential to a full knowledge of the course. The design of the book, therefore, is to secure for the student a logical and consecutive outline of his work, and to aid him in classifying the knowledge he acquires in the lecture room. The author desires to express his indebtedness to Professor Hirst for many suggestions and his kindly interest in the preparation of the work, and to indulge the hope that the medical class may find the book of some service to them. Book excerpt: Excerpt from A Lecture, Introductory to a Course on Obstetrics and the Diseases of Women and Children: Delivered November 5, 1848 Allow me to ofi'er my salutations, and extend to you all a cor dial and earnest welcome to the halls of this University. You have assembled for the purposes of instruction and self-improvement and you have visited this school of medicine in the full hope that the prin ciples of your profession will be properly inculcated, and its great dogmas, as transmitted to us by the venerable and learned fathers of our science, faithfully expounded. These are duties which our posi tion necessarily calls upon us to discharge - duties too sacred to be re. Garded lightly, for they involve consequences momentous not only to you, but to those, also, who are hereafter to be confided to your profes. Sional care. In this school, then, you are to learn the principles which are to guide you in the treatment of disease and may they, in the hour of peril, when all around is gloom and darkness, prove beacon-lights to illumine your way to truth and science; may they, under those try ing circumstances, redeem the pledge which we make to you this night, to spare no effort, shrink from no labor necessary for the zeal ous and faithful performance of the trust imposed on us as your in structors. To the Faculty of this Institution has been delegated the high privilege of preparing you for the emergencies of professional life; soon you will be called upon to exhibit, through your acts, testi mony of the fidelity with which you have pursued your studies - testi mony, too, ofthe manner in which we, your teachers, have discharged our duty. These are considerations which I know full well no mem ber of this Faculty can contemplate without appreciating, to the full extent, the fearful responsibility of his position; and whatever may be the feelings of my colleagues on this subject - I know they are deep and abiding - allow me to say, for myself personally, that I never enter on the performance of duty in this University, without a senti. Ment of apprehension and distrust. It will, however, be my earnest care to consecrate to your service every energy of my mind; and feeling, as I do, an undisguised interest in your advancement, you may rely on my best efforts to impress upon you the principles and practice of the department assigned to me in this Institution. Book excerpt: Excerpt from Clinical Lectures 0n Subjects Connected With Medicine, Surgery, Obstetrics But, on the other hand, there are numerous acute febrile diseases in which splenic tumours develope to a size far beyond that of those just referred to, and increase to such an extent as to be easily made out by percussion, or even project so much below the ribs as to be clearly recognised on palpation. For such cases as these the assumption of simple fluxion, or the fever heat of the blood, as exciting, causes does not appear sufficient explanation, and we are compelled to seek for other causes to account for such enormous hyperplastic proliferation of the pulpous elements of the spleen. In all acute diseases whose origin is cer tainly due to infection (typhus, septicaemia, intermittent fever, the constant occurrence Of large splenic tumours can be demonstrated both clinically and pathologically, and hence we are compelled to recognise foreign substances infecting the blood as special irritants of the splenic pulp. It follows, therefore, . That the existence of a considerable enlargement of the spleen positively justifies us in forming a decided opinion as to the infectious nature of the malady, and thus, with regard to 'the origination of doubtful forms, of disease, a step will be gained in the still extremely Obscure domain of etiology. The readiness with which the spleen reacts to certain substances of an injurious nature, present in the blood, is explained not only by the copiousnress of its blood supply, but also by its anatomical peculiarities. We know that the arteries in the interior Of the spleen break up into very numerous, extremely fine branches and capillaries, perforated by stomata; that the blood passes from the vessels into wide spaces destitute of walls (intermediary blood canals), from which the veins begin to arise, in like manner, principally as cribriform canals. The blood flows with extreme slowness through these wide intermediary canals, and both here and in the cribriform perforations Of the capillaries, is in. Intimate connection with the lymphoid cells so susceptible to irritation, and the delicate, fibrous network of the pulp. Hence it follows that matters Of an injurious nature infecting the blood are extremely liable to be retained in the spleen, to accumulate therein, and to act with peculiar intensity as excitants to the splenic tissue. Book excerpt: Excerpt from The Principles and Practice of Obstetrics Although only two years have elapsed since this treatise was first presented to the profession, for the second edition it has been subjected to a most pains taking revision, and every chapter has been brought up to the date of going to press. Considerable new matter and numerous new illustrations have been added, but, though the book was already large, it was felt. That nothing could be omitted. By using thinner paper the volume was actually reduced. The chapters on the Abderhalden pregnancy reaction, on twilight sleep, on dry labor, labor in old primiparae, blood-pressure, and extraperitoneal cesa rean section were much enlarged. Book excerpt: Excerpt from The Principles and Practice of Obstetrics: Illustrated With One Hundred and Fifty-Nine Lithographic Figures From Original Photographs, and With Numerous Wood-Cuts The study of the medical sciences, thus inaugurated in America, in two of its most impor tant cities, was completely interrupted by the Revolutionary war. At the end of this conflict, medical instruction was resumed in Philadelphia. About the same time Harvard University, at Cambridge, Massachusetts, which had been founded in 1638, instituted a Medical Department, with three professorships. Dr. John Warren, who had for three years been a private lecturer upon anatomy and surgery, was appointed professor of these branches in 1783. In the arrangement of the duties of the professors, it does not appear that midwifery received any Special attention, and this omission seems to have continued until 1815, when Dr. Walter Channing was appointed Lec turer upon Obstetrics and Medical Jurisprudence. For a series of years this gentleman greatly contributed, by his personal and professional character, to elevate the science Of Obstetrics in public Opinion. He was among the first, in this country, to authorize the employment Of anaesthesia during labor. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. It has also permitted me to set forth my personal views on eclampsia, which I have done in the Appendix, by publishing a reprint of my paper read before the Obstetrical Section of the British Medical Association, 1911. The Appendix also contains observations by two of my former Assistant Masters on scopolamine-morphine in labour; and in addition a detailed account of the treatment of acute sepsis by vaccines. Book excerpt: Excerpt from A Manual of Obstetrics The Chief purpose of this book is to present, in an easily intelligible form, such an outline of the rudiments and essen tials of Obstetric Science as may constitute a good ground work for the studentat the beginning of his Obstetric studies; and one by which it is hoped he will be the better prepared to understand and assimilate the extensive knowledge and classical descriptions contained in larger and more elaborate text-books. Confessedly, in great part, a compilation from these, it is upon the more recent treatises Of Leishman, Playfair, and Lusk that I have most largely depended, as authorities, in dealing with matters that are still unsettled, and it is with pleasure I acknowledge my indebtedness to these authors.
