Available in PDF, EPUB and Kindle. Book excerpt: The impetus for this book came from the recent appearance of single specialty books pertaining to reoperative surgery on various organs in the pelvis, as well as from the recognition that several different disciplines are involved with the challenges of reoperative pelvic surgery. Surgeons often encounter challenging dilemmas involving organ systems that have historically been attended to by surgeonsrepresentingcloselyrelatedbut distinctspecialtyareas. Withincreasing sophisticationand knowledge about management of anatomically adjacent organs by the specialties of gynecologic oncology, gynecology, urology, and colon and rectal surgery, as well as the emergence of specialty training programs in urogynecology and pelvic floor disorders, we thought it appropriate and timely to create a textbook acknowledging this increasing knowledge and interspecialty collaboration. To this end, where appropriate, we haveincluded collaborative authors fromeach of the specialties, any ofwhommaybecalledupontoaddressaparticularanatomicarea. Itseemsinevitablethatsituations willariseinwhichthecollaborativeexpertiseofseveralseparatespecialtiesmayconvergetoprovide surgeons the benefit of the combined thought processes that would prove invaluable when such difficult problems are encountered. With this in mind, the editors, from the fields of gynecologic oncology, urology, and colon and rectal surgery, identified experts in theirown fields who could bestcontribute to the management of specific problem areas. For example, since reoperations for endometriosis may involve uterus, adnexae, ovaries, or the colorectum, the chapter concerning this condition has been coauthored by specialists in colorectal surgery and gynecologic oncology. We have been fortunate to find experts who have collaborated to bring available evidence-based medicine, best demonstrated practices, and personal experience to their contributions.

Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Surgery of the Rectum: And Pelvis In preparing the following pages for publication, I have endeavored to condense into convenient form, for both student and practitioner, as great an amount as possible of practical information concerning diseases of the rectum and anus. The advances which have been made during the past few years In this special branch of surgery have been very great. The whole pathology of malignant disease has been rewritten; and the close relationship of the so-called benign polypoid growths to epithelial cancer has been worked out after careful study with the microscope. The operation of excision of cancerous growths in the lower part of the rectum has again become a legitimate surgical procedure, and what is better, its range of applicability has been definitely determined. The advances in abdominal surgery, for which the present century will always be famous, have also had a bearing on diseases of the rectum, and the operation of excision of cancer of the upper part of the rectum and the sigmoid flexure, through an incision made in the abdominal wall, has brought within treatment a class of cases formerly beyond the reach of art. New methods of treatment of benign stricture have been devised as a substitute for colotomy. New methods of palliation in benign and malignant stricture have been devised as substitutes for the repulsive operation of colotomy, and that operation bids fair in the immediate future to assume deservedly a place of less prominence than it has occupied in the past. In each chapter, international experts address how to avoid being in tough surgical situations through preoperative planning, how to better deal with commonly encountered intra-operative findings, how to deal with difficult laparoscopic, open, endoscopic, and anorectal cases, and how to avoid medico-legal issues. Colorectal Surgery Consultation is simple and succinct and provides pragmatic advice and reproducible techniques that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems through endoscopic and endoluminal approaches that may make the difference in patient outcomes.
