Available in PDF, EPUB and Kindle. Book excerpt: An analysis of the cultural and economic drivers of the growing phenomenon of FGCS, written by cross-disciplinary experts, this book challenges the concept of individual consumer choice in FGCS: a decision that is rarely exercised in a socio-cultural vacuum. Four distinct aspects of FGCS are covered: variations in female genital anatomy; surgical techniques and evidence; historical contexts and ethical dilemmas; norm-critical understandings to inform professional responses. Rendering philosophical critiques accessible, and exposing dubious social values that underpin the practice, this text is crucial in driving a broader understanding of FGCS as a cultural phenomenon of our times. Only with a fuller understanding of the multiple perspectives of FGCS, can there be sensible alternatives for women and girls psychologically troubled by their natural, healthy form. Offering explanations and interventions at individual, institutional and societal levels, this text will be valued by both professional and non-professional audiences.

Book excerpt: In recent years advances in laparoscopic technologies have led to renewed interest in the vaginal approach to hysterectomy, which has many proven benefits for patients. This volume, dedicated to explaining and promoting the vaginal route of hysterectomy, is written and edited by an international team of experts and provides a much-needed source of up-to-date information and instruction. Importantly, the authors caution that laparoscopic technology can provide a valuable source of assistance for the gynaecological surgeon in certain circumstances, though only in a percentage of cases. This book, beautifully illustrated with line drawings and full-colour photographs, contains step-by-step surgical techniques, enabling the surgeon to gain confidence and experience so that gradually more challenging operations can be managed successfully via the vaginal route. Acknowledged authorities from around the world take the reader through the indications and contra-indications for the vaginal approach, explain crucial preoperative assessment procedures, and offer an evidence-based elucidation of the 'why', 'when' and 'how' of vaginal hysterectomy. Specific topics considered here include: the nulliparous patient, uterine fiboids, debulking, the use of gonadtrophin-releasing hormone agonists, oophorectomy and prophylactic oophorectomy, adnexectomy for adnexal pathology, genital prolapse, the place of sacrospinous colpopexy, urethral sphincter incompetence, hormone replacement therapy, and the psychological and sexual outcomes of hysterectomy. In addition, the authors set forth the arguments for and against vaginal hysterectomy, abdominal hysterectomy, laparoscopic assistance, and transcervical resection of the endometrium. Combined with a full review of the potential complications, morbidity and mortality associated with the vaginal approach, this book provides the reader with a well-balanced, thorough and considered appraisal of vaginal hysterectomy. As the incidence of hysterectomy worldwide continues to rise, Vaginal Hysterectomy will be an indespensable reference for practising gynaecologists, surgeons, consultants and postgraduates.

Available in PDF, EPUB and Kindle. Book excerpt: In the West, a specific ideal for female genitalia has emerged: one of absence, a "clean slit," attained through the removal of pubic hair and, increasingly, through female genital cosmetic surgery known as FGCS. In The Perfect Vagina: Cosmetic Surgery in the Twenty-First Century, Lindy McDougall provides an ethnographic account of women who choose FGCS in Australia and the physicians who perform these procedures, both in Australia and globally, while also examining the environment in which surgeons and women come together. Physicians have a vested interest in establishing this surgery as valid medical intervention, despite majority medical opinion explicitly acknowledging that a wide range of genital variation is normal. McDougall offers a nuanced picture of why and how these procedures are performed and draws parallels between FGCS and anthropological discussions of female genital circumcision (cutting). Using the neologism biomagical, she argues that cosmetic surgery functions as both ritual and sacrifice due to its promise of transformation while simultaneously submitting the body to the risks and pain of surgery, thus exposing biomedicine as an increasingly cultural and commercial pursuit. The Perfect Vagina highlights the complexities involved with FGCS, its role in Western beauty culture, and the creation and control of body image in countries where self-care is valorized and medicine is increasingly harnessed for enhancement as well as health.
