Book excerpt: The molecular basis for atrial fibrillation continues to be largely unknown, and therapy remains unchanged, aimed at controlling the heart rate and preventing systemic emboli with anticoagulation. Familial atrial fibrillation is more common than previously suspected. While atrial fibrillation is commonly associated with acquired heart disease, a significant proportion of individuals have early onset without other forms of heart disease, referred to as "lone" atrial fibrillators. It is also well recognized that atrial fibrillation occurs on a reversible or functional basis, without associated structural heart disease, such as with hyperthyroidism or of atrial fibrillation following surgery. It remains to be determined what percentage in these individuals is familial or due to a genetic predisposition. Mapping the locus for familial atrial fibrillation is the first step towards the identification of the gene. Isolation of the gene and subsequent identification of the responsible molecular genetic defect should provide a point of entry into the mechanism responsible for the familial form and the common acquired forms of the disease and eventually provide more effective therapy. We know that the ionic currents responsible for the action potential of the atrium is due to multiple channel proteins as is electrical conduction throughout the atria. Analogous to the ongoing genetic studies in patients with familial long QT syndrome, it is highly likely that defects in each of these channel proteins will be manifested in familial atrial fibrillation.

Book excerpt: The ESC Textbook of Intensive and Acute Cardiovascular Care is the official textbook of the Acute Cardiovascular Care Association (ACVC) of the ESC. Cardiovascular diseases (CVDs) are a major cause of premature death worldwide and a cause of loss of disability-adjusted life years. For most types of CVD early diagnosis and intervention are independent drivers of patient outcome. Clinicians must be properly trained and centres appropriately equipped in order to deal with these critically ill cardiac patients. This new updated edition of the textbook continues to comprehensively approach all the different issues relating to intensive and acute cardiovascular care and addresses all those involved in intensive and acute cardiac care, not only cardiologists but also critical care specialists, emergency physicians and healthcare professionals. The chapters cover the various acute cardiovascular diseases that need high quality intensive treatment as well as organisational issues, cooperation among professionals, and interaction with other specialities in medicine. SECTION 1 focusses on the definition, structure, organisation and function of ICCU's, ethical issues and quality of care. SECTION 2 addresses the pre-hospital and immediate in-hospital (ED) emergency cardiac care. SECTIONS 3-5 discuss patient monitoring, diagnosis and specific procedures. Acute coronary syndromes (ACS), acute decompensated heart failure (ADHF), and serious arrhythmias form SECTIONS 6-8. The main other cardiovascular acute conditions are grouped in SECTION 9. Finally SECTION 10 is dedicated to the many concomitant acute non-cardiovascular conditions that contribute to the patients' case mix in ICCU. This edition includes new chapters such as low cardiac output states and cardiogenic shock, and pacemaker and ICDs: troubleshooting and chapters have been extensively revised. Purchasers of the print edition will also receive an access code to access the online version of the textbook which includes additional figures, tables, and videos to better to better illustrate diagnostic and therapeutic techniques and procedures in IACC. The third edition of the ESC Textbook of Intensive and Acute Cardiovascular Care will establish a common basis of knowledge and a uniform and improved quality of care across the field.

Book excerpt: From senior electrophysiologist and world-class educator George Klein, a fully illustrated guide with over 100 intracardiac tracings and figures that allow the physician to approach electrophysiologic problems effectively and systematically. The book is especially focused on electrophysiological maneuvers and provides a clear and understandable guide to their proper selection and interpretation using abundant clinical examples. Defines the integral role for "traditional" electrogram (EGM) analysis in order to understand the mechanism of a tachycardia. It goes without saying that a correct arrhythmia diagnosis is a prerequisite to catheter ablation regardless of the presence of sophisticated mapping and imaging technologies. Electrophysiological maneuvers are fundamental to this process, and proper selection and interpretation of maneuvers constitute a core skill of the electrophysiologist. In this volume, we make the case that most maneuvers are fundamentally similar in principle and can be understood by appreciating a few basic physiological and anatomical principles. The art lies not in a comprehensive knowledge by rote of every maneuver or its application, but rather a systematic approach using common principles. We illustrate this by showing abundant examples and emphasizing the "game plan," including checklists that can be applied to virtually any maneuver. —George J. Klein In my opinion, this book should be on the shelf of every electrophysiologist trainee as well as every clinical cardiac electrophysiologist. It is a classic, like its editor. Dr. Klein deserves high praise for organizing his and his colleagues’ clinical experiences and thought processes into a concise, practical text that should be part of all training programs in electrophysiology. —From the foreword by Mark E. How can we feel another's pain, especially when pain can be assumed, distorted, or performed? Is empathy a tool by which to test or even grade each other? By confronting pain—real and imagined, her own and others'—Jamison uncovers a personal and cultural urgency to feel. She draws from her own experiences of illness and bodily injury to engage in an exploration that extends far beyond her life, spanning wide-ranging territory—from poverty tourism to phantom diseases, street violence to reality television, illness to incarceration—in its search for a kind of sight shaped by humility and grace.

Available in PDF, EPUB and Kindle. Book excerpt: Although the electrocardiogram (ECG) is accepted as a standard clinical tool, electrograms (EGMs) recorded during electrophysiology studies are considered complex and confusing. In this brief paperback, an award-winning teacher provides the newcomer with an introductory guide to electrophysiology studies and the interpretation of electrograms. Dr. Kusumoto divides the 15 chapters into two main sections, Electrophysiology Concepts and Specific Arrhythmias. First, he reviews the basics of electrophysiology testing and the diagnostic evaluation of general types of arrhythmias. From that foundation, he proceeds to discuss specific arrhythmia types and techniques for evaluation and ablation: • Accessory pathways • AV node reentry • Focal atrial tachycardia • Atrial flutter • Atrial fibrillation • Ventricular tachycardia The final chapter considers implantable cardiac devices as they relate to ECGs and electrograms. Each short chapter includes a bullet-point summary and helpful review questions. Plentiful ECG and EGM tracings illustrate the text. Cardiology and electrophysiology fellows, allied professionals working in the electrophysiology laboratory, and all professionals interested in beginning a study of heart rhythms and electrophysiology will want to keep Understanding Intracardiac EGMs and ECGs close at hand for frequent reference.
