Worldwide, about one-third of adolescents report lifetime suicidal thoughts, one in ten adolescents have attempted suicide and almost 46,000 adolescents end their own lives every year. In the Netherlands, one adolescent dies by suicide every week. Adolescents with suicidal behavior also have a higher risk of problems later in life. Therefore, early screening and prevention of suicidal behavior is important. Although much epidemiological evidence on suicidal behavior is available, there are still gaps in our understanding of suicidal behavior in adolescents. The general aim of the research presented in this dissertation was therefore to contribute to a better understanding of suicidal behavior among adolescents (aged 13-16 years) in Amsterdam and vicinity. We used data from the school-based screening questionnaires and from the Digital Child Health Care Registry, which is a registration system for the outcomes of health assessments. Both of these sources are part of the routine health assessments of the Public Health Service (GGD) of Amsterdam. We considered the data to be representative of all adolescents 13-16 years old living in Amsterdam and vicinity. Part I of this dissertation describes time trends for emotional and behavioral problems and suicidal behavior. An important finding was that sociodemographic risk groups for emotional and behavioral problems and suicidal behavior changed over time. The prevalence of total difficulties in adolescents increased by 19% between school years 2004-2005 and 2013-2014 and the time trends differed by sex. The prevalence of suicidal thoughts decreased between 2010-2011 and 2014-2015 from 18% to 13% and of suicide attempts from 3% to 2%. Time trends differed between subgroups according to ethnicity and educational level. Part II of this dissertation describes possible risk factors of suicidal behavior during adolescence.

It has been accompanied by substantial changes in the leading methods of youth suicide, especially among young girls. Much work is underway to elucidate the relationships between psychopathology, substance use, child abuse, bullying, internet use, and youth suicidal behaviour. Recent evidence also suggests sex-specific and moderating roles of gender in influencing risk for suicide and suicidal behaviour. Suicide is the third leading cause of death among adolescents globally, accounting for 11% of all deaths to youth ages 12 to 19 between 1999 and 2006, and rates of attempting suicide and suicidal ideation are higher than those of completed suicide. According to the Centers for Disease Control and Prevention, 157,000 youth between the ages of 10 and 24 receive medical care at emergency departments for self-inflicted injuries each year. Data from the 2011 National Youth Risk Behavior Survey show that 16% of youth reported seriously considering suicide, 13% reported creating a plan, and 8% reported trying to take their own lives in the 12 months preceding the survey. The bulk of existing research on adolescent suicide focuses on psychological explanations and individual-level risk factors for suicide, including mental health, substance use patterns, and exposure to traumatic life events, such as sexual abuse. This work is critical because it identifies individuals at the greatest risk of suicide and provides clear intervention strategies to address individuals’ unique paths to suicidal ideation.

As the second leading cause of death for young people in Canada between the ages of 15 and 24, nearly one-quarter of all annual deaths for this age group are accounted for by those who have died by suicide. Greater still is the prevalence of suicidal behaviour. Annually, suicide and suicidal behaviours across the age span are estimated to cost the Canadian economy $2.2 billion. Given the high personal costs to victims, survivors, and taxpayers, it seems crucial that prevention strategies be implemented to reduce the high rate of suicide and suicidal behaviours, as well as to promote mental health, particularly in youth. The promotion of positive mental health and well-being is especially relevant for youth living in rural areas. Fewer mental health resources are available in rural communities than in urban centres. Moreover, youth living in rural, low population density areas often do not believe that their anonymity and right to privacy would be protected if they were to seek mental health services. The present research was carried out to provide a potential foundation for stimulating mental health promotion and suicide prevention approaches, differing by rural and urban community and by gender, before such concerns emerge. Yet there is limited research on suicidal behaviors and other non-fatal self-harming behaviors such as non-suicidal self-injury (NSSI) among Indian youth. In the Indian cultural context, knowledge of local epidemiology, especially the identified risk factors, is vital to the formulation of effective strategies to prevent suicidal behaviors and NSSI. The present research therefore aimed to investigate the prevalence and psychosocial correlates of NSSI and suicidal behaviors in a community-based sample of college youth in India. Guided by social-ecological perspectives, the premise of this research was that risk pathways for these behaviors emerge via interactions between individual characteristics and environmental factors. The research reported in this dissertation comprised two separate studies, each of which followed a distinct line of inquiry but collectively contribute to a cultural understanding of NSSI and suicidal behaviors. Study 1 was focused primarily on NSSI while Study 2 focused on suicidal behaviors. Data for both studies was collected from 1,817 undergraduate students (1,017 female, 793 male) in the western state of Gujarat. Study 1 sought to investigate the prevalence and psychosocial correlates of NSSI. A second objective was to identify risk factors for suicide ideation among those with NSSI history. Results showed that about 7.8% of the sample reported engaging in NSSI. Logistic regression analysis revealed that risk for NSSI was significantly associated with higher income, living alone, family-related stress and psychological distress. Among those with a history of NSSI, the risk for suicide ideation was significantly increased by female gender, economic stress, and psychological distress, but not NSSI severity. Study 2 sought to identify psychosocial risk factors for suicidal behaviors (ideation and attempt). Recent research indicates that in the context of a rapidly globalizing culture, one emerging concern is the rise of a consumerist culture that promotes materialistic aspirations. Studies have found that individuals who focus on materialistic or extrinsic aspirations at the cost of intrinsically satisfying ones report poorer mental health. Therefore a second objective of Study 2 was to examine whether extrinsically or intrinsically motivated aspirations moderated the relationship between psychosocial risk factors and suicidal behaviors. Results showed that about 12.7% of participants reported a lifetime history of suicidal behaviors. Results of logistic regression analyses revealed that economic stress, social stress and psychological distress predicted suicidal behaviors. When the moderating role of aspirations was tested, results showed that placing greater importance on extrinsic aspirations increased the risk of suicidal behavior in the presence of social stress while valuing intrinsic aspirations buffered against risk. Findings draw attention to the social pressures faced by youth and the role of inherently satisfying goal pursuits in buffering the adverse effects of social pressures. Taken together, results from this research indicate that risk factors for NSSI and suicidal behaviors span individual, familial, societal and wider cultural levels, underscoring the need for multipronged prevention and intervention strategies that are aligned with these multiple contexts. Culturally meaningful approaches to help students at risk for suicidal behavior and NSSI may be developed at both the individual as well as institutional level, consistent with a social-ecological model for suicide prevention.
